The program projected here is: Connecticut Thrives. This program is intended to work with existing state structures to utilize Community Health Workers for the intended purpose of ensuring seamless healthcare. Taking a social …
Exposure To 1,4-Dioxane Dysregulates Stool Metabolomic Network, Yensheng Wang
Exposure To 1,4-Dioxane Dysregulates Stool Metabolomic Network, Yensheng Wang
Public Health Theses
1,4-dioxane is an environmental toxin found in drinking water that is highly associated with liver cancer. The interactions between environmental toxins and the human body can result in the development of tumorigenesis, it can also result in changes to metabolism and thus metabolites that circulate within the body. Some of these metabolites can be procarcinogenic, or can reflect changes to host processes that control metabolism. Stool contains copious information on metabolism that can provide an overview of the well-being of our body, and metabolites in the stool can provide information regarding the pathogenesis of disease and the toxicokinetics of toxicants. …
Assessing Risk Factors For Sudden Infant Death Syndrome And Caregivers’ Perceptions Of The Cardboard Box For Infant Sleep, Nisha Dalvie
Assessing Risk Factors For Sudden Infant Death Syndrome And Caregivers’ Perceptions Of The Cardboard Box For Infant Sleep, Nisha Dalvie
Yale Medicine Thesis Digital Library
Some US hospitals are giving out cardboard boxes as a way to address behaviors associated with Sudden Infant Death Syndrome (SIDS). Our goal was to evaluate the cardboard box for this purpose by quantifying current practices and qualitatively assessing caregivers’ perceptions of the cardboard box. Study participants were English or Spanish-speaking caregivers of 2-16 week old infants presenting to primary care clinics in New Haven, CT. Caregivers completed a survey asking about demographic data and SIDS risk factors, such as non-supine positioning and bed-sharing. Some caregivers also participated in a semi-structured interview about the cardboard box, created used a grounded …
Inhibition Of The Akt1-Mtorc1 Axis Alters Venous Remodeling To Improve Arteriovenous Fistula Patency, Arash Fereydooni
Inhibition Of The Akt1-Mtorc1 Axis Alters Venous Remodeling To Improve Arteriovenous Fistula Patency, Arash Fereydooni
Yale Medicine Thesis Digital Library
Arteriovenous fistulae (AVF) are the most common access created for hemodialysis, but up to 60% do not sustain dialysis within a year, suggesting a need to improve AVF maturation and patency. In a mouse AVF model, Akt1 regulates fistula wall thickness and diameter. We hypothesized that inhibition of the Akt1-mTORC1 axis alters venous remodeling to improve AVF patency. Daily intraperitoneal injections of rapamycin reduced AVF wall thickness with no change in diameter. Rapamycin decreased smooth muscle cell (SMC) and macrophage proliferation; rapamycin also reduced both M1 and M2 type macrophages. AVF in mice treated with rapamycin had reduced Akt1 and …

This study examined the mental health burden of LGBTQ asylum seekers and associated psychosocial risk factors with a focus on barriers to social integration. This study also characterized LGBTQ asylum seekers’ interest in interventions aimed at alleviating mental distress and social isolation. Respondents (n = 308) completed an online survey which included the Refugee Health Screener (RHS-15), the NIH loneliness scale, and an adapted scale of sexual identity disclosure. Most respondents (80.20%) screened positive for mental distress. Loneliness (OR = 1.14, 95% CI = 1.09, 1.19) and LGBTQ identity disclosure (OR = 3.46, 95% CI = 1.01, 12.02) were associated …
A Genomic Approach To Idiopathic Liver Disease In Adults, Aaron Hakim
A Genomic Approach To Idiopathic Liver Disease In Adults, Aaron Hakim
Yale Medicine Thesis Digital Library
Adult patients suffering from liver disease of unknown cause represent an understudied and underserved population. Over the past 15 years, next-generation sequencing technologies have matured into an inexpensive, effective, and widely available set of tools to do genomic analysis. One of these technologies, whole-exome sequencing (WES), allows for high throughput sequencing of all of the genome’s protein coding regions (exons). In pediatric cohorts, WES combined with deep clinical phenotyping has been shown to be an effective and unbiased method of identifying rare protein-altering coding variants in individual genes. WES has also contributed to the diagnosis and individualization of medical care …
The Reproductive Health Needs Of Women At Risk Of Hiv Infection In Connecticut, Emily Hoff
The Reproductive Health Needs Of Women At Risk Of Hiv Infection In Connecticut, Emily Hoff
Yale Medicine Thesis Digital Library
Background: The number of women involved in criminal justice systems (WICJ) and women who use opioids has been dramatically increasing in the United States. An often-overlooked aspect of healthcare for women at risk of HIV infection (either WICJ or women who use drugs) is reproductive health. We aim to provide a framework, informed by a systematic review and primary data, to guide future interventions addressing the sexual and reproductive health (SRH) needs of women at risk of HIV infection.
Methods: We completed a systematic literature review of the pregnancy prevention and planning needs of US WICJ published in English from …
The Worsening Trajectory Of Social Impairment In Preterm Born Young Adults And Its Association With Altered Amygdalar Functional Connectivity, Christina Johns
The Worsening Trajectory Of Social Impairment In Preterm Born Young Adults And Its Association With Altered Amygdalar Functional Connectivity, Christina Johns
Yale Medicine Thesis Digital Library
Survivors of preterm birth experience long-lasting behavioral problems characterized by increased risk of depression, anxiety, and impaired social functioning. The amygdala is a key region for social functioning, and alterations in amygdalar structure and connectivity are thought to underlie social functioning deficits in many disorders, including preterm birth. However, the trajectory of social impairments in PT and their association with functional connectivity of the amygdala are not well-studied in former preterm born individuals (PTs).
It was hypothesized that PTs would show impaired social functioning compared to term controls beginning in early childhood and continuing to young adulthood. It was also …
Csf Lymphocyte And Monocyte Activation And Trafficking In Primary Hiv Infection, Aveline Xiang Li
Csf Lymphocyte And Monocyte Activation And Trafficking In Primary Hiv Infection, Aveline Xiang Li
Yale Medicine Thesis Digital Library
Trafficking of immune cells to the central nervous system (CNS) is hypothesized to facilitate HIV entry and immune-induced neuronal injury, and is mediated by cell surface proteins such as chemokine receptors and integrins. We longitudinally assessed immune cell activation and surface marker expression in cerebrospinal fluid (CSF) and blood and their relationship with CSF HIV RNA during primary HIV infection (PHI) before and after combination antiretroviral therapy (ART). Longitudinal paired blood and CSF were obtained in initially ART-naïve PHI (infection) participants; some subjects independently initiated ART during follow up. Multiparameter flow cytometry was used to determine activation (% CD38+HLADR+) and …
Spatiotemporal Analysis Of Neuronal Activity During The First Second Of Visual Conscious Perception, Anusha Raja
Spatiotemporal Analysis Of Neuronal Activity During The First Second Of Visual Conscious Perception, Anusha Raja
Yale Medicine Thesis Digital Library
Visual conscious perception occurs in less than one second. Using cerebral cortex electrical readings from human subjects during a behavioral visual task, we studied the spatiotemporal sequence of these events. Nine subjects, implanted with 100-300 subdural and depth electrodes in both hemispheres, were enrolled to complete the threshold perception task. The analysis investigated the role of broadband gamma power changes in electric potential in the range of 40-115 Hertz (Hz). In addition to implementing the behavioral task with the research subjects, my contribution focused on using clustering algorithms to analyze the broadband gamma power time-series to visualize the “switch and …

Neurodevelopmental Risks Of Non-Syndromic Craniosynostosis, Robin T. Wu
Yale Medicine Thesis Digital Library
Purpose: Nonsyndromic craniosynostosis may manifest with complex cognitive,
language, behavioral, and emotional sequelae, depending on the suture fusion involved.
De-novo or rare transmitted mutations in the SMAD6 gene affect midline synostosis in
7% of patients. Current standards of assessment, such as the Bayley Scales of Infant
Development (BSID), may not predictive of long-term development, paving the way for
newer assessments such as functional magnetic resonance imaging (fMRI) and the event
related potentials (ERP), which measures passive neurological responses to speech
sounds.
Methods: Cranially-mature, post-operative unilateral coronal, metopic, midline SMAD6
mutated and age/race/gender/synostosis/operation matched non-SMAD6 controls from
the Yale Craniofacial Clinic …

Amazon River Infl Uence On Nitrogen Fi Xation In The Western Tropical North Atlantic, Joseph P. Montoya, Jason P. Landrum, Sarah C. Weber
Journal of Marine Research
We measured rates of N- and C-fixation with a direct tracer method in regions of the western tropical North Atlantic influenced by the Amazon River plume during the high flow period of 2010 (May–June 2010). We found distinct regional variations in N-fixation activity, with the lowest rates in the plume proper and the highest rates in the plume margins and in offshore waters. A comparison of our N- and C-fixation measurements showed that the relative contribution of N-fixation to total primary production increased from the plume core toward oceanic waters, and that most of the C-fixation in this system was …

Ossification Of The Phalanges Of The Foot And Its Relationship To Peak Height Velocity And The Calcaneal System, Mekka Garcia
Ossification Of The Phalanges Of The Foot And Its Relationship To Peak Height Velocity And The Calcaneal System, Mekka Garcia
Yale Medicine Thesis Digital Library
Background: There are multiple skeletal maturity grading systems, but none of them utilizes the phalanges of the foot. To minimize radiation, it would be ideal if one could assess the skeletal maturity of a foot based on bones seen on routine foot x-rays, if guided growth is being considered as a treatment option, as in hallux valgus. We developed a system that in combination with the calcaneal system, can closely predict skeletal maturity and help with the timing of surgical interventions of the foot.
Methods: We selected 94 healthy children from the Bolton-Brush study, each with consecutive radiographs from age …

Cardiotoxicity In Hematologic Stem Cell Transplant: Keeping The Beat, Julie K. Baker
Cardiotoxicity In Hematologic Stem Cell Transplant: Keeping The Beat, Julie K. Baker
Yale School of Nursing Digital Theses
Abstract
The number of hematopoietic stem cell transplants (HSCT) performed in the U.S. and worldwide is increasing. Cardiac events have been well described in HSCT, and incidence and type of cardiac events have not changed over recent decades. This study adds to the body of evidence in describing incidence and type of cardiac events experienced by an allogeneic and autologous HSCT population at one institution from 2012-2017. Sixty-five patients (9.8%) experienced cardiac events, including atrial arrhythmia (N = 39), acute heart failure (N = 9), acute coronary syndrome (N = 7), new onset hypertension (N = 9), with a few …
A Quality Improvement School Nurse Self-Assessment Tool For School Nursing Practice, Monica Chan
A Quality Improvement School Nurse Self-Assessment Tool For School Nursing Practice, Monica Chan
Yale School of Nursing Digital Theses
School nurses play a critical role in promoting student health and supporting academic success. The goal of this project was to develop a school nurse self-assessment tool (SN-SAT) for school nurses to evaluate their school nursing practice. The self-assessment tool was constructed based on the Framework for 21st Century School Nursing Practice. A panel of school nurse experts (n=5) was asked to rate the tool on two dimensions, Importance and Relevance, on a Likert Scale for the selected chronic health condition asthma. Content Validity Indexes for each item (I-CVI), and for the overall scale (S-CVI), were computed. The final SN-SAT …
Medication Access Using An Evidence-Based Adaptive Transitional Care Guidance Tool For Planned Release Of Older Inmates, Erin Iturriaga
Medication Access Using An Evidence-Based Adaptive Transitional Care Guidance Tool For Planned Release Of Older Inmates, Erin Iturriaga
Yale School of Nursing Digital Theses
The aging population is growing faster in the correctional system than in the overall U.S. population. Yet, an effective geriatric transitional care process is not available for older persons released from jail. This article documents the development and results of an evidence-based, adaptive transitional care tool guided by Meleis’ transitions theory to improve access to medications for older persons released from jail. Three sources of evidence were used to develop the tool: different categories of literature, expert panel determination of the tool’s domain and content relevance and importance, and a focus group interview of five people working in a transitions …
Methods For The Visualization Of Placental Vasculature In Fetoscopic Surgery, Praneeth Sadda
Methods For The Visualization Of Placental Vasculature In Fetoscopic Surgery, Praneeth Sadda
Yale Medicine Thesis Digital Library
Twin-to-twin transfusion syndrome is a potentially fatal disease of placental vasculature in twin pregnancies. It is treated with fetoscopic laser photocoagulation surgery, a procedure in which a surgeon uses an endoscope to find and cauterize abnormal vascular anastamoses on the placental surface. However, even after surgery, mortality remains as high as twenty to forty-eight percent, largely due to anastamoses being missed during the operation. Many technical challenges contribute to anastamoses being missed, including poor endoscopic image quality and a limited field of view. Any tools capable of addressing these challenges would be clinically valuable. This thesis presents the design and …
Counterfactuals With Latent Information, Dirk Bergemann, Benjamin Brooks, Stephen Morris
Counterfactuals With Latent Information, Dirk Bergemann, Benjamin Brooks, Stephen Morris
Cowles Foundation Discussion Papers
We describe a methodology for making counterfactual predictions when the information held by strategic agents is a latent parameter. The analyst observes behavior which is rationalized by a Bayesian model in which agents maximize expected utility given partial and differential information about payoff-relevant states of the world. A counterfactual prediction is desired about behavior in another strategic setting, under the hypothesis that the distribution of and agents’ information about the state are held fixed. When the data and the desired counterfactual prediction pertain to environments with finitely many states, players, and actions, there is a finite dimensional description of the …
