Saudi Medical Journal
OBJECTIVE: To find the actual cause of death in death certificates that had ‘ill-defined’ causes in 2006, evaluate the correctness of the completion of those certificates, and recommend ways to decrease the proportion of ‘ill-defined’ causes of death in Bahrain. METHODS: This was a retrospective review of all death certificates that had ‘ill-defined’ as a cause of death (International Classification of Diseases-10 codes R0-R99) from January through December 2006 in Bahrain. RESULTS: Of the decedents with ‘ill-defined causes’ of death in 2006, 76.7% were Bahraini, 70.6% males, 37% older than 70 years, and 62.7% died in their homes. The underlying …

A cross-sectional representative sample of the Saudi population of healthy children below 5 years of age was used to calculate the prevalence of malnutrition. The study was carried out in the College of Medicine, King Saud University, Riyadh, Kingdom of Saudi Arabia. Body measurements of the weight, length, and height were performed according to standard recommendations. Standard deviation scores were determined using the Lambda, Mu, and Sigma (LMS) statistical methodology. The 1978 NCHS/WHO …

OBJECTIVE: To evaluate the ability of preadmission criteria used in most health professional schools in Saudi Arabia to predict the in-program performance. METHODS: This retrospective cohort study was conducted at King Fahd Medical City, Faculty of Medicine, Riyadh, Kingdom of Saudi Arabia between July and September 2008. Four sets were used to examine the predictive power of preadmission variables. The variables are the academic abilities (high school grades), aptitude test, achievement test, and an interview. The criterion variables were the undergraduate grade point averages9 (GPAs) of medical college students (n=193). The correlation between admission variables and the GPA was examined …

A Giant Ganglion Cyst Of Hip Joint Causing Lower Limb Edema, Weihua Gong, Fangmin Ge, Li Chen
A Giant Ganglion Cyst Of Hip Joint Causing Lower Limb Edema, Weihua Gong, Fangmin Ge, Li Chen
Saudi Medical Journal
Lower limb edema caused by a giant ganglion cyst of hip joint, rather than deep venous insufficiency or lymphedema, is unusual particularly for guiding clinical practice. In this study, we present a rare case of a 76-year-old man with a 3-month history of progressive swelling of right lower extremity and 2-month history of a known inguinal mass, which was initially thought as hemangioma by using ultrasound. But, subsequent computed tomography examination and exploration surgery revealed a giant ganglion cyst compressing the right femoral vein. Resection of the cyst led to complete resolution of symptoms postoperatively. To the best of our …

Incidence Trends Of Childhood Type 1 Diabetes In Eastern Saudi Arabia, Mohammad A. Abduljabbar, Jamal M. Aljubeh, Antony Amalraj, Mathew P. Cherian
Saudi Medical Journal
OBJECTIVE: To assess the epidemiology and incidence rate of type 1 diabetes in children <15 years of age in a subpopulation in the eastern province of the Kingdom of Saudi Arabia (KSA). METHODS: This is a subpopulation-based observational incidence study. Admission dates, diagnosis, age, and gender were collected for all Saudi children <15 years of age with new onset type 1 diabetes that received medical care at the Saudi Aramco Medical Services Organization, Dhahran Health Center, Dhahran, KSA during the period 1990-2007. The case ascertainment rate was estimated to be 100%. RESULTS: There were 438 patients diagnosed during the study period. The average incidence rate was 27.52/100,000/year (95% CI: 26.72-28.32), increasing from 18.05/100,000/year in the first 9 years of the study period to 36.99/100,000/year in the last 9 years. Twenty-one percent of patients were <5 years of age, with no significant difference in the rate of increase in the incidence rate of this age group compared to the older age groups. Of the total patients, 55% were females, and 40% presented with diabetic ketoacidosis. CONCLUSION: The incidence rate of childhood type 1 diabetes increased alarmingly over the past 18 years in our study population. A Kingdom-wide diabetes registry is essential to study the epidemiology of this disease in the whole country.

Biphasic Modulation Of Cocaine-Induced Conditioned Place Preference Through Inhibition Of Histone Acetyltransferase And Histone Deacetylase, Bin Hui, Wei Wang, Jing Li
Saudi Medical Journal
OBJECTIVE: To examine the causative relationship between aberrant histone acetylation changes and cocaine-induced reward. METHODS: Male Sprague-Dawley rats (n=160) were tested by conditioned place preference (CPP) procedure, to evaluate the effects of inhibitors of histone deacetylase (HDAC) and histone acetyltransferase (HAT) on the conditioned effects of cocaine. Conditioning sessions were conducted twice daily for 2-4 days. For each conditioning session, rats were injected with either HDAC (or HAT) inhibitors or saline in home cages, followed by cocaine (intraperitoneally [ip]) or saline (ip) 30 minutes later, and then immediately confined for 50 minutes in the cue-specific chamber. On the day following …

Two clinical teaching units (CTU) were compared head-to-head. Each CTU has 3 consultants. The CTU-control provides standard care, while the CTU-intervention was designed to provide better physician-consultant availability. Three outcomes were evaluated: patient outsourcing to another hospital, patient discharge during weekends, and overall admissions. Statistical analysis was carried out by electronic statistics calculator from the …

An Overview Of Various Labeled Assays Used In Medical Laboratory Diagnosis. Immune And Non-Immune Assays, Mohammad-Ayman A. Safi
Saudi Medical Journal
In this review, some light is thrown on various labeled immunoassays that depend on antigen-antibody (Ag-Ab) reactions, including immunofluorescence, radioimmunoassay, and enzyme immunoassay (EIA or ELISA). Their definitions, principles, and applications are described, then they are discussed chronologically to show their stepwise development that led finally to full automation. Enzyme labeled immunoblot assays (Western blot, blot spot, and recombinant immunoblot assay), and luminescence (bioluminescence and chemiluminescence) are also discussed chronologically. Labeled assays, that do not involve Ag-Ab reaction but rather, utilizing biotin-streptavidin (BS) interaction and probe-target DNA interaction, are described, together with their applications for DNA/RNA detection and genotyping. Finally, …

Magnetic Toy Ingestion Leading To Jejunocecal Fistula In A Child, Ali M. Ahmed, Mohamed H. Hassab, Abdulrahman A. Al-Hussaini, Tariq I. Al-Tokhais
Saudi Medical Journal
The accidental ingestion of a foreign body is a common problem in children, but ingestion of magnets is rare. When multiple magnets are ingested, they may attract each other and cause pressure necrosis through the bowel walls and eventually lead to serious complications like obstruction, perforation, and fistula formation. We report a case of a 5-year-old girl with jejunocecal fistula following ingestion of 2 magnet toys; it highlights the diagnostic challenge and the need for early surgical intervention in children especially when multiple magnets are ingested.

Testicular Schistosomiasis, Saeed M. Al-Qahtani, Stephane J. Droupy
Saudi Medical Journal
Testicular schistosomiasis is extremely rare, with only 12 cases reported. Reaching the diagnosis for this lesion that mimics malignant tumor is a great dilemma for the radiologist as well as the urologist. We report a case of testicular schistosomiasis from France that was managed conservatively. A 31- year-old Egyptian male presented to the outpatient clinic with primary infertility for 9 years. Physical examination revealed healthy male without any congenital malformations. Blood count and C-reactive protein level were normal. Hormonal tests were normal. Scrotal ultrasound showed small mass at the right testis. Scrotal exploration was carried out and frozen section results …
