Regulating Mismeasured Pollution: Implications Of Firm Heterogeneity For Environmental Policy, Eva Lyubich, Joseph S. Shapiro, Reed Walker Jan 2018

Regulating Mismeasured Pollution: Implications Of Firm Heterogeneity For Environmental Policy, Eva Lyubich, Joseph S. Shapiro, Reed Walker

Cowles Foundation Discussion Papers

This paper provides the first estimates of within-industry heterogeneity in energy and CO2 productivity for the entire U.S. manufacturing sector. We measure energy and CO2 productivity as output per dollar energy input or per ton CO2 emitted. Three findings emerge. First, within narrowly de ned industries, heterogeneity in energy and CO2 productivity across plants is enormous. Second, heterogeneity in energy and CO2 productivity exceeds heterogeneity in most other productivity measures, like labor or total factor productivity. Third, heterogeneity in energy and CO2 productivity has important implications for environmental policies targeting industries rather than plants, including technology standards and carbon border …

Measuring The Burden Of Childhood Cancer Treatment: Decrements In Bone Health And Financial Toxicity, Hadley Bloomhardt Jan 2018

Measuring The Burden Of Childhood Cancer Treatment: Decrements In Bone Health And Financial Toxicity, Hadley Bloomhardt

Yale Medicine Thesis Digital Library

With research and clinical advances over the past several decades, survival rates for childhood cancer have improved to greater than 80%. However, the path to survivorship poses a substantial burden to patients and families, with lengthy treatment courses requiring frequent hospital visits, and with late complications of treatment that can present years after therapy ends. This thesis explores each of these areas: one study investigating bone health and morbidity in leukemia and lymphoma survivors, and another study examining the cost burden to families of children during treatment for average-risk acute lymphoblastic leukemia (ALL). We aimed to determine the utility of …

Synergy Of Bcl2 And Histone Deacetylase Inhibition Against Leukemic Cells From Cutaneous T-Cell Lymphoma Patients, Benoit Cyrenne Jan 2018

Synergy Of Bcl2 And Histone Deacetylase Inhibition Against Leukemic Cells From Cutaneous T-Cell Lymphoma Patients, Benoit Cyrenne

Yale Medicine Thesis Digital Library

The presence and degree of peripheral blood involvement in patients with cutaneous T-cell lymphoma (CTCL) portend a worse clinical outcome. Available systemic therapies for CTCL may variably decrease tumor burden and improve quality of life, but offer limited effects on survival; thus, novel approaches to the treatment of advanced stages of this non-Hodgkin lymphoma are clearly warranted. Mutational analyses of CTCL patient peripheral blood malignant cell samples suggested the anti-apoptotic mediator BCL2 as a potential therapeutic target. To test this, we developed a screening assay for evaluating the sensitivity of CTCL cells to targeted molecular agents, and compared a novel …

A Novel Staining Method For Detecting Bloodborne Bacteria After Their Dielectrophoretic Isolation, John Gaudet Jan 2018

A Novel Staining Method For Detecting Bloodborne Bacteria After Their Dielectrophoretic Isolation, John Gaudet

Yale Medicine Thesis Digital Library

Current methods to diagnose bacteremia typically require 18 to 36 hours to detect positive samples and five days to identify negative ones. This delay stems in part from the incubation time necessary for bacteria to multiply and achieve detectable concentrations. In recent years, microfluidic chips have been developed that isolate and concentrate bacteria from the blood on a time scale of minutes to hours (1). This research seeks to stain and visualize these bacteria after their microfluidic processing thereby significantly shortening the time required to diagnose bacteremia.

The traditional Gram stain is not sufficiently sensitive to detect bacteria in this …

Parental Perspectives On Myopia In An Urban Chinese Population, Wendy Li Jan 2018

Parental Perspectives On Myopia In An Urban Chinese Population, Wendy Li

Yale Medicine Thesis Digital Library

In this study, we aimed to understand how Chinese parents view myopia, including causes, progression, and treatment. We performed a cross-sectional study from July 2016 to August 2016 at Peking University 3rd Hospital in Beijing, China. Parents of myopic children completed a questionnaire including sociodemographics and Likert-style questions assessing perspectives on myopia. We employed logistic regression models to analyze relationships between responses and demographic data/degree of myopia. 109 parents completed the survey. Of these, 97 responses were included in the analysis. Almost all parents surveyed (97.9%) found their child’s myopia concerning and believe myopia is dangerous to their child’s vision. …

Simulation-Based Nurse Mentoring To Promote Preeclampsia Care: What Is The Impact In Bihar, India?, Julia Haven Raney Jan 2018

Simulation-Based Nurse Mentoring To Promote Preeclampsia Care: What Is The Impact In Bihar, India?, Julia Haven Raney

Yale Medicine Thesis Digital Library

Background:

Inadequately treated, severe preeclampsia and eclampsia (PE/E) may rapidly lead to severe complications in both mothers and neonates and are estimated to cause 60,000 maternal deaths globally each year. Simulation-based training where health providers review basic emergency obstetric and newborn care through highly realistic cases have demonstrated promising results in low- and middle-income countries (LMIC). Two international simulation training programs, Helping Mothers Survive and PRONTO International, have demonstrated improved overall use of evidence-based practices (EBPs) in active management of third stage of labor and hemorrhage management, though individual skills varied. However, the impact of simulation training on use of …

Optimal Counseling And The Utility Of Imaging Parameters In Patients With Idiopathic Normal Pressure Hydrocephalus, Harry Eswar Subramanian Jan 2018

Optimal Counseling And The Utility Of Imaging Parameters In Patients With Idiopathic Normal Pressure Hydrocephalus, Harry Eswar Subramanian

Yale Medicine Thesis Digital Library

It is very difficult to counsel patients who are suspected to have idiopathic normal pressure hydrocephalus (iNPH) due to the variability in diagnostic criteria and clinical presentations, as well as the difficulty in evaluating prognosis. This two part study was conducted in order to identify ways to improve the counseling of patients with iNPH. A qualitative analysis of patient experience with iNPH and shunting was initially performed. This was followed by an imaging analysis to identify predictors of outcome after shunt surgery.

A cohort of patients with iNPH who were shunted at a single institution were identified retrospectively and interviewed …

Targeted Nanoparticle Delivery For In Utero Treatment Of Congenital Disorders, Valerie Luks Jan 2018

Targeted Nanoparticle Delivery For In Utero Treatment Of Congenital Disorders, Valerie Luks

Yale Medicine Thesis Digital Library

Congenital disorders represent a large unmet clinical need with significant morbidity and premature mortality resulting from mutations and anomalies that can be diagnosed, and eventually treated, in utero. Nanoparticles (NP) have been used as versatile protective vessels for the delivery of a variety of therapeutic agents. Importantly, the potency of NPs can be amplified by modifications to the molecular composition as well as to the delivery to allow targeting to specific tissues. We hypothesize that optimization of NP targeting both by nanoparticle modification and the route of administration can increase overall uptake in specific cells and tissues of interest. To …

Altered Brain Functional Connectivity Varies By Form Of Craniosynostosis, Alexander Haosi Sun Jan 2018

Altered Brain Functional Connectivity Varies By Form Of Craniosynostosis, Alexander Haosi Sun

Yale Medicine Thesis Digital Library

This study uses functional MRI (fMRI) to study long-term neurocognitive sequelae of nonsyndromic craniosynostosis (NSC), and understand if these aberrations vary by form of synostosis. Twenty adolescent participants with treated NSC (10 sagittal (SSO), 5 right unilateral coronal (UCS), 5 metopic (MSO)) were matched to controls by age, gender, and handedness. A subgroup of MSO was classified as severe metopic synostosis (SMS) based on the endocranial bifrontal angle. Resting-state fMRI was acquired in a 3T Siemens TIM Trio scanner, and data was motion- corrected, cluster-corrected with nonparametric permutation tests, and analyzed with BioImage Suite. SSO had decreased intrinsic connectivity compared …

Reproductive Health And Fertility Among Transgender Adolescents: A Knowledge And Attitudes Survey, Aimee Alphonso Jan 2018

Reproductive Health And Fertility Among Transgender Adolescents: A Knowledge And Attitudes Survey, Aimee Alphonso

Yale Medicine Thesis Digital Library

The goal of this study was to determine the knowledge and attitudes of transgender adolescents regarding fertility and future parenthood. We developed and administered a cross-sectional questionnaire to a convenience sample of 23 transgender and gender non-conforming adolescents (mean age 16.2 ± 2.6) who attended the Yale Pediatric Gender Program between October 2016 and August 2017. Our results indicate that transgender adolescents have a basic understanding of reproductive health and fertility (mean total knowledge score of 3.78 ± 0.80 out of 5). Knowledge scores were significantly higher in participants with previous information about gender-affirming hormone therapy (GAH; p

High-Frequency Variability In The North Icelandic Jet, B. E. Harden, R. S. Pickart Jan 2018

High-Frequency Variability In The North Icelandic Jet, B. E. Harden, R. S. Pickart

Journal of Marine Research

We describe the high-frequency variability in the North Icelandic Jet (NIJ) on the Iceland Slope using data from the densely instrumented Kögur mooring array deployed upstream of the Denmark Strait sill from September 2011 to July 2012. Significant sub-8-day variability is ubiquitous in all moorings from the Iceland slope with a dominant period of 3.6 days. We attribute this variability to topographic Rossby waves on the Iceland slope with a wavelength of 62 ± 3 km and a phase velocity of 17.3 ± 0.8 km/day–1 directed downslope (–9° relative to true-north). We test the theoretical dispersion relation for these …

Smoking Cessation And Outcome After Ischemic Stroke Or Tia, Katherine Abigail Epstein Jan 2018

Smoking Cessation And Outcome After Ischemic Stroke Or Tia, Katherine Abigail Epstein

Yale Medicine Thesis Digital Library

People who smoke cigarettes are 2-4 times more likely to have a stroke and 2-5 times more likely to have a myocardial infarction compared with people who do not smoke. On a population level, it is estimated that smoking accounts for 33% of all cardiovascular deaths. Fortunately, quitting smoking can rapidly normalize risk. After an MI, smoking cessation reduces the risk of recurrent coronary events to the level of a never-smoker's after 3 years. While the benefits of smoking cessation in patients with cardiovascular disease are well-known, the benefits of smoking cessation after stroke have never been adequately examined and …

Objectives: To study communication in medicine within the context of readmission rates and patient satisfaction, by assessing 1) the perspectives of primary care physicians (PCPs) and home care nurses (HCNs) on why older adults are readmitted to the hospital within 30 days of discharge, and 2) patient perceptions regarding the implementation and value of bedside rounding.

Design: Two studies were performed independently. 1) A qualitative study consisting of PCPs and HCNs of patients readmitted to the hospital within 30 days of discharge home. 2) A concurrent mixed methods study consisting of patients admitted to the inpatient medicine service who participated …

Quality Assessment Of Clinical Practice Guidelines In Emergency Medicine, Alyssa B. Zupon Jan 2018

Quality Assessment Of Clinical Practice Guidelines In Emergency Medicine, Alyssa B. Zupon

Yale Medicine Thesis Digital Library

Clinical practice guidelines (CPGs) have been published by the American College of Emergency Physicians (ACEP) since 1990 to advance the delivery of emergency medical care. These guidelines have raised controversy, and recent research shows that they are largely based on lower classes of evidence and expert opinion despite Institute of Medicine recommendations. The rigor of development and overall quality of these guidelines have not yet been assessed. Thus, we performed a systematic review and meta-analysis to evaluate the quality of ACEP Clinical Policies using a recognized, validated appraisal instrument: Appraisal of Guideline for Research & Evaluation (AGREE II).

The AGREE …

Convergence Towards An Optimal Currency Area In The European Union, Santiago Botto Tornielli Jan 2018

Convergence Towards An Optimal Currency Area In The European Union, Santiago Botto Tornielli

Harvey M. Applebaum ’59 Award

This essay uses the European Union as a case study to analyze the conditions for an optimal currency area, the regional differences that give rise to divergences from optimal conditions, and how to measure divergence. The essay reviews the extensive body of theoretical literature on optimal currency areas (OCA), analyzes the historical evidence and documents that outline the economic institutions of the EU, and performs an empirical factor analysis on different possible indicators of convergence to identify to the most important variables. In the absence of adjustment mechanisms, economic geography plays an essential role in forming an OCA because members …
