The Role Of Cross-Reactive Immunity To Emerging Coronaviruses: Implications For Novel Universal Mucosal Vaccine Design, Wael Alturaiki Oct 2023

The Role Of Cross-Reactive Immunity To Emerging Coronaviruses: Implications For Novel Universal Mucosal Vaccine Design, Wael Alturaiki

Saudi Medical Journal

Host immune response to coronaviruses and the role of cross-reactivity immunity among different coronaviruses are crucial for understanding and combating the continuing COVID-19 outbreak and potential subsequent pandemics. This review paper explores how previous exposure to common cold coronaviruses and more pathogenic coronaviruses may elicit a protective immune response against SARS-CoV-2 infection, and discusses the challenges posed by some variants of concern that may escape current vaccines. It also highlights the need for a mucosal universal vaccine that can induce long-term protection against current and emerging coronaviruses by leveraging cross-reactive immunity. We propose a novel mucosal universal vaccine that consists …

Understanding The Prevalence, Progression, And Management Of Metabolic Syndrome In Saudi Arabia, Faez Falah Alshehri Oct 2023

Understanding The Prevalence, Progression, And Management Of Metabolic Syndrome In Saudi Arabia, Faez Falah Alshehri

Saudi Medical Journal

Metabolic syndrome (MetS) is characterized by the coexistence of several disorders comprising hypertension, abdominal obesity, insulin sensitivity, and dyslipidemia. In recent times, MetS has gained increased attention due to the global prevalence of obesity. Adipose tissue plays a crucial role in this syndrome by releasing various molecules significantly affecting lipid/insulin regulation, oxidative stress, and cardiovascular function. Tumor necrosis factor-α (p-α), an inflammatory cytokine, and adiponectin, an adipose tissue-specific protein, are considered vital adipokines that play a significant role in the pathogenesis of MetS. The impact of dietary ingredients on MetS management has been extensively studied over the past few decades. …

Overview Of Hypertension In Saudi Arabia: A Systematic Review And Meta-Analysis, Sulaiman A. Alshammari, Abdullah S. Alshammari, Hotoon S. Alshammari, Shaik S. Ahamed

Saudi Medical Journal

Objectives: To assess the prevalence, awareness, treatment, and control of hypertension in Saudi Arabia. Methods: We searched electronic databases and the references lists of found publications between 1990 and 2022. Original cross-sectional studies in English were included using PubMed, Google Scholar, and the Saudi Digital Library. A meta-analysis was performed to assess the combined prevalence, awareness, treatment, and control rates of hypertension. Results: Twenty-nine studies with 278873 individuals aged 14-100 were considered. The pooled prevalence of hypertension was 22.66% (95% CI:18.95-26.60), Cochran’s Q=6221.98, dff=22, p <0.0001; I 2 =99.65%, Egger’s test ( p =0.0033) across 23 studies with 272378 people. The pooled hypertension awareness rate was 42.8% from 6 studies with 36046 participants (95% CI:35.66-50.01), Cochran’s Q=781.86, dff=5, p <0.0001; I 2 =99.4% and Egger’s test p =0.3772. The pooled proportion of hypertension patients treated in 6 studies involving 46075 samples was 59.4% (95% CI=38.14-79.02), Cochran’s Q=9793.79 dff=5, p <0.0001; I 2 =99.95%, Egger’s test p =0.8284. The pooled proportion of hypertension-controlled participants across 15 studies comprising 264817 subjects was 34.97% (95% CI: 27.62-42.68), Cochran’s Q=11048.28, dff=14, p <0.0001; I 2 =99.87% and Egger’s test p =0.9760. Conclusion: The prevalence of hypertension was high, with low awareness, treatment, and control rates among Saudis. Therefore, policymakers and healthcare providers must work harmoniously to promote health and to prevent, detect, and control hypertension early. PROSPERO Reg. No.: CRD42023407978

Clostridioides (Clostridium) Difficile: A Silent Nosocomial Pathogen, Ibrahim A. Al-Zahrani

Saudi Medical Journal

Clostridioides difficile (C. difficile) infection is still a threat to many healthcare settings worldwide. Clostridioides difficile epidemiology has changed over the last 20 years, largely due to the emergence of hypervirulent and antimicrobial-resistant C. difficile strains. The excessive use of antimicrobials, the absence of optimal antibiotic policies, and suboptimal infection control practices have fueled the development of this pressing health issue. The prudent use of antimicrobials, particularly broad-spectrum agents, and simple infection control measures, such as hand hygiene, can significantly reduce C. difficile infection rates. Moreover, the early detection of these infections and understanding their epidemiological behavior using accurate laboratory …
