Adhesive Intestinal Obstruction Following Blunt Abdominal Trauma, Ashraf F. Hefny, Karl Lunsjo, Sandhya Joshi, Fikri M. Abu-Zidan
Saudi Medical Journal
Advances in diagnosis and management of multiple trauma patients have lead to adopting a conservative approach for most patients with blunt abdominal trauma. Intestinal obstruction is a rare complication for this approach. Herein, we report a 37-year-old male, who did not have an abdominal operation, and who developed adhesive intestinal obstruction 7 weeks following blunt abdominal trauma. We detected no signs of peritonitis or intra-abdominal bleeding clinically or radiologically on admission. We initially treated the intestinal obstruction conservatively, but the obstruction did not resolve. Finally, we performed laparotomy, which showed that the small bowel was matted together by thick fibrous …

Kasabach-Merritt Phenomenon, Khalid A. Al-Mazrou, Mark A. Richardson
Saudi Medical Journal
An association between a vascular lesion and a life threatening coagulopathy is called Kasabach-Merritt phenomenon KMP. It includes thrombocytopenia, microangiopathic hemolytic anemia, and disseminated intravascular coagulopathy. We cannot overstate the need for excellent and careful screening. Treatment modalities of KMP have included medication, radiation, embolization and surgery. Corticosteroids have traditionally been the mainstay of treatment. We report a 4-month-old girl with an extensive vascular lesion involving the left parotid, submandibular, and parapharyngeal regions, and with KMP. We treated her with a mega dose of corticosteroids. Her coagulopathy resolved, and her vascular lesion improved.

Saudi Medical Journal
The administration of drotrecogin alfa activated improves outcome in adult patients with severe sepsis. Since the published pediatric experience with this drug is limited, the role of drotrecogin alfa activated in children, and especially in newborns is not well established. We describe a 3-day-old neonate with septic shock and multiorgan system failure, including circulatory, respiratory, renal failure, and disseminated intravascular coagulation, refractory to intensive fluid resuscitation and inotrope support. Within hours of drotrecogin alfa activated administration, the neonate experienced dramatic improvement in hemodynamic parameters. The infusion was discontinued after 48 hours, without clinical deterioration. Aside from transient thrombocytopenia, no significant …

OBJECTIVE: The gene encoding the DNA repair enzyme O6-methylguanine-DNA methyltransferase (MGMT) is transcriptionally silenced by promoter hypermethylation in several human cancers including diffuse large B-cell lymphoma (DLBCL). We explored the aberrant promoter methylation of MGMT in Saudi diffuse large B-cell lymphoma and to investigate MGMT hypermethylation has an effect on patient9s overall survival. METHODS: In a retrospective cohort study, 100 cases of DLBCL were collected from the Department of Pathology at King Faisal Specialist Hospital and Research Centre, Riyadh, Kingdom of Saudi Arabia. We used methylation specific polymerase chain reaction to analyze the MGMT promoter methylation status in 100 tumor …
