Treating The Children Of Bolivia Infected With Chagas Disease: A Cost-Benefit Analysis, Gregory A. Magee

The purpose of this study was to perform a cost-benefit analysis of an intervention to treat all the children in Bolivia (under 15 years of age) who are infected with Chagas disease. This research was carried out in La Paz, Bolivia where the author lived for a year collecting data in collaboration with the National Chagas Control Program, Bolivian Ministry of Health. Operational costs were based on current prices for laboratory testing and pharmaceuticals, average hourly wages for health care workers, and the number of children who would be treated. Behavioral economics has played a fundamental role historically in innovation in economic institutions, even long before behavioral economics was recognized as a discipline. Examples from history, notably that of the invention of workers’ compensation, illustrate this point. Though scholarly discussion develops over decades, actual innovation tends to occur episodically, particularly at times of economic crisis. Fortunately, some of the major professional societies, the Verein für Sozialpolitik, the American Economic Association and their successors, have managed to keep a broad discourse going, involving a variety of research methods including some that may be described today as behavioral economics, thereby maintaining an …

Patterns Of Efavirenz Use Among Hiv-Positive Women In Urban Hiv And High-Risk Obstetrics Clinics, Matthew Kronman

We investigated patterns of efavirenz use among physicians caring for HIV-positive women in the primary HIV and high-risk obstetrics clinics (HROC) of an academic teaching hospital. Efavirenz, a non-nucleoside reverse transcriptase inhibitor approved for HIV treatment, is a potential teratogen. The FDA recommends that women prescribed efavirenz use two forms of birth control and avoid becoming pregnant. We conducted a retrospective analysis of medical records of reproductive-age HIV-positive women seen between September 1998 and December 2002, recording information documented at initial prescription of efavirenz, pregnancies occurring on efavirenz, other antiretroviral medications taken during pregnancy, and fetal malformations noted at birth. …

Demographic Characteristics Of Adoptees Presenting To The Yale International Adoption Clinic And The Utility Of Pre-Adoption Video Review, Jon L. Boone

The major purpose of this study was to examine whether the assessment of pre-adoption video (pre-vid) by an experienced pediatrician accurately predicts the post-adoption developmental (post-dev) status of the adoptee on arrival and to examine any difference in the extent of developmental delay between those adoptees with and those without a pre-vid review. As a foundation for the study, an extensive database for all adoptees seen at the Yale International Adoption Clinic was created and their demographic characteristics were analyzed. The role of membrane ICAM-1 in intercellular adhesion, through interaction with the leukocyte beta2 integrin receptors LFA-1 and Mac-1, is well established. However, until recently, ICAM-1 was widely regarded as being simply an "anchor" molecule, promoting, for example, firm adhesion of lymphocytes to vascular endothelial cells. The demonstration that ICAM-1 is connected to the actin-based cytoskeleton through interactions with subcortical actin-binding proteins, such as alpha-actinin, enhanced this concept. However, ICAM-1 has been increasingly shown to have signaling properties of its own. Engagement of ICAM-1 extracellularly, either through its natural counter-receptors, LFA-1/Mac-1, or through antibody mediated clustering, can produce numerous changes …
