SAME-DAY REFERRAL OF PATIENTS WITH ELEVATED PREOPERATIVE BLOOD PRESSURE TO PRIMARY CARE PROVIDERSShaunte Butler, Natalie Holt, Feng Dai, Catherine Quick, Jeffrey D. Kravetz, Albert Perrino Jr., Robert B. Schonberger. Department of Anesthesiology, Yale University, School of Medicine, New Haven, CT.
Although recent studies show that elevated blood pressures as measured during the preoperative evaluation are often predictive of true hypertension, it is not yet known what interventions during the preoperative evaluation may result in modifications of longitudinal blood pressure trajectories. We herein report the results of a quality improvement project at a preoperative evaluation clinic that aimed to improve hypertension …

Preauthorizations In Orthopaedic Surgery: An Analysis Of A Single Institution’S Spine Center, Kareem Jamal Kebaish
Preauthorizations In Orthopaedic Surgery: An Analysis Of A Single Institution’S Spine Center, Kareem Jamal Kebaish
Yale Medicine Thesis Digital Library
Introduction
Preauthorizations are frequently required by commercial insurers before patients can undergo magnetic resonance imaging (MRI) of the spine. In addition to the administrative burden, such processes can lead to delays in care. The current study assessed the rates and eventual disposition of preauthorizations required prior to spine MRIs ordered from an academic spine center.
Methods
Patients from a single academic institution’s spine center who were covered by commercial insurance and referred for a spine MRI between January 2013 and December 2019 were included in the analysis. The requirement for preauthorization and eventual disposition of each of these studies was …
Integrative Genomics Implicates Disruption Of Prenatal Neurogenesis In Congenital Hydrocephalus, Shreyas Panchagnula
Integrative Genomics Implicates Disruption Of Prenatal Neurogenesis In Congenital Hydrocephalus, Shreyas Panchagnula
Yale Medicine Thesis Digital Library
Congenital Hydrocephalus (CH) affects 1/1000 live births and costs the US healthcare system over $2 billion annually. Mainstay therapies, hinging on surgical cerebrospinal fluid diversion, exhibit high failure rates and substantial morbidity. Limited understanding of pathogenesis warrants identification of crucial genetic drivers underlying CH and their impact on brain development. This pioneering study integrates gene discovery from the largest whole-exome sequenced CH cohort with transcriptional networks (modules) and cell-type markers from the latest transcriptomic atlases of the mid-gestational human brain to uncover the genomic and molecular architecture of CH. Exome analysis of 381 radiographically-confirmed, neurosurgically-treated sporadic CH probands (including 232 …
The Role Of Knowledge In End-Of-Life Decision-Making By Older Persons, Laura Van Dyck
The Role Of Knowledge In End-Of-Life Decision-Making By Older Persons, Laura Van Dyck
Yale Medicine Thesis Digital Library
As physicians, we aim to support our patients to engage in and make end-of-life medical decisions. Ideally, patients engage in advance care planning (ACP) to direct their care if they become incapable of participating in decision-making. For many patients, however, their first encounter with end-of-life care—e.g. code status—occurs at hospital admission. This thesis describes two studies: the ACP Study and Code Status Study. The ACP Study examines ACP knowledge and its relationship to engagement. This study utilizes baseline data from 921 participants age 55 years enrolled in the STAMP randomized controlled trial, including a knowledge scale consisting of 7 questions …

Editor's Commentary: Gravitational Circulation In Straits And Estuaries, Kenneth H. Brink, Maurice Rattray Jr.
Journal of Marine Research
For decades, people understood the rudiments of how idealized estuaries work: fresh water flows outward above saltier water that intrudes into the estuary. At the same time, mixing smooths out contrasts, so that the ultimate shallow outflow water is much saltier that the upstream riverine inflow had been. But, this vision is qualitative, so that it is hard to predict or diagnose flow structures or the outflow salinity. Hansen and Rattray provided a major step forward in an age before any sophisticated numerical models could deal simultaneously with the many competing effects...
Climate Niche Evolution In C4+Cam Portulaca And Closely Related C3+Cam Lineages, Nora M. Heaphy
Climate Niche Evolution In C4+Cam Portulaca And Closely Related C3+Cam Lineages, Nora M. Heaphy
Library Map Prize
With at least a hundred independent origins among land plants, the CAM and C4 photosynthetic pathways represent one of the most notable examples of global convergent evolution of a complex trait. While biochemically similar, CAM and C4 are generally understood to be two distinct ecological adaptations evolving along separate trajectories. However, the purslanes (Portulaca), a globally widespread clade of around 100 species of annual and perennial succulents, are able to operate both CAM and C4 cycles in the same leaf. Portulaca likely originated from a facultative CAM ancestor and then evolved a C4 system at least three times …

Modeling Black Literature: Behind The Screen With The Black Bibliography Project, Brenna Bychowski, Melissa Barton
Library Staff Publications
The Black Bibliography Project (BBP) plans to produce a bibliographic database of printed works by Black writers from the eighteenth to the twenty-first centuries. With the support of the Beinecke Library and a grant from the Mellon Foundation, project co-PIs and codirectors Jacqueline Goldsby and Meredith McGill collaborated with a team of librarians from Yale to develop the data model for their database. Drawing on Beinecke’s James Weldon Johnson Memorial Collection to pull case studies, the team of librarians developed a Linked Data model for BBP in an instance of Wikibase and trained and supported a group of graduate student …

Achieving Health Equity: Mitigating Implicit Bias Among Healthcare Professionals, Caroline Frazier
Achieving Health Equity: Mitigating Implicit Bias Among Healthcare Professionals, Caroline Frazier
Yale School of Nursing Digital Theses
AbstractImplicit bias is a contributing factor to health care disparities. Research shows that health care professionals exhibit the same level of implicit bias as the larger population (Fitzgerald & Hurst, 2017). The presence of implicit bias among health care professionals affects clinical decision-making and health outcomes, but it especially affects patient-clinician interaction (Joint Commission, 2016 and Burgess, Beach & Somnath, 2016). Health care professionals play a vital role in eliminating health care disparities by mitigating implicit bias through increased awareness and evidence-based debiasing strategies. The purpose of this project was to develop a didactic and experiential curriculum for healthcare professionals …

The Future Of Leadership Training: An Immersive Web-Based Program Enhancing Nurse’S Critical Soft Leadership Skills In New Healthcare Contexts, Nina Vaid Raoji
Yale School of Nursing Digital Theses
As the care of health shifts to a more decentralized model of care, it essential that the role of nursing facilitates new paths to meeting patients and communities where they are in the context of their lives, forging new partnerships outside of the traditional health care setting, and focusing on advocating for collective upstream solutions (Pittman, 2020). However, this shift requires a set of skills that are not a focus of current nurse leadership training. These are Soft Leadership Skills: networking, communication, teamwork, and innovation/creativity, but not in the traditional sense of the terms (Rao, 2017). The goal of this …

The program's intent was to support a moral culture and contribute to the retention of frontline nurses and improved safe patient care. Methods This project had three …
Establishing A Peer Supported Second Victim Program For Healthcare Providers: Team Heal, Renee N. Rathbun
Establishing A Peer Supported Second Victim Program For Healthcare Providers: Team Heal, Renee N. Rathbun
Yale School of Nursing Digital Theses
Medical errors and adverse events in healthcare resulting in serious patient harm have a substantial impact on patients, families, healthcare providers, and healthcare organizations (Coughlan, Powell and Higgins 2017, Dukhanin et al., 2018). When adverse events occur, the patient and often family members are the “first victims.” However, less attention is given to the healthcare providers known as the “second victims.” Involvement in adverse events leading to patient injury can leave healthcare providers traumatized with emotional distress. Healthcare professionals frequently suffer in silence, experiencing feelings of anxiety, fear, anger, depression, guilt, isolation, and shame (Edrees et al., 2016a). Although the …
Utilizing Covid-19 Models In Humanitarian Crisis Settings, Duane Barksdale
Utilizing Covid-19 Models In Humanitarian Crisis Settings, Duane Barksdale
Public Health Theses
Since the emergence of the COVID-19 pandemic in late 2019, several researchers and decision-makers have turned to statistical modeling to inform predictions of the severity of the burden of infection and disease in various settings. While it is understood that no model can perfectly describe the transmission of a novel and highly transmissible pathogen, there still lies the potential for models to be useful tools for local and international decision makers to act proactively to mitigate the burden of disease. In Syria, these challenges are compounded by humanitarian crises resulting from the ongoing civil war in the region. This conflict …

Without A Trace: Is Technology The Next Stage In Contact Tracing’S Evolution?, Madeline Bender
Public Health Theses
Contact tracing has become both a buzzword and a priority during the COVID-19 pandemic. Though it was deployed nationwide in the U.S., its success varied regionally. This special project thesis, in the form of a longform journalism article, traces the public health practice back to its roots to explore who contact tracers are, what they do, and what factors contributed to tracing’s limited success in 2020–21, centering around concepts of stigma and privacy. With this foundation, it assesses the promise and perils of incorporating technology into contact tracing during the COVID-19 pandemic.
Unmasking America's Reaction To Face Masks: A Review Of The Literature, Tyler Caruthers
Unmasking America's Reaction To Face Masks: A Review Of The Literature, Tyler Caruthers
Public Health Theses
The United States has faced challenges in reestablishing normalcy as we learn to live with COVID-19. This has included adhering to competent infection containment guidelines. Despite evidence that masks can assist in COVID-19 transmission reduction, the mere suggestion has sparked outrage, violence, and even death in communities across the country. To safely reintroduce any semblance of normalcy that has been lost over the last year, it is critical to consider the variables that influence one's internal personal perspectives and to comprehend how this influences various behavioral responses. Several critical risk communication principles can aid scientists in better understanding how people …
