Purpose: The threefold purpose of this research is to identify the essential antecedents of cultural competence as identified by international nurse researchers, to compare the content of the extant cultural competence instruments to these antecedents and to potentially identify gaps in their conceptualization. A secondary aim of this research is to initiate validation of Harper's model of ethical multiculturalism. Conceptual Basis: The model of ethical multiculturalism depicts the attributes of ethical multiculturalism as the fulcrum of a balance between two ethical philosophies of fundamentalism and relativism. This thesis studies the traveling wavefront created by the autocatalytic cubic chemical reaction A + 2B → 3B involving two chemical species A and B, where A is the reactant and B is the auto-catalyst. The diffusion coefficients for A and B are given by DA and DB. These coefficients differ as a result of the chemical species having different size and/or weight. Theoretical results show there exist bounds, v* and v*, depending on DB/DA, where for speeds v ≥ v*, a traveling wave solution exists, while for speeds v < v*, a solution does not exist. Moreover, if DB ≤ DA, and v* and v* are similar to one another and in the order of DB/DA when it is small. On the other hand, when DA ≤ DB there exists a minimum speed vmin, such that there is a traveling wave solution if the speed v > vmin. The determination of vmin is very important in determining …

Latinos are the fastest growing minority population in the United States (U.S.) and have the worst access to health care of any ethnic group. The chronic disease of diabetes is twice as common in adult Latinos as in non-Latino whites, and the risk of death related to diabetes is twofold. Reasons for this disparity have yet to be clearly identified. This study had two purposes: 1) to explore cultural beliefs regarding health-care seeking behaviors in Puerto Ricans with diabetes who live in South Florida; and 2) to examine Puerto Ricans' perceptions about their health-care providers. Studies on the effects of immigration are becoming more important as the number of immigrants into the United States continues to grow. This study was implemented in order to discover the infant feeding practices and beliefs of Haitians living in Central Florida. A food frequency card sort and interview were conducted with thirty-four Haitian mothers. Mothers were asked to identify which foods they fed their children. Foods pictured on the cards included a majority of Haitian staple foods, some of which are primary to the American diet as well. In 1989 the hepatitis C virus was identified as a significant cause of post-transfusion hepatitis. Nearly two decades later there is still no vaccine, inadequate treatment options, and limited understanding of how the virus establishes chronicity in the majority of the people it infects. Recent reports suggest that the interaction of a negative co-stimulatory pathway mediated by PD-1 and PDL-1 is associated with persistent viral infection. The role, if any, that PD-1/PDL-1 has in HCV infection is unknown. In this study we report that PD-1 is upregulated in T-cells from persons with chronic HCV infection when compared to healthy donors. …

Microsporidia are obligate intracellular pathogens that cause severe disease in immunocompromised humans. While albendazole is the treatment of choice, no therapy exists that effectively treats all forms or causes of human microsporidiosis. Recent studies show that the microsporidian Encephalitozoon intestinalis binds glycosaminoglycans (GAGs) associated with the host cell surface, and that the divalent cations manganese (Mn2+) and magnesium (Mg2+) augment spore adherence to host cells by activating a constituent on the spore surface. Erythromycin is a macrolide antibiotic that inhibits not only mRNA translation but also 50S ribosomal subunit assembly in bacterial cells. An important mechanism of erythromycin resistance is the methylation of 23S rRNA by erm methyl transferase enzymes. We are interested in investigating the true substrate for methylation because it is known from our work and the work of others that fully assembled 50S subunits are not substrates for methylation. We have published a model for 50S ribosomal subunit formation where, the precursor particle that accumulates in erythromycin treated cells is a target for methyl transferase activity. The United States currently provides a health care system that is neither efficient nor equitable. Despite outspending the world on health care, over three-fourths of developed countries produce better health outcomes (Auerbach et al., 2000). Simultaneously, the "Ecological School of Thought" has documented the large impact that social, economic, and environmental circumstances play in health outcomes. Unfortunately, these 'ecological" studies are frequently conducted without theoretical justification, and rely solely on a cross-sectional research design and a myriad of unrelated variables. In this study, the bacterial growth dynamics of 14 pilot drinking water distribution systems were studied in order to observe water quality changes due to corrosion inhibitor addition. Empirical models were developed to quantity the effect of inhibitor type and dose on bacterial growth (biofilm and bulk water). Water and pipe coupon samples were taken and examined during the experiments. The coupons were exposed to drinking water at approximately 20°C for at least 5 weeks to allow the formation of a measurable quasi- steady-state biofilm. Bulk water samples were taken every week. Even though the number of women with Human Immunodeficiency Virus (HIV) grows annually, medical treatment has rendered this illness a chronic but manageable disease. Nurses must expand their knowledge beyond issues of survival and end of life care and focus on ways to maintain health and well being in this population. Efforts to create public policy integrating traditional/indigenous medical practices and practitioners into national healthcare strategies are currently being pursued in a variety of nations in the Africa region. The purpose of this research is to present a historical case study of the social and political factors associated with the passage of the Traditional Medicine Practice Act of 2000 in the West African nation of Ghana. In this thesis, we examine epidemiological models of two different retroviruses, which infect the human body. The two viruses under study are HIV or the human immunodefiency virus and HTLV-I, which is the human T lymphotropic virus type I. A retrovirus is a virus, which injects its RNA into the host, rather than it's DNA. We will study each of the different mathematical models for each of the viruses separately. Glucans are fungal cell wall PAMPs that promote survival in polymicrobial and candidal sepsis. Dectin-1 is the primary PRR for glucans. The goals of the present study were to characterize 1) the effects of fungal infection on Dectin-1; 2) the effects of polymicrobial sepsis in the presence and absence of glucan on Dectin-1; 3) the effects of systemic administration of glucans on Dectin-1; and 4) the intracellular trafficking of glucans. Sciadopitys verticillata was subjected to three propagation treatments designed to inhibit coagulation of its latex-like sap at the cut ends of the stem cuttings. Twenty-four hour soaking in water prior to rooting hormone application significantly enhanced production of adventitious roots. Old wood stem cuttings from shade-grown trees rooted at higher proportions than stem cuttings collected from sun-grown trees. Height, age, and place of origin of the source trees were not important factors in successful rooting. Antibacterial activity against some human pathogens and soil bacterial species was detected in latex application trials but the antibiotic activity was not related to the …

This thesis is an historical, theoretical and empirical examination of women's health concepts and health behavior in contemporary consumer-oriented American society. It begins with an historical overview of social health movements to illustrate how health concepts are shaped by cultural, social, political, and moral beliefs and practices. It then traces the socio-cultural construction of "health" to understand how this concept is shaped by various social and individual factors. To combat widespread infections caused by Staphylococcus aureus, mupirocin was introduced at the Veterans Affairs Medical Center, Mountain Home, Tennessee. Soon after introduction, high-level mupirocin-resistance emerged. The rapid emergence was hypothesized to be due to conjugative transfer of the mupA resistance gene from S. epidermidis to S. aureus. Results have shown that transfer of high-level mupirocin-resistance from S. aureus donors commonly occurs. However, transfer from naturally-occurring S. epidermidis donors was not attainable. Staphylococcus epidermidis transconjugants, however, were capable of serving as donors. Control of antibiotic resistance in bacteria is based on the concept that resistance incurs a fitness cost in non-selective conditions. Fitness costs were assessed for low- and high-level mupirocin resistance in locally-derived Staphylococcus aureus and S. epidermidis. Costs of resistance were assessed in pure cultures by comparing growth curve characteristics and in mixed culture as the proportion of resistant cells surviving. Costs were not present in comparisons of growth rates among groups of naturally-occurring isolates from the different resistance categories. However, in S. aureus, growth rates within resistance categories differed by approximately 30 – 90%. Among near-isogenic pairs …

The chloroplast has been demonstrated to be an ideal compartment to accumulate certain proteins or their biosynthetic products that would be harmful if they were accumulated in the cytoplasm. Hyper-expression of foreign proteins in chloroplast transgenics has accumulated up to 46% total soluble protein, this is possible due to the ~100 chloroplast genomes per chloroplast and ~100 chloroplasts per cell which can therefore, contain up to 10,000 copies of the transgene. Maternal gene inheritance of plastids in most crop plants results in natural gene containment. We examine an early model for the interaction of HIV with CD4+ T cells in vivo and define possible parameters and effects of said parameters on the model. We then examine a newer, more simplified model for the interaction of HIV with CD4+ T cells that also considers four populations: uninfected T cells, latently infected T cells, actively infected T cells, and free virus. The stability of both the disease free steady state and the endemically infected steady state are examined utilizing standard methods and the Routh-Hurwitz criteria. This study explored the relationship between Intimate Partner Violence and HIV Risk-Propensity in African-American women. Current literature on HIV reveals that one of the groups most seriously impacted by the continued scourge of HIV is African-American heterosexual women. The study design was based on the Utrecht School of phenomenology based on the Dutch school of phenomenology. As part of the pollution control effort several developing countries have established economic-based initiatives to improve environmental quality. In this study the Colombian water pollution charge, which was implemented in 1997, is analyzed. The two key questions that this study attempts to answer are: 1) Does the performance of the pollution charge program in Colombia undermine the notion that economic instruments are not feasible in developing countries? 2) Which factors determine the performance of the water pollution charge program across regional environmental agencies in Colombia?

In this work, two differential equation models for smallpox are numerically solved to find the optimal intervention policy. In each model we look for the range of values of the parameters that give rise to the worst case scenarios. Since the scale of an epidemic is determined by the number of people infected, and eventually dead, as a result of infection, we attempt to quantify the scale of the epidemic and recommend the optimum intervention policy.
