Peripheral artery disease impacts greater than 200 million people worldwide and is the third leading cause of atherosclerotic vascular morbidity. The early stages of the disease are ideally treated behaviorally with exercise. However, there is poor exercise adherence due to fear of pain, poor perceived control over the disease, low motivation, and lack of education surrounding the diagnosis. Imaging has been used to change exercise behaviors among patients with other diseases, such as coronary artery disease. This study will test whether allowing participants to see ultrasound images of their vasculature in response to exercise-associated claudication pain will increase adherence to …

Type 1 diabetes is a metabolic disease characterized by an inability to produce insulin, leading to hyperglycemia and other serious complications. Type 1 diabetes can be managed with exogenous insulin and careful dietary monitoring. However, adolescents with type 1 diabetes often have difficulty adhering to optimal treatment regimens, resulting in poorer diabetic control than patients in any other age group. In this study, we will test the efficacy of a serious videogame for increasing adherence to effective treatment regimens. We hypothesize that this intervention will lead to a significant reduction in glycosylated hemoglobin A1c, the standard assessment of diabetic control. …

Aerobic Exercise: An Adjunctive Therapy For Buprenorphine/Naloxone Retention In Opioid Use Disorder, Timothy Fong

Opioid agonist treatment has been shown to reduce mortality, comorbid infections, and opioid cravings in patients with opioid use disorder. However, patient long-term retention on opioid agonist treatment is low and hindered by neurobiological and physiological changes caused by chronic opioid use, such as increased baseline noradrenaline and hyperalgesia. Some of these changes have been shown to be reversible or preventable with aerobic exercise. In this randomized clinical trial, we will determine whether an outpatient, adjunctive, aerobic exercise program designed around personal preferences can improve the retention on opioid agonist treatment. Coronary artery bypass graft surgery is a high-risk surgical intervention and commonly accounts for hospital readmissions within 30 days of discharge. Pleural effusions that require therapeutic thoracentesis are a reason for many of the readmissions. Studies have examined the adverse clinical effects that large volume pleural effusions have in these patients. However, effusion volume itself may have little influence on patient outcomes. Complications occur without close monitoring and drainage. In this study, we propose a randomized controlled trial to determine whether frequent follow-up and drainage of pleural effusions ≥320 ml can improve the need for rehospitalization and influence 6-minute walk …

Iliotibial band syndrome is a highly prevalent and painful condition characterized by anterolateral knee pain from an inflamed iliotibial band, with the current standard of care being physical therapy. However, we lack sufficient adjunct treatment options for patients with refractory iliotibial band syndrome who fail physical therapy. This study will evaluate the efficacy and safety of using botulinum toxin type A injections into the tensor fasciae latae as an adjunct therapy for patients with refractory iliotibial band syndrome. Spatial autoregressive (SAR) and related models offer flexible yet parsimonious ways to model spatial or network interaction. SAR specifications typically rely on a particular parametric functional form and an exogenous choice of the so-called spatial weight matrix with only limited guidance from theory in making these specifications. The choice of a SAR model over other alternatives, such as spatial Durbin (SD) or spatial lagged X (SLX) models, is often arbitrary, raising issues of potential specification error. To address such issues, this paper develops an omnibus specification test within the SAR framework that can detect general forms of misspecification including that …

I consider a bargaining game with two types of players – rational and stubborn. Rational players choose demands at each point in time. Stubborn players are restricted to choose from the set of “insistent” strategies that always make the same demand and never accept anything less. However, their initial choice of demand is unrestricted. I characterize the equilibria of this game. I show that while pooling equilibria exist, fully separating equilibria do not. Relative to the case with exogenous behavioral types, strong behavioral predictions emerge: in the limit, players randomize over at most two demands. However, unlike in a world …

Do new migration opportunities for rural households change the nature and extent of informal risk sharing? We experimentally document that randomly offering poor rural households subsidies to migrate leads to a 40% improvement in risk sharing in their villages. Our model of endogenous migration and risk sharing shows that risky and temporary migration opportunities can induce an improvement in risk sharing enabling profitable migration. Accounting for improved risk sharing, the migration experiment increased welfare by 12.9%. However, permanent declines in migration costs improve outside options for households and can lead to reductions in risk sharing. The short-run experimental results for …
