OBJECTIVE: To determine the risk of transfusion associated infection for human immunodeficiency virus and Hepatitis C virus using nucleic acid testing. METHODS: During March 1998, 400 donor blood samples from the Saudi population that were negative by serology were further tested for human immunodeficiency virus 1 and 2 and Hepatitis C virus using nucleic acid testing. An Evaluation Of Theophylline Dosing. Suggested By The Saudi National Protocol For Children With Severe Asthma, Samira I. Islam, Ahmed S. Ali, Nadia M. Fida

OBJECTIVE: To evaluate the impact of Saudi national protocol on the diagnosis and management of asthma for optimizing theophylline use in children with severe asthma. To also review theophylline clearance and provide guidelines for individualization of doses. METHODS: Theophylline level was estimated in blood samples of 66 children with severe asthma who were admitted to King Abdulaziz University Hospital during the period 1998-1999. The theophylline doses given to these patients was reviewed and patients were categorized into 2 groups: Group one had received doses recommended by Saudi national protocol ± 10%, group 2 had received doses <90% of that recommended by Saudi national protocol. The distribution of theophylline levels in blood samples of each group was estimated. Theophylline steady state level was used to estimate theophylline clearance using a standard pharmacokinetic equation. RESULTS: Out of the total samples from group one, 70% of theophylline levels were within therapeutic range, while only 10% of the total samples of group 2 were within therapeutic range. The mean theophylline clearance in children (1-8 years) was estimated as 0.092 ± 0.023 and was found significantly higher than the mean theophylline clearance (0.069 ± 0.014) which is observed in older children (9-13 years). CONCLUSION: Saudi national protocol theophylline dose guidelines had a favorable impact on the optimization of theophylline use in children with severe asthma. Guidelines that ensure accurate adjustment of doses on individual basis in view of drug level were suggested.

Pityriasis lichenoides et varioliformis acuta is an uncommon disease, especially during pregnancy. In review of the obstetric literature, there was no report of pityriasis lichenoides et varioliformis acuta during pregnancy. A 25-year-old female was seen at 24 weeks of gestation for consultation about a cutaneous disease. She was admitted at 30 weeks of gestation because of threatened premature labor, and some active cutaneous papules presented themselves at that time. After the treatment, cutaneous papules remitted. OBJECTIVE: To assess the frequency of malnutrition in children with congenital heart defects in a hospital outpatient setting. METHODS: One hundred and fifty-two children with congenital heart defects (patients), and an equal number of children with innocent cardiac murmurs (controls) attending the Pediatric Cardiology Outpatient Clinic at Sultan Qaboos University Hospital, Muscat, Oman from 1997 to 1998 were prospectively studied. Weight, height and head circumference were used for evaluation of nutritional status. Patients and controls were categorized into acute malnutrition (weight <3rd percentile for age, and height >3rd centile), and chronic malnutrition (weight and height <3rd centile), based on the National Centre for Health Statistics standards. Children with symptomatic congenital heart defects (Group I, number = 73) were also compared with those without symptoms (Group II, number = 79). RESULTS: The study patients showed a higher frequency of both acute (27%) and chronic (24%) malnutrition (p<0.01). Children in Group I had a significant reduction in weight (p<0.01), height (p=0.02) and head circumference (p<0.01) compared to controls, however for those in Group II the differences were not significant. Acute malnutrition was higher in both groups, 31% (p<0.01) and 16% (p=0.04), whilst chronic malnutrition was significantly higher only in Group I, 31% (p<0.01). Infants in Group I were more frequently affected (33/45) compared to older children (13/28; p=0.02). CONCLUSION: Malnutrition remains a problem among children with congenital heart defects, especially in those with either heart failure or cyanosis, and symptomatic infants are the worst affected. Greater attention is required in the dietary management, early diagnosis and intervention to restore normal growth.

Fistulae-in-ano remains a significant surgical problem due to the limitations pertinent to accurate assessment and the magnitude, shortcomings and side effects of the various procedures currently employed for their management. Six consecutive fistulae-in-ano patients (males, age range 30 to 58 years) were successfully treated by KTP laser (20-watt continuous aim for 20 to 30 seconds delivering a total energy of 400 to 600 joules) with rapid solidification fibrin glue then followed up for 3 months without recurrence. This approach offers a simple, expeditious and effective treatment for fistulae-in-ano thereby overcoming many of the restrictions associated with other methods.

An attempt has been made to review the history of preservation of human cadavers from earliest mummification methodology used by ancient Egyptians to the most modern technique of embalming and plastination of human cadavers, developed by Von Hagens during the period from 1979 to 1987.
