Do Outcomes From A Behavioral Weight Loss Intervention Differ In Alabama Vs Colorado?, Julianne Clina
Do Outcomes From A Behavioral Weight Loss Intervention Differ In Alabama Vs Colorado?, Julianne Clina
All ETDs from UAB
There are well-established regional differences in obesity prevalence in the U.S., but relatively little is known about whether these differences impact efforts for weight loss. The objective of the study was to determine whether changes in body weight, engagement in physical activity (PA), and psychosocial factors differed in Colorado (CO) vs Alabama (AL) in response to a 16 week standardized behavioral weight management program. We hypothesized that weight loss would be greater in Colorado due to a more favorable physical and social environment. This is an ancillary study to a weight loss intervention being conducted simultaneously in AL and CO with …
The Hocl Response Of Escherichia Coli., Rhea Marie Derke
The Hocl Response Of Escherichia Coli., Rhea Marie Derke
All ETDs from UAB
Inflammatory bowel diseases (IBDs), like Chron’s disease and ulcerative colitis, affect millions of people worldwide. There are limited treatment options available for these diseases because their direct causes are unknown. IBDs are characterized by chronic inflammation of the gastrointestinal tract and changes in the composition of the gut microbiome. Enterobacteria, including Escherichia coli, bloom to high levels in the gut during inflammation and contribute to the pathology of IBDs. E. coli must tolerate high levels of antimicrobial compounds produced by the immune system to survive during persistent inflammation. A large proportion of such compounds are reactive oxygen and reactive chlorine …
Race, Feeding Practices And Infant Weight Gain During The First 6 Weeks Of Life, Alysha Breanne Everett
Race, Feeding Practices And Infant Weight Gain During The First 6 Weeks Of Life, Alysha Breanne Everett
All ETDs from UAB
Background: Non-Hispanic black (NHB) infants are disproportionately affected by rapid weight gain compared to non-Hispanic white (NHW) infants, which in turn, is associated with future disparities including obesity and cardiometabolic disease. Feeding practices, such as the use of formula or bottles, differ by race and are associated with rate of weight gain. Data from the Pregnancy and Early Life in the South (PEARLS) birth-cohort study was used to test the hypotheses that change in body weight and weight-for-age z-scores (WAZ) from birth to 6 weeks would be greater for (1) NHB versus NHW infants; and (2) infants fed formula (exclusively …

Cognitive Progression And Care Patterns In All-Cause Dementia By Prior Cancer Diagnosis, Mackenzie Elisha Fowler
All ETDs from UAB
Cancer and dementia are common aging-related diseases. Cancer and itstreatments are associated with cognitive impairment referred to as cancer-related cognitive impairment (CRCI). Dementias demonstrate similar impairment. Only one study has examined the longitudinal association between cancer and dementia. This dissertation goes beyond limitations of the prior study and has three aims: 1.) to evaluate dementia progression by prior cancer diagnosis, 2.) to evaluate progression of all-cause dementia by cancer treatments, and 3.) to evaluate patterns and predictors of care among dementia patients by cancer history. This study uses EHR data to evaluate cognitive progression by cancer history /cancer treatment characteristics, …

The Role Of Visa Status And Acculturation In The Health Of Us Immigrants, Serra Sevde Hatipoglu
All ETDs from UAB
Using data from the 2003 New Immigrant Survey, I examined the association between visa status and acculturation with physical, mental, and self-rated health among immigrants residing in the United States. The primary predictor variable in the study is immigrant visa status. The outcome variables of interest include physical health as any morbidity (chronic diseases), mental health as depressed (as defined by the DSM-IV) and self-rated health (poor self-rated health). Using logistic regressions, I found those with unfavorable visa statuses have worse mental, physical, self-rated health compared to the favorable visa holders. I used inverse odds ratio weighted (IORW) mediation method …

Disparities, Telemedicine, And Covid-19: Rhetorical Approaches To Health And Medicine For Positive Behavior Change In Older African American Women, Audrey S. Wrenn
All ETDs from UAB
Overlapping exigencies experienced during the COVID-19 pandemic illustrate the need for adaptable and flexible public health responses that include cultural perspectives and lived experiences of specific groups. The rhetoric surrounding COVID-19 has proven to be multifocal because of the social and environmental experiences occurring simultaneously. Americans experienced periods of civil unrest, unequitable health care, overall lifestyle changes, and an immediate shift to telehealth use as the pandemic unfolded. This research examines telehealth engagement during the pandemic among African American (AA) women ages 75 and older to indicate the benefits and challenges to patient care in a virtual environment and from …
Experience-Dependent Plasticitiy Of Functional Connectivity In Human Visual Cortex Following Central Vision Loss, Leland Lanelle Fleming
Experience-Dependent Plasticitiy Of Functional Connectivity In Human Visual Cortex Following Central Vision Loss, Leland Lanelle Fleming
All ETDs from UAB
The human brain is shaped by a dynamic interplay between innate factors and life experiences. Neuroscience has long grappled with understanding this relationship, particularly with regard to how changes in experience impact the brain during adulthood. This question is especially important in the context of macular degeneration, a disease that causes significant visual impairment and drastically alters day-to-day visual experiences for individuals afflicted by the disease. Specifically, people with macular degeneration lose the ability to use vision in the center of the visual field, the region of the retina with the highest spatial resolution. Subsequently, these individuals must rely on …

This dissertation examined the physical and mental health disparities in the transgender male community in a Southern community-based medical LGBTQ (lesbian, gay, bisexual, transgender, queer) organization via a convergent mixed methods study design. There is a paucity of research on the transgender male community in terms of their physical and mental health care needs, services, and disparities. Quantitative data were col-lected via a chart review of 130 transgender male medical charts at the Magic City Well-ness Center (MCWC). The MCWC transgender male medical chart data were compared to three groups separately of the 2018 Behavioral Risk Factor Surveillance System (BRFSS) …
Circadian Clock And Lipid Metabolism Disruption In Fatty Liver Disease, Jennifer Valcin
Circadian Clock And Lipid Metabolism Disruption In Fatty Liver Disease, Jennifer Valcin
All ETDs from UAB
Hepatic Steatosis, characterized by the accumulation of triglyceride (TG) in the liver is the first stage of Alcoholic Liver Disease (ALD) and Nonalcoholic Fatty Liver Disease (NAFLD). Many metabolic pathways, including lipid metabolism, are regulated by the molecular circadian clock. Importantly, alcohol consumption and obesity induce circadian clock disruption, a risk factor for many diseases, including liver disease. In our first study, we investigated the combined effect of clock disruption and alcohol feeding on liver pathology, and diurnal rhythms in the liver molecular clock and lipid metabolism. Male liver-specific Bmal1 knockout (LKO) mice and control littermates were fed a control …
Contribution Of Adaptive Immune Responses In Chronic Lung Diseases, Thi Kim Tran-Nguyen
Contribution Of Adaptive Immune Responses In Chronic Lung Diseases, Thi Kim Tran-Nguyen
All ETDs from UAB
Immune dysregulation is a hallmark of various chronic lung diseases such as Chronic Obstructive Pulmonary Diseases (COPD) and Idiopathic Pulmonary Fibrosis (IPF). Previous studies in our lab have shown evidences of autoimmunity such as increases in autoantibodies, abnormal T cell profile, immune complex deposition and the presence of ectopic lymphoid structures in the lung of these patients. This thesis consolidates two separate projects about various aberrant immune responses in these diseases. The first project identified Glucose-Regulated-Protein 78 (GRP78) as the common autoantigen in COPD and characterized how GRP78 autoantibodies may increase COPD mortality via its atherogenic effects. I demonstrated that …

Understanding Financial Hardship In Women With Breast Cancer, Courtney Williams
All ETDs from UAB
Individuals with cancer are at risk of material, psychological, and behavioral financial hardship due to costly new treatments, increasing time on treatment, and longer survival. Breast cancer is an ideal environment to study financial hardship. Treatment options for metastatic breast cancer are heterogeneous due to differences in patient characteristics, disease biology, and because it has the largest number of treatment options of any metastatic cancer. In early stage breast cancer, treatment options include multimodality treatments, including surgery, radiation, and medical therapy (chemotherapy, hormone therapy, targeted therapy), with the choice for each modality being dependent on the other modality decisions. Despite …

Assessment Of Emergency Department High Utilizer Program In Public Health System Setting, Michael Malaise
All ETDs from UAB
is study examined whether a case management model designed to reduce emergency department utilization among patients described as super-utilizers in a public health system could also reduce the per patient cost of those super-utilizers. The setting for this study was a large public health system in a major metropolitan area. The study focused on a low-income, urban population. Given the program’s stated desire to improve the self-efficacy of patients over time so that they could take more ownership of their own health status, this study reviewed the program within the framework of self-efficacy theory. This research predicted that the case …
Molecular Regulation Of Glioblastoma Spatial Heterogeneity And Therapeutic Resistance, Soniya Bastola
Molecular Regulation Of Glioblastoma Spatial Heterogeneity And Therapeutic Resistance, Soniya Bastola
All ETDs from UAB
Glioblastoma multiforme (GBM) is a highly invasive, highly vascularized, and heterogeneous malignant tumor of the brain. Due to the highly infiltrative phenotype of GBM, surgery often leaves behind residual tumor cells. In many cases, recurrence occurs close to the surgical margin suggesting the role of these remaining cells in promoting tumor aggressiveness. Rapidly growing tumor creates subsequent hypoxic, and hypovascular core due to limited nutrients, whereas tumor cells in the leading edge have access to nutrients from vasculature enriched microenvironment. Studies have identified the cellular and molecular heterogeneity between the tumors in core and edge. Still, their mechanisms of intercellular …
Race, Type Ii Diabetes Mellitus And Vertebral Fragility Fractures: A Cross-Sectional Analysis From Nhanes, Yuchen At Xiao
Race, Type Ii Diabetes Mellitus And Vertebral Fragility Fractures: A Cross-Sectional Analysis From Nhanes, Yuchen At Xiao
All ETDs from UAB
IntroductionThis study examined the association between race and bone health, and if T2DM modifies this association in a diverse sample of adults. Methods We conducted a cross-sectional study using the 2013-2014 National Health and Nutrition Examine Survey (NHANES) data. Bone health outcomes included lumbar spine BMD (lowered BMD based on T-scores <-1), and vertebral deformities, assessed via vertebral fracture assessment (VFA), including presence, number of deformities, and severity ratio of deformities. T2DM status and other demographic, lifestyle, and health characteristics were self-reported via questionnaire data. To assess if T2DM modifies the association between race and bone health outcomes, we conducted stratified analyses using logistic regression for dichotomous outcomes, Poison regression for count outcomes, and linear regression for continuous outcomes. Results Non-Hispanic (NH) Blacks have significantly less odds of being lowered BMD and osteoporosis comparing to NH-Whites in the crude model and significantly less odds of having vertebral deformities comparing to NH-White in both crude and adjusted model for non-diabetic participants. Such differences are not observed in diabetic groups. Participants in Other race group have significantly more odds of being lowered BMD comparing to NH-Whites in the adjusted model for both diabetic and non-diabetic groups. And have significantly more odds of having vertebral deformities in the diabetic group, crude and adjusted, yet the differences are not significant in non-diabetic groups. Similar results were seen in the number of deformities and total bone loss ratio. The potential modification effect of T2DM was not significant in these models. Conclusions In our national evaluation, bone health differs not only by race and ethnic group but also by T2DM status. The protective vertebral fragility effects observed among NH-Blacks was no longer present among those with diabetes. Likewise, among participants in other racial and ethnic groups, T2DM seemed to magnify the association of worse vertebral fragility. Such trends indicate a potential modifying effect of participants’ T2DM status and should be noted clinically, yet more studies are required for more precise results.

Obesity manifests physiologically in multiple organ systems of the body, typically increasing the risk of developing cardiometabolic disorders. Researchers have questioned the causative role that western dietary patterns have on disease pathogenesis and have speculated that quality and quantity of dietary fats and/or carbohydrates have a predictive role in the development of these disorders. Standard reference diets such as the AIN-93 rodent diet have historically been used to promote optimal animal model health and reduce variation of results across experiments. These diets have been invaluable to murine research; however, their translational value in studying human nutrition-related pathologies is limited. Early …

Impact Of Diet And Meal Timing On Time-Of-Day Dependent Hippocampal Function, Jennifer Davis
All ETDs from UAB
Circadian rhythms are biological processes that cycle every ~24 hours and have allowed life to adapt for optimal function across the day. These rhythms are present in all tissues in the body and are coordinated to the light/dark cycle by the suprachiasmatic nucleus (SCN) in the hypothalamus. Along with active-rest cycles, cognitive performance fluctuates across the course of the day, peaking during the active phase. In addition to the light cycle, food and meal timing can provide signaling cues to extra-SCN tissues. Food access only during the inactive phase results in weight gain and memory impairment. High fat diet protocols …

Early Childhood Quality Of Education And Late-Life Cognitive Function In A Population-Based Sample From Puerto Rico, Cheyanne Celine Barba
Early Childhood Quality Of Education And Late-Life Cognitive Function In A Population-Based Sample From Puerto Rico, Cheyanne Celine Barba
All ETDs from UAB
In general, greater years of education is associated with reduced risk of cognitive impairment in older adulthood, but the strength of this association has been found to differ by study population. Additionally, quality of education is understudied, despite evidence that quality of education and, in turn, literacy, may be additional indicators of cognitive reserve. This thesis examined years of education, literacy, and childhood quality of education indicators in relation to late-life cognitive outcomes in an older Puerto Rican population. We hypothesized that greater years of education, literacy, and quality of childhood education would be positively associated with each other, as …

Self-Awareness Of Functional Deficits In Older Adults With Mild Cognitive Impairment, Kayla Ann Steward
All ETDs from UAB
Performance of instrumental activities of daily living (IADLs) can become compromised in older adults with mild cognitive impairment (MCI). Poor self-awareness of deficits, also known as anosognosia, occurs in a substantial minority of those with MCI and varies in extent depending on the domain examined. This dissertation attempts to explain the inter- and intra-patient variability in anosognosia by examining (1) differences in objective IADL performance and self-awareness of functional difficulties between those with amnestic single- (-ASD) and multi-domain (-AMD) MCI, and by examining (2) the neuropsychological and (3) neuroanatomical correlates of anosognosia for IADLs in older adults across a broad …
Psychometric Evaluation Of A Perceived Barriers To Medication Adherence Instrument And Association Between Perceived Barriers And Patient Characteristics, Lynn Andreae
All ETDs from UAB
Medication adherence is an important component of diabetes management, yet nonadherence is common and has substantial patient and economic costs. In order to achieve meaningful improvements in medication adherence, clinicians and researchers need good ways of both identifying those at risk of nonadherence and ascertaining the specific reasons driving nonadherence in those patients. Current options for self-reported medication adherence are limited in their vulnerability to response bias, which may lead to overestimation of adherence. A measure that assesses perceived barriers to medication adherence may be a useful alternative, as it may identify patients at risk of nonadherence while reducing socially …

Prediction Of The Optimal Catheter Placement Selection For Therapy Infusion To Treat Brain Tumors, Lisa Hope Antoine
All ETDs from UAB
ABSTRACT In 2020, the National Brain Tumor Society reported that physicians would diagnose more than 87,000 new cases of brain tumors. These cases will add to the approximately 700,000 individuals in the United States living with brain and central nervous system tumors. Almost 18,000 of these cases will prove fatal [1]. The blood-brain barrier (BBB) hinders the effective treatment of brain tumors. The task of this barrier is to protect the brain from composition instabilities in blood plasma and shield the brain from agents that may adversely impact brain functionality. However, by performing its tasks the barrier prevents potential effective …
Understanding The Impact Of Diet And Obesity On Anti-Tumor Immunity And Therapeutic Efficacy In Kidney Cancer, Rachael Miller Orlandella
Understanding The Impact Of Diet And Obesity On Anti-Tumor Immunity And Therapeutic Efficacy In Kidney Cancer, Rachael Miller Orlandella
All ETDs from UAB
Obesity is regarded as a major risk factor for developing renal cell carcinoma (RCC). Advanced-stage RCC exhibits chemotherapeutic resistance, but is responsive to immunotherapies such as the immune checkpoint inhibitor anti-programmed cell death-1 (anti-PD-1). Despite some clinical successes, response rates remain low for anti-PD-1 monotherapy (20-30%). Preclinical evaluations of immunotherapeutic strategies typically use lean mice and do not account for patient comorbidities. This may explain the underwhelming success rates following clinical translation of new cancer treatments. Recent studies found that obesity was associated with favorable outcomes and responses to immunotherapy in melanoma. However, the effects of obesity on anti-tumor immunity …

Investigating Growth-Related Hormones And Their Roles In Metabolism In Fish Skeletal Muscle, Ross Michael Lee Reid
All ETDs from UAB
The endocrine system is essential for the maintenance and regulation of skeletal muscle growth and metabolic processes, as secretions of growth-related hormones directly regulate these systems. The roles of these growth-related hormones are well understood in mammals yet are not well defined in teleosts. Thus, the work outlined in these chapters provides novel insight for the actions of two growth-related hormones, Growth Hormone (GH) and Teneurin C-terminal Associated Peptide (TCAP), in fish skeletal muscle. First, Chapter 2 highlights the work expanding the application of resazurin as a novel method of in vivo metabolic assessment across fish life-stages, which was utilized …

Dietary Nitrate And The Oral Microbiome In Obesity, Health And Exercise, Christian E. Behrens
All ETDs from UAB
Obesity ranks among the top growing health concerns worldwide. The prevalence of obesity at all life stages continues to rise with nearly 40% of adults and 35% of children/adolescents being affected. Equally troublesome is the increased risk for a number of comorbidities associated with obesity including cardiovascular disease, stroke, type 2 diabetes mellitus, and certain types of cancer. The successful treatment of obesity, particularly long-term, continues to prove challenging and is often met with limited success. This highlights the importance of continued investigation into alternative strategies that may reduce the incidence and severity of obesity and associated pathophysiologies. Regular physical …

Cognitive Factors And Treatment Adherence In Individuals With Cystic Fibrosis, Benjamin Edwin Burgess
All ETDs from UAB
Cystic fibrosis (CF) is a life-threatening chronic illness which requires adherence to a many complex and time-consuming treatments. Adherence is suboptimal in this population. Thus, it is important to consider predictors of treatment adherence. Research has evaluated a number of cognitive factors that are related to adherence in groups with chronic illnesses, including those with CF. The present study considers three factors (health locus of control (HLC), perceived social support, and perceived disease severity) in order to address important gaps in the literature evaluating their relationships with treatment adherence. For each of these factors, the current research is limited by …

Examining The Role Of Neighborhood Quality On The Association Of Mammography Adherence Among Racial/Ethnic Groups, Roman Johnson
All ETDs from UAB
Although several racial/ethnic minority women groups (e.g., Latina, Black, Asian-American, and Native-American women) have lower breast cancer incidence than white women, they are more likely to die from breast cancer. One possible explanation for this discrepancy is racial/ethnic differences in mammography use. Mammography has been shown to contribute to the discovery of tumor proliferation earlier which translates to less breast cancer complications like greater risk of breast cancer-related mortality. Previous studies on racial/ethnic differences in mammography adherence (e.g., annual and biannual mammography adherence) have been inconclusive, and they generally overlook important contextual factors, namely neighborhood quality and geographic location (e.g., …
