Diagnostic Performance Of Waist-To-Height Ratio In Identifying Cardiovascular Risk Factors And Metabolic Syndrome Among Adult Saudis: A Cross-Sectional Study, Rasmieh Alzeidan, Amel Fayed, Fatemeh Rabiee, Ahmad Hersi, Hala Elmorshedy
Saudi Medical Journal
Objectives: To evaluate the diagnostic performance of waist to height ratio (WHtR) to screen for cardiovascular risk factors (CVRF) and metabolic syndrome (MetS) among Saudis. Methods: Between June 2013 and August 2014, a cross-sectional study of 3,063 adult Saudis of both genders from King Khalid Hospital, Riyadh, Saudi Arabia was conducted. Using the “WHO STEPwise Surveillance-Instrument V2.1”, which uses sequential steps including questionnaires and anthropometric and biochemical measurements of MetS and CVRF. Waist to height ratio validity in defining central obesity, MetS, and CVRF were tested using receiver operating characteristic curve (ROC), sensitivity, specificity, positive and negative predictive values, and …

The Management Of Prehypertension In Young Adults, Ma Jun, Xiang Yali
Saudi Medical Journal
The incidence of prehypertension (blood pressure 120-139 and/or 80-89 mm Hg) in young adults worldwide ranges from ~37.5% to 77.1%. Identifying high-risk groups of prehypertension in young adults is helpful for early and effective interventions and treatments to reduce the occurrence of future hypertension and organ damage. This review summarized the epidemiological characteristics, disease intervention measures, and disease progression characteristics of prehypertension to provide a basis for the development of targeted intervention measures for young adults with prehypertension.
Idiopathic Chylopericardium: A Case Report From Saudi Arabia, Abeer M. Kawthar
Idiopathic Chylopericardium: A Case Report From Saudi Arabia, Abeer M. Kawthar
Saudi Medical Journal
Chylopericardium is a rare clinical condition in which chyle leaks into the pericardial space owing to lymphatic system derangement. It can be idiopathic or occur secondarily to another clinical condition. Optimal management is accomplished through surgical intervention to impede leaks and drain fluid. In this report, we discuss an initially asymptomatic patient with a pre-employment chest radiograph that revealed cardiomegaly. An echocardiogram revealed pericardial effusion. Medical workup was unable to determine secondary causes for effusion. Pericardiocentesis revealed milky-colored fluid with a high lymphocyte count, and a lymphoscintigraphy lymphangiogram revealed a leak into the pericardium and decreased flow through left iliac …

Brca2 Gene Mutation And Prostate Cancer Risk: Comprehensive Review And Update, Noor N. Junejo, Sultan S. Alkhateeb
Brca2 Gene Mutation And Prostate Cancer Risk: Comprehensive Review And Update, Noor N. Junejo, Sultan S. Alkhateeb
Saudi Medical Journal
The second most common type of tumor worldwide is prostate cancer (PCa). Certain genetic factors contribute to a risk of developing PCa of as much as 40%. BRCA1 and BRCA2 mutations have linked with an increased risk for breast, ovarian, and PCa. However, BRCA2 is the most common gene found altered in early-onset of PCa in males younger than 65. BRCA2 mutation has a higher chance of developing an advanced stage of the disease, resulting in short survival time. This review aimed to describe the genetic changes in BRCA2 that contribute to the risk of PCa, to define its role …

The Association Between The Timing Of Energy Intake And The Risk Of Overweight And Obesity Among Saudi Female University Students, Eman S. Alamri
Saudi Medical Journal
Objectives: To investigate the relationship between the timing of energy intake and obesity. Methods: A cross-sectional study was conducted between September 2018 and December 2018. A total of 450 Saudi female students from the University of Tabuk, Tabuk, Saudi Arabia were recruited. A 4-day dietary record was used to estimate the total energy intake per day and per meal. Height and weight were measured to calculate body mass index. Results: The study showed a significant negative association between obesity and energy intake at breakfast (r = -0.70, p<0.05) and mid-morning (r = -0.53, p<0.05) as well as a significant positive association between obesity and energy intake at dinner (r = 0.85, p<0.05). Additionally, there was no significant difference between the obese and normal weight groups in the total energy intake per day or the total percentage of energy from fat, carbohydrate, and protein per day. However, the total energy intake for each meal and the percentage of energy from fat, carbohydrate, and protein per meal were significantly different between the groups. Conclusion: The greater intake of energy at dinner was positively associated with obesity, while greater energy intake at breakfast were negatively associated with overweight and obesity.

Objectives: To describe causes of blindness and visual impairment (VI) in children in Eastern province, Saudi Arabia. Methods: A record-based descriptive cross-sectional study was conducted. Medical records of patients aged 2 to 16 years who were following up in the Pediatric Ophthalmology Clinics, Dhahran Eye Specialist Hospital, Dhahran, Saudi Arabia between September and December 2018 were reviewed. Causes of vision loss according to visual acuity (VA) with best correction were recorded. Blindness was defined as VA <20/400, VI as VA from 20/400 to 20/60, and visual loss as VA of ≤20/60. Results: Of 818 patients, 39% had visual loss, 22.9% were blind, and 71.2% had VI. Common etiologies of bilateral blindness were retinal dystrophy disease and Leber’s congenital amaurosis, whereas unilateral blindness was most common due to trauma and refractive error (RE). Common etiologies of bilateral VI were RE, esotropia, and retinal dystrophy. Unilateral VI was mainly due to RE, cataract, congenital esotropia, and trauma. Of all patients, 58.8% had treatable causes, 22.6% had preventable causes, and 19.5% had non-preventable and non-treatable causes; mostly genetic or congenital (59.7%) rather than acquired (40.2%). Conclusion: Genetic or congenital causes are major factors causing blindness. Most causes are treatable and preventable, emphasizing on early detection and treatment of those causes.

Saudi Medical Journal
Objectives: To evaluate the outcomes of fundoplication and gastrostomy (GT) in neurologically impaired (NI) children. Methods: A retrospective review of medical charts was performed on 178 NI children up to the age of 14 years inclusive, who underwent fundoplication and GT between 1999 and 2014. Results: After fundoplication, the incidence-rate (person-month) of all hospital admissions (0.95 versus 0.13; p <0.001), gastroesophageal reflux (GER) - related admissions (0.67 versus 0.09; p <0.001), and admissions for seizures (0.21 versus 0.01; p <0.001 were significantly decreased. Furthermore, all emergency department visits (0.94 versus 0.23; p <0.001), GER visits (0.61 versus 0.12; p <0.001), seizure visits (0.24 versus 0.01: p <0.001) were significantly reduced. The mortality rate after fundoplication was 35%. The risk factors for predicting mortality were being male (odds ratio: 2.2, p =0.027) and being a do not resuscitate (DNR) child (odds ratio: 5.2, p <0.001). Majority of the children that died within a year after the procedure were DNR. Conclusions: Fundoplication with GT is effective in reducing hospital admissions and emergency department visits from GER and seizures in NI children. Because of high mortality within a year of fundoplication with GT in DNR children, anti-reflux medications with GT might be an alternative.
