There are no published medication possession interventions for heart failure patients at hospital discharge. This randomized control trial will pilot test Meds to Beds, in which medications are delivered before discharge. Results will indicate whether receiving medications at the hospital improves adherence and health outcomes among heart failure patients.

In January 2017, the "10-Year Long-Term Care plan 2.0" policy (Long-Term Care 2.0) promoted by the Government in Taiwan. Intervention improvement strategy, it showed that the effect of discharge planning linked to Long-Term Care plan 2.0 for elderly orthopedic surgery patient, which were elevated to four times in half a year.

As literature reveals that inefficient communication is a contributing factor to adverse events, communication boards has been an intervention implemented to help increase family/patient understanding of the plan of care. Thirty-three parents/caregivers and twenty-six health care professionals participated in a study that looked at the usefulness of communication boards.
