Melatonin is produced in the human pineal gland, particularly at night, with the circadian rhythm of blood melatonin levels closely paralleling its production within the pineal gland. Light exposure at night, or rapid transmeridian travel severely compromises the circadian production of melatonin. The disturbed melatonin rhythm contributes to jet lag and sleep inefficiency, both of which are improved by melatonin administration. Melatonin is also a highly effective direct free radical scavenger and antioxidant. In this capacity, melatonin reduces experimental cataractogenesis, traumatic injury to the spinal cord and brain, and protects against oxidative damage to neurons and glia in models of …

OBJECTIVE: To audit the sleep service at Sultan Qaboos University Hospital (SQUH), Muscat, Oman, and to explore deficiencies to introduce new measures of improvement. METHODS: Polysomnography (PSG) reports and SQUH medical records of all patients who underwent sleep studies from January 1995 to December 2006 in the sleep laboratory at SQUH were reviewed and analyzed. RESULTS: Out of a total of 1042 sleep studies conducted in the specified period, 768 PSG recordings were valid for analysis. OBJECTIVE: To determine the tigecycline in-vitro susceptibility of naive gram-negative pathogens from serious infections in Delhi, India. METHODS: During July to October 2007 investigations were carried out to determine the tigecycline in vitro susceptibility of 50 consecutive gram-negative pathogens from serious infections at the Sant Parmanand Hospital, Delhi, India. Minimum tigecycline inhibitory concentrations were determined employing the E test method (AB Biodisk). RESULTS: Twenty-four percent of isolates were found to be tigecycline resistant or partly susceptible. Susceptibility of the isolates was lower than meropenem but similar to piperacillin-tazobactam, amoxicillin-clavulanic acid, and amikacin. OBJECTIVE: To investigate the relationship between survivin expression and invasiveness of pituitary adenoma. METHODS: A total of 66 patients, on whom trans sphenoidal surgery had been performed between July 2006 and March 2008, were enrolled in our study at the Department of Neurosurgery in Shandong Provincial Hospital and Jinan Central Hospital, Shandong, P. OBJECTIVE: To determine the relationship between elevated liver enzymes with iron overload and viral hepatitis in thalassemic patients. METHODS: This descriptive cross-sectional study was carried out in the thalassemic ward of Tonekabon Hospital, Mazandaran, Northern Iran from 20 April to 20 September of 2006. Patients were studied with respect to age, liver enzymes, anti-hepatitis C virus (anti-HCV) antibody, and hepatitis B surface antigen (HBsAg), transferrin saturation (TSAT), and blood transfusion index (multiplication of frequency and units of transfusion). METHODS: We carried out a cross-sectional study of 438 subjects (10-15 years), attending the public junior secondary schools, between March and November 2005. OBJECTIVE: To explore the knowledge of university students in Jeddah, western Saudi Arabia, regarding the national premarital screening (PMS) program. METHODS: A self-administered questionnaire was distributed to a sample of 800 students at King Abdulaziz University (KAU), Jeddah, Kingdom of Saudi Arabia during the first semester of the academic year 2005-2006. This included questions regarding socio-demographic data, personal history of hereditary disease, or premarital screening, knowledge on hereditary diseases, and on the national PMS program. OBJECTIVE: To observe the short-term effects of irbesartan treatment on microalbuminuria in patients with normotensive type 2 diabetes. METHODS: A total of 40 normotensive type 2 diabetes patients (mean age 55.1±11.4 years) who had microalbuminuria were included in this non-comparative and prospective research study. Hemoptysis is a frightening experience for both patient and clinician and warrants prompt evaluation and diagnosis. We report a case of a vallecular cyst (VC) in a 30-year-old gentleman presenting with hemoptysis. Carbon dioxide laser dissection of the VC after needle decompression was carried out with no complications or recurrence in a 2 months follow-up. In such rare lesions, the authors tend to have conflicting opinions in their classification, evaluation, and management. In this report, these opinions are reviewed and discussed.

OBJECTIVE: To compare total intravenous anesthesia (TIVA) with ondansetron, and metoclopramide in preventing postoperative nausea and vomiting (PONV) in laparoscopic cholecystectomy patients. METHODS: A prospective randomized double-blinded study was performed at King Abdulaziz University Hospital, Jeddah, Saudi Arabia in 2007. Seventy-five patients scheduled for laparoscopic cholecystectomy under TIVA were randomized to receive either: metoclopramide 10 mg (n=25), 4 mg ondansetron (n=25), or placebo (n=25) at the end of surgery. Postoperative nausea and vomiting episodes, analgesic supply, rescue medication, adverse events, and patient satisfaction were collected over 24 hours. RESULTS: Nineteen patients developed PONV. Sepsis and septic shock are major contributors to mortality in children worldwide. While many factors may contribute to poor outcomes, resource limitations play a role in developing countries. However, early recognition and aggressive therapy using simple measures can reverse shock in most children. Indeed, great gains have been realized in some areas with limited resources. In this manuscript, the significant gains over the last few years are outlined, and issues to consider in resource limited environments are discussed. In addition, an invitation is extended to participate in a global sepsis initiative.

OBJECTIVE: To assess the prevalence of HIV-1 genetic subtypes in Saudi Arabia in samples that are serologically positive for HIV-1, and compare the HIV-1 genetic subtypes prevalent in Saudi Arabia with the subtypes prevalent in other countries. METHODS: Thirty-nine HIV-1 positive samples were analyzed for HIV-1 subtypes using molecular techniques. The study is a retrospective study that was conducted in Dammam, Kingdom of Saudi Arabia, and in Abbott laboratories (United States of America) from 2004 to 2007. OBJECTIVE: To compare spinal shrinkage in obese and non-obese young male adults and to find any correlation between them. METHODS: In 2006, 123 second-year male students studying in the Colleges of Medicine and Dentistry, King Faisal University, Dammam, Kingdom of Saudi Arabia, were examined for their weights, standing heights, and recumbent lengths. OBJECTIVE: To compare maternal and neonatal outcomes of fetal macrosomia in diabetic and nondiabetic women. METHODS: A retrospective case-control study was conducted at Riyadh Military Hospital, Riyadh, Kingdom of Saudi Arabia from January 2004 to December 2006. A total of 766 macrosomic newborns met the inclusion criteria. We compared maternal and neonatal characteristics and outcomes between diabetic (group 1, n=207) and nondiabetic (group 2, n=559) women. RESULTS: There were significantly more macrosomic newborns in nondiabetic women; 73% versus 27% in diabetic women, p=0.0001. Cesarean deliveries were significantly higher in diabetic women compared to nondiabetic women (30.4% versus 19.5%, p=0.002). There …
