Updated Overview Of Pediatric Headache And Migraine, Mohammed M. Jan

Saudi Medical Journal

Headache is a common complaint, occurring in >90% of school age children. The frequency increases with increasing age and the etiologies range from tension to life-threatening infections and brain tumors. Migraine is the most frequent cause of acute and recurrent headaches in children. The overall prevalence of non-migraine headaches is 10-25%. A thorough history, physical and neurological examination, and appropriate diagnostic testing if indicated will enable the physician to distinguish migraine and tension headaches from those of a secondary etiology. In this review, we present an updated overview of childhood headaches. The recently developed International Classification of Headache Disorders, second …

Current Treatment Strategy For Hepatocellular Carcinoma, Kelvin K. Ng, Ronnie T. Poon

Saudi Medical Journal

With the current practice of surveillance programs in high-risk patients, early stage hepatocellular carcinoma HCC is commonly diagnosed. This poses great challenge to clinicians, in terms of prognostic estimation, patient stratification to various treatment modalities and patient management during long-term follow-up. This review focuses on the current trends in the management of HCC, with special attention to tumor staging, treatment algorithm, and outcome of various treatment modalities. According to the American Association for the Study of Liver Diseases AASLD practice guideline, Barcelona Clinic Liver Cancer BCLC staging system has fulfilled the criteria that HCC patients can be stratified into different …

Intraventricular Hemorrhage In Very Low Birth Weight Infants. Associated Risk Factors In Isfahan, Iran, Zohreh Badiee Sep 2007

Intraventricular Hemorrhage In Very Low Birth Weight Infants. Associated Risk Factors In Isfahan, Iran, Zohreh Badiee

Saudi Medical Journal

OBJECTIVE: Prematurely born infants with intraventricular hemorrhage (IVH) suffer significant morbidity and mortality, particularly those infants with high-grade hemorrhage. Previous studies have proposed a number of risk factors for IVH; however, lack of adequate matching for gestational age and birth weight may have confounded the results. The purpose of this study was to identify variables that affect the risk of IVH. METHODS: We identified 31 preterm infants with gestational age <32 weeks and birth weight <1500 g, with IVH, from November 2003 to January 2005 at Alzahra University Hospital, Isfahan, Iran. A control group of 90 infants, matched for gestational age and birth weight was selected. Maternal factors, labor and delivery characteristics and neonatal parameters were colleted in both groups. Results of cranial ultrasound examinations were also collected. RESULTS: The significant risk factors for IVH were found to be: patent ductus arteriosus (PDA) [4 (13.3%) versus 3 (3.3%) (p=0.01)], longer duration of assisted ventilation [2.8 ± 5.8 days versus 0.5 ± 2.1 days (p=0.002)], higher number of endotracheal suctioning [9.7 ± 18.4 versus 3.06 ± 13.7 (p=0.004)], surfactant use, [7 (22.6%) versus 3 (3.4%) (p=0.003)], positive pressure ventilation at birth [(12 (38.7%) versus 12 (13.3%), (p=0.004)] and low Apgar score at 1 and 5 minutes. Whereas antenatal steroid treatments and cesarean section have a protective effect on the occurrence of IVH. CONCLUSION: Our data emphasize the importance of obstetric and perinatal care to improve outcomes of premature infants.

Fucosidosis is a rare, autosomal recessive lysosomal storage disorder caused by a severe deficiency of a-L-fucosidase. Patients usually have some problems with glycoprotein storage in the brain and other organs, and some structural abnormalities that need special consideration in anesthesia. It has 2 types, the early onset or infantile, and the juvenile. Here we present an 8-year old girl with deformities in the maxillofacial region, with big tongue, small and retracted chin, saddle nose, and short neck that could not be extended, causing difficult intubation, and congenital cardiac problems requiring a special anesthetic strategy.

Mobile Phone Induced Sensorineural Hearing Loss, Surayie H. Al-Dousary

Saudi Medical Journal

The increased use of mobile phones worldwide has focused interest on the biological effects and possible health outcomes of exposure to radiofrequency fields from mobile phones, and their base stations. Various reports suggest that mobile phone use can cause health problems like fatigue, headache, dizziness, tension, and sleep disturbances; however, only limited research data is available in medical literature regarding interaction between electromagnetic fields emitted by mobile phones and auditory function; and the possible impact on hearing. We report a case of sensorineural hearing loss due to Global System for Mobile Communications mobile phone use, in a 42-year-old male.
