Purpose: This DNP project developed an ED-initiated Advanced ED Care Management Program (AECM) to reduce the frequency of and costs associated with ED overutilizers with four or more ED visits in a rolling calendar year, excluding those with private insurance in an assisted living or skilled nursing facility, or who have a primary behavioral health diagnosis or chief complaint. Background: Overutilizers of the emergency department (ED) consume a significant proportion of healthcare resources and makeup approximately 21-28% of all ED visits in the United States. Only 20.5% of Youth with Special Healthcare Needs (YSHCN) receive transitional health care support services. This rate is even lower for minority and low-income YSHCN, as well as among subset populations of those with certain Intellectual and Developmental Disabilities. Multiple authorities in child health emphasize the importance of effective transitional care services targeted towards YSHCN to mitigate potential adverse health outcomes and reduced quality of life. Float nurses report frustration and dissatisfaction with their patient assignment distribution on a continual basis; citing that they often receive the highest workload and most acute patients. This experience is closely related to nurses’ job satisfaction with an established association between perception of unfair patient assignment and intent to leave the workplace. This project aims to utilize a workload score to create a balanced and transparent nursing assignment. Limited health literacy (LHL) has profound implications on measured transplant outcomes as it is associated with higher incidence of graft loss, organ rejection, decreased immunosuppression medication adherence, and increased hospital re-admissions. The COVID-19 pandemic brought forward a crisis that the current U.S. healthcare system was not prepared for. This increased the risk for burnout of teams and individuals, including the nurse leader. Nurse managers were selected as the target audience for support because they are key clinical leaders at the unit and staff level, yet they report feeling undervalued and are at higher risk for turnover than other leaders. This project developed a program that used reflective practice principles to support nurse manager well-being during periods of chronic distress.

The prevalence of PTSD has increased from 6.8% to 20% since the start of the Covid-19pandemic. For American Indians, the prevalence of PTSD ranged between 16 to 24% prior to the start of the pandemic. American Indians living in rural and tribal communities often experience significant barriers to accessing quality mental health treatment. For these reasons, telepsychiatry is a necessary tool to increase access to care. Insomnia occurs in as many as 75% of people with Opioid Use Disorder (OUD). The prevalence of insomnia is attributable to concurrent factors such as mental health disorders, chronic pain, and medication use; however the contribution of social factors is not well understood. Persistent social factors and concerns within this population include stigma and discrimination. People who use prescribed medication for OUD (MOUD) report stigma and discrimination related to OUD, MOUD, and possibly other characteristics (e.g., race, gender, socioeconomic status, and incarceration history). Between 1931 and 1941, the United States population over the age of sixty-five increased by over 35 percent. These individuals, however, did not receive increased support. Rather, American politicians and society at large labeled the elderly a ‘burden,’ and passed off their care to the overworked medical personnel of state and federal mental institutions. Sparked by clinical exposure to the influx of elderly patients to mental institutions, Massachusetts psychiatrist David Rothschild identified an erroneous assumption in care practices for senile patients.
