Placenta Percreta With Urinary Bladder Involvement, Eftekhar H. Al-Ojaimi, Balameenakshi V. Subramaniam

Saudi Medical Journal

A 37-year-old Pakistani lady, who had previously undergone one cesarean delivery and one uterine curettage, was admitted to the labor ward at 29 weeks of gestation with history of a sudden severe painless vaginal bleeding from a sonographically diagnosed placenta previa. An immediate cesarean section was performed and a live male infant was delivered. The placenta was morbidly adherent to the lower uterine segment and attempts at removal caused torrential bleeding, necessitating cesarean hysterectomy. Vitamin D Receptor, Her-2 Polymorphisms And Risk Of Prostate Cancer In Men With Benign Prostate Hyperplasia, Mohammed T. Tayeb, Caroline Clark, Neva E. Haites, Linda Sharp, Graeme I. Murray, Howard L. Mcleod

Saudi Medical Journal

OBJECTIVE: Prostate cancer (PRCa) is one of the most common causes of cancer death in men and determinants of PRCa risk remain largely unidentified. Benign prostatic hyperplasia (BPH) is found in the majority of ageing men and has been associated with PRCa. Many candidate genes have been suggested to be involved in PRCa, such as those that are central to cellular growth and differentiation in the prostate gland. The vitamin D receptor (VDR) and HER-2 protooncogene have been shown to be involved in the regulation of cell proliferation and differentiation in prostate cells. Genetic variations of these genes could be …

Sickle Cell Disease In Pregnancy. Obstetric And Anesthetic Management Perspectives, Khalil E. Rajab, Jonathan H. Skerman Mar 2004

Sickle Cell Disease In Pregnancy. Obstetric And Anesthetic Management Perspectives, Khalil E. Rajab, Jonathan H. Skerman

Saudi Medical Journal

Recent advances in the pathophysiology, clinical investigations, and management of sickle hemoglobinopathies enables all physicians to better manage these disease states and their sequelae. Patients with sickle cell disease SCD are living longer and are thus more likely to contract unrelated diseases that require surgery and anesthesia. Patients with SCD continue to be a challenge to all branches of medicine particularly in obstetrics, surgery and anesthesia; however, the armamentarium of new knowledge and practice places a different perspective on the care of this old disease. In general, the literature to date suggests that neither prophylactic transfusion of pregnant sicklers nor …

Can We Reduce Mortality In Sepsis? Mar 2004

Can We Reduce Mortality In Sepsis?

Saudi Medical Journal

Considerable progress has been made over the last 2 decades in diagnosing and treating sepsis. Although the mortality rate is beginning to decline with the development of new therapeutic interventions, it still remains unacceptably high. Five such interventions are discussed in this review article to provide guidance for intensivists on the integration and implementation of new interventions into the intensive care unit. They were shown in randomized, controlled trials to reduce mortality by limiting the tidal volume in acute lung injury or acute respiratory distress syndrome, the early goal directed therapy, the use of recombinant human activated protein C, the …

Concurrent Papillary And Medullary Thyroid Carcinomas With Mixed Metastases To Lymph Nodes, Abdul-Wahed N. Meshikhes, Mukund Tingura, Jamal Y. Al-Saeed

Saudi Medical Journal

We report here a rare concurrence of medullary and papillary carcinomas of the thyroid in a 39-year-old Saudi male. He presented with a solitary nodule of the right thyroid lobe and underwent total thyroidectomy based on the cytological diagnosis of papillary carcinoma. Subsequent detailed histopathologic examination of the thyroid revealed 2 different types of malignancies in the 2 lobes papillary cancer in the right and medullary cancer in the left lobe. A single small lymph node showed metastatic replacement by both cancers. The extreme rarity and the interesting pathological features of such combinations are discussed.
