OBJECTIVE: We assessed the role of bibliometric methods in representing quantitative and qualitative differences in biomedical research outputs in Lebanon and the United Arab Emirates UAE. METHODS: Data on biomedical research productivity for years 1988-2007 were obtained from PubMed then imported into a specifically designed local database system and normalized to the population size for each country. RESULTS: Data reveal a continuous increase in research production in Lebanon, whereas a plateau phase is observed in the UAE between 1998 and 2007. In Lebanon, most of the citations originated from the capital city of Beirut, mainly the American University of Beirut. …

Venous thromboembolic VTE complications are leading causes of maternal mortality in the developed world. Over the past 20 years, there has been an increase in the incidence of deep venous thrombosis DVT in pregnant women, and this increase may be explained by the risk factors including older age, cesarean section, history of VTE, and presence of thrombophilia. OBJECTIVE: To estimate the penetrance of breast cancer genes 1 and 2 BRCA1/BRCA2 specific gene mutations in Iranian women with breast cancer. METHODS: We conducted this study in the Department of Biostatistics, Tarbiat Modares University, Tehran, Iran between January and May 2008. The information was collected from the referral database of the Cancer Clinics, Day General Hospital, Tehran, Iran. Incidental Invasive Thymoma During Coronary Artery Bypass Surgery, Moaath Al-Smady, Farouq F. Hammdan, Mahmood M. Abu-Abeeleh, Islam M. Massad

We encountered 2 incidental cases of invasive thymomas at Jordan University Hospital, Amman, Jordan; during routine coronary artery bypass graft surgery between 2005 and 2008 with an incidence of 0.6%. Both patients presented with angina pain. None of the 2 patients had pressure symptoms cough, shortness of breath or superior vena cava syndrome or Myasthenia Gravis symptoms. Total thymectomy with dissection of perithymic fat was performed on both cases. No radiotherapy was given. No recurrence of the tumor was seen in 2 years follow up. These cases are presented to emphasize the occurrence of this tumor.

This case report describes a 21-year-old female patient with a complex medical condition of end-stage chronic renal failure and secondary hyperparathyroidism presenting with a history of gradual enlargement of the facial bones over a period of one year. The facial enlargement primarily involves the maxilla causing a bizarre facial and dental deformity. Based on the clinical, radiographic, and laboratory investigations the facial deformity was confirmed as a rare manifestation of renal osteodystrophy presenting as maxillary hyperplasia and hyperostosis cranialis.

Water Status Of Breastfed Infants During Weaning In Yazd, Iran, Mehrdad Shakiba, Talie Montehaee, Zahra Nafei, Mohamad H. Falahzade

OBJECTIVE: To investigate the state of hydration in infants during the weaning period in dry and hot climates in the center of Iran. METHODS: Using a refractometer, 162 urine specific gravity (USG) was measured from 400 infants, between 4 to 6 months old. They were chosen among infants who visited the primary clinics in the city of Yazd, Iran for routine vaccination during the summer and winter of 2005. A questionnaire was filled out on air conditioning system and diet from mothers. A USG >1.020 was accepted as dehydrated, and a USG <1.010 as well hydrated. RESULTS: Thirty-one percent of the infants were dehydrated, and 34% were well hydrated, infants were more dehydrated in summer (p<0.05). Twenty-seven percent of them started solid food without water supplementation, and dehydration was significant in most of them (p<0.05). We could not determine which type of air conditioning devices now used at home affect water status, but there was a trend toward dehydration in those using both fan and cooler (p=0.096). CONCLUSION: In the desert area in the center of Iran, during weaning, approximately one third of the infants are exposed to the danger of dehydration. Our primary health care provider should consider the priority of water during weaning when water supplementation is safe.

OBJECTIVE: To determine the prevalence of both pre-hypertension and hypertension, and risk factors associated with the newly diagnosed Saudi military active duty personnel. METHODS: A community-based cross-sectional screening of 1238 Saudi military active duty service personnel was conducted during the period from September to December 2007 at the military units of Taif region, western Saudi Arabia. OBJECTIVE: To test for synergy between daptomycin (DAP) and rifampin (RIF) against RIF-resistant methicillin-resistant Staphylococcus aureus (MRSA) isolates. METHODS: Synergy testing using time-kill assay (TKA) was performed on 6 clinically, and genetically unique RIF-resistant MRSA isolates. The isolates were identified out of 489 (1.2%) samples collected during April 2003 to August 2006, from patients at the Ochsner Medical Center in New Orleans, Louisiana, United States of America. RESULTS: Synergy testing of DAP plus RIF by TKA showed that 5 isolates were indifferent, but one isolate was antagonistic. OBJECTIVE: To assess the outcome of patients with extra-abdominal desmoid tumor treated with low dose chemotherapy (methotrexate and vinblastine) both for tumor response and treatment related toxicity. METHODS: We retrospectively reviewed the outcome of 12 patients who underwent low dose chemotherapy for extra abdominal desmoid of different locations. The study took place in the McGill University Health Center, Montreal, Canada between 1996 and 2003. OBJECTIVE: To evaluate the efficacy of medical abortion at first trimester of pregnancy with vaginal misoprostol. METHODS: This is a descriptive analytic study. It was performed on 100 women at first trimester of pregnancy referred to teaching hospitals related to the Medical Sciences of Mashhad University, Mashhad, Iran for termination of pregnancy in 2006. OBJECTIVE: To investigate the possible relationships between plasma bilirubin levels and concentrations of nitric oxide (NO), malondialdehyde (MDA), and erythrocyte antioxidant enzyme activities in newborn infants with hyperbilirubinemia. METHODS: Thirty term (gestational age >/= 37 weeks) newborn infants with indirect hyperbilirubinemia aged less than 10 days were prospectively recruited in the Kahramanmaras Sutcu Imam University Neonatal Unit, Kahramanmaras, Turkey, between January and July 2007. OBJECTIVE: To assess the patients awareness of their medical conditions, identify the factors affecting their awareness, and assess patient9s satisfaction with their doctors explanations of medical conditions. METHODS: A cross-sectional study was conducted in October 2005 in the outpatient clinics of King Khalid University Hospital in Riyadh, Kingdom of Saudi Arabia. A self-administered questionnaire was used for data collection. The statistical package for Social Science was used for analysis. RESULTS: Five hundred and one patients were included in the study. OBJECTIVE: To report our experience with measurements of the objective structured clinical examination (OSCE) conducted for the final surgical clerkship year. METHODS: A pilot study of the OSCE was conducted on 64 students split over 2 consecutive days lasting 160 minutes each day in May 22-23, 2005 at the Department of Surgery, College of Medicine, King Saud University, Riyadh, Kingdom of Saudi Arabia. Acute Kidney Injury Secondary To Lymphomatous Infiltration And The Role Of Kidney Biopsy, Ahmed M. Alkhunaizi, Riyadh A. Daabil, Mohammad F. Dawamneh

We report a 47-year-old male patient who developed acute kidney injury requiring hemodialysis, associated with massive enlargement of both kidneys. A part from intra-abdominal lymphadenopathy, there was no other organ or lymph node involvement. A kidney biopsy established the diagnosis of non-Hodgkin9s lymphoma. The patient received chemotherapy with good response. This case demonstrates that the kidney could be the primary organ involved in non-Hodgkin9s lymphoma. In addition, we have shown that renal biopsy is adequate to make a diagnosis of lymphoma without the need to do more invasive testing.

Inflammatory myofibroblastic tumor (IMT) is associated in 15-30% of cases with systemic symptomatology, such as prolonged fever, weight loss, elevated erythrocyte sedimentation rate (ESR), anemia, thrombocytosis, and leukocytosis. We report the case of a 4-year-old Lebanese boy who presented with high-grade fever of long duration, and a single (unpaired) positive Widal agglutination test. Blood culture was negative. A diagnosis of typhoid fever was made. An abdominal (mesenteric) IMT was incidentally discovered, 30 days after the fever had appeared. After surgery, the fever disappeared immediately, and the ESR returned to normal. We strongly favor the possibility of a false positive Widal …
