An Evidence Based Curriculum On Early Recognition And Prevention Of Delirium In Hospitalized Elderly, Albert Belaro Jan 2016

An Evidence Based Curriculum On Early Recognition And Prevention Of Delirium In Hospitalized Elderly, Albert Belaro

Yale School of Nursing Digital Theses

Abstract

Background: Delirium in older adults is common, but there is evidence that programs utilizing a set of non-pharmacologic interventions are effective in preventing it. Early recognition and delirium prevention programs may save lives and reduce healthcare costs.

Problem: Hospitalized older adults have special needs that can be addressed using evidence-based clinical practice. However, hospital nurses may lack the competency to care for patients at risk of developing delirium and can benefit from a delirium educational training program.

Design: The development of an Education program.

Methods: Critical content was identified, synthesized from the literature and developed into a curriculum outline. …

How To Provide Effective Pain Management After Cardiac Surgery In The Geriatric Patient, Katrien Derycke-Chapman Jan 2016

How To Provide Effective Pain Management After Cardiac Surgery In The Geriatric Patient, Katrien Derycke-Chapman

Yale School of Nursing Digital Theses

Of the >500,000 adult cardiac surgeries performed in the U.S. yearly, 54% are performed on the elderly. The American College of Critical Care Medicine indicated that 82% of cardiac patients reported pain as the most common traumatic memory of their ICU stay. Pain is an important stressor in critical care settings. The lack of effective postoperative pain management impedes early mobilization and sufficient sleep, leads to postoperative complications and higher costs, and contributes to a longer length of hospital stay. Adequate multi-modal and individualized postoperative cardiac surgery pain relief and postoperative pain relief in general, are imperative. Effective pain control …

Policy Brief On Diversity And Underutilization Of Nurse Practitioners: An Illinois Example, Mastura Zalwango Jan 2016

Policy Brief On Diversity And Underutilization Of Nurse Practitioners: An Illinois Example, Mastura Zalwango

Yale School of Nursing Digital Theses

Health Disparities among racial minorities cost the economy $309 billion annually (Kaiser Foundation, 2012). A component of healthcare disparities deals with lack of access to primary care providers and lack of diversity within the provider workforce. Nurse practitioners as a group of primary care providers have been extensively researched and deemed vital in addressing the issue of lack of access. Unfortunately, this group of health care providers lacks diversity. A federal government initiative created a program called the National Health Service Corps (NHSC) aimed to improve health access for racial minorities. However, the NHSC does not address diversity of the …

Early Intervention In Suspected Sepsis Patients, Timothy Richard Layman Jan 2016

Early Intervention In Suspected Sepsis Patients, Timothy Richard Layman

Yale School of Nursing Digital Theses

Objectives: Inform the development and implementation of an early response algorithm for suspected sepsis patients via a rapid response team.

Background: Recent literature supports the need for early recognition and intervention of suspected sepsis patients, potentially reducing morbidity and mortality.

Methods: A clinically and professionally reviewed algorithm was developed to execute early, sepsis-specific intervention. The algorithm design was carried out in 3 steps: (1) The establishment of recognition criteria based on evidence; (2) The validation of the algorithm by a panel of clinical experts; and (3) The development of a plan to initiate inclusion of the criteria into a rapid …

Salivary Cortisol As A Measure Of Stress Reactivity In Adolescents With Psychiatric Disorders, Cortlyn Wilhelmina Brown Jan 2016

Salivary Cortisol As A Measure Of Stress Reactivity In Adolescents With Psychiatric Disorders, Cortlyn Wilhelmina Brown

Yale Medicine Thesis Digital Library

Objective: The aims of this study were to 1) examine salivary cortisol distributions in a nationally representative sample of U.S. adolescents; and to 2) evaluate whether youth with mood and anxiety disorders are associated with differential salivary cortisol response to an acute mild stressor.

Background: An association between mood disorders and hypothalamic-pituitary-adrenal (HPA) axis dysfunction, as indicated by elevated levels of cortisol, has been well established [1, 2]. Although studies have suggested that these differences emerge early in the development of mood disorders in children and adolescents [3] the samples have generally been very small and primarily derived from clinical …

Characterizing Pediatric Primary Care In Greater New Haven, Meredith Ann Camp Binford Jan 2016

Characterizing Pediatric Primary Care In Greater New Haven, Meredith Ann Camp Binford

Yale Medicine Thesis Digital Library

Healthcare access is a complex concept that involves more than insurance coverage and resource utilization. Influenced by milieu, an understanding of the local dimensions of access is essential to develop community strengths as well as create appropriate solutions to access gaps. Accordingly, the objectives of the study were to identify, enumerate, and characterize medical practices delivering primary care to children within greater New Haven.

In partnership with community leaders, a community-based participatory approach was used to design a 57-question and 21-observation item, practice-based survey. The survey was administered in-person to 17 pediatric practices and 19 school-based health centers (SBHCs) in …

Clinical And Histologic Features Of Cutaneous Toxicities Due To Anti-Programmed Cell Death-1 Therapy, Veronica Shi Jan 2016

Clinical And Histologic Features Of Cutaneous Toxicities Due To Anti-Programmed Cell Death-1 Therapy, Veronica Shi

Yale Medicine Thesis Digital Library

Novel immunotherapies for oncologic treatment include anti-programmed cell death-1 (PD-1) and anti-programmed cell death ligand-1 (PD-L1) agents. These therapies activate the body’s inherent immune response against tumor cells by stimulating T cell proliferation. With the recent approval of agents such as nivolumab and pembrolizumab for the treatment of melanoma, lung cancer, and renal cell cancer, there is a growing need to better characterize their toxicity profiles. This study sought to provide a clinical and histologic description of the cutaneous toxicities seen in patients receiving anti-PD-1/PD- L1 treatment. Cases of patients on anti-PD-1/PD-L1 therapy who developed cutaneous adverse effects were collected …

Joining Forces:Establishing A Cardio-Oncology Program, Caroline Austin-Mattison Jan 2016

Joining Forces:Establishing A Cardio-Oncology Program, Caroline Austin-Mattison

Yale School of Nursing Digital Theses

Abstract

Patients have been surviving cancer diagnoses at a steady increasing rate over the past few decades. In spite of the encouraging decline in cancer morbidity, the cardiovascular effect of some chemotherapy medications is concerning. Moreover, even though there is extensive knowledge in regards to the pathophysiology and increased risk of cardiotoxicity, there is a lack of specific guidelines and adequate cardio-oncology programs focused on reducing cardiovascular risks or disease in patients undergoing cancer treatment. The high incidence of both cardiovascular disease (CVD) and cancer warrants the collaboration of oncology and cardiology providers to screen, treat promptly, and thereby provide …
