The Anesthesiologist-Anesthetist Team, Bernard A. Kuzava Jan 1972

The Anesthesiologist-Anesthetist Team, Bernard A. Kuzava

MCV/Q, Medical College of Virginia Quarterly

In summary, I call upon anesthesia educators to abandon technician-training programs and instead help us develop nurse anesthetist programs to a higher level nationally. Let us remove our finger from the panic button and begin working together as a team, acknowledging our shortcomings and limitations. The time for us to establish the strongest possible ties both in our educational systems and our service-oriented institutions is right now. Mutual respect and cooperation are the keys to a smooth, efficient operation of an anesthesiologist-anesthetist team. And finally, let us be certain that the next meeting with the format of "What is New …

Nitrous Oxie-Curare Anesthesia: Reappraisal, Richard L. Keenan Jan 1972

Nitrous Oxie-Curare Anesthesia: Reappraisal, Richard L. Keenan

MCV/Q, Medical College of Virginia Quarterly

Of the many inhalation drugs available today, nitrous oxide is the only one which is both non-explosive and non-toxic. Although the compound is rarely listed as a primary agent, it is used on almost every patient. The usefulness of nitrous oxide tends to be dismissed because it is considered to be relatively weak. However, with the continuing trend toward lighter anesthesia seen during the past quarter century, this viewpoint is no longer reasonable. The fact that MAC for halothane can be reduced to one third by the addition of 70% nitrous oxide attests to the drug's potency.

Pulmonary Function Testing For Preoperative Study Of Patients For Anesthesia And Surgery, Orhan Muren Jan 1972

Pulmonary Function Testing For Preoperative Study Of Patients For Anesthesia And Surgery, Orhan Muren

MCV/Q, Medical College of Virginia Quarterly

In conclusion, if history and physical examination indicate pulmonary dysfunction, simple pulmonary function tests should be obtained and proper measures taken to reduce morbidity and mortality in surgery patients, especially those with underlying chronic lung disease.

The Chronically Ill Child: A Challenge To Family Adaptation, Ake Mattsson Jan 1972

The Chronically Ill Child: A Challenge To Family Adaptation, Ake Mattsson

MCV/Q, Medical College of Virginia Quarterly

In conclusion, by studying the emotional stress and the coping behavior of children with chronic physical illness, we can learn considerably about how these patients and their families learned to "play a poor hand well." In this regard it is noteworthy how often adolescent patients relate their good adjustment and positive outlook on life, despite their handicap, to their parents' early and consistent attempts to raise them with realistic and minimal restrictions. As one 16-year-old boy put it, as he was lying in bed receiving plasma infusion during a hemophilic bleeding episode, "Don't worry about the kids, Doc, but help …

Financing An Office, Personal Investments, And Money Management, James L. Keeler Jan 1972

Financing An Office, Personal Investments, And Money Management, James L. Keeler

MCV/Q, Medical College of Virginia Quarterly

Today's medical doctor is very much like today's lawyer, engineer, or accountant in that he is trained very highly in his own field, but has received very little educational background for successfully managing the office he operates. Although I am not an expert in any of the areas which I am about to discuss, I would like to pass along some observations, most of which are drawn from experiences I have had while practicing as a CPA. There is no set way to handle many of these problems, and I can only present some of the pros and cons I …

Cogan-Guerry Microcystic Corneal Epithelial Dystrophy: A Clinical And Electron Microscopic Study, Robert G. King Jr., Ragnit Geeraets Jan 1972

Cogan-Guerry Microcystic Corneal Epithelial Dystrophy: A Clinical And Electron Microscopic Study, Robert G. King Jr., Ragnit Geeraets

MCV/Q, Medical College of Virginia Quarterly

In summary, microcystic epithelial dystrophy of the cornea as described by Cogan and Guerry has become a well documented clinical entity. In this paper, a case report and preliminary electron microscopic findings of a patient with this dystrophy were presented. This case showed a thickened, two-layered basement membrane which was completely missing in some areas. The second characteristic change was that of cystic epithelial areas containing debris. The possible significance of these findings and a diagnostic technique for detecting of these corneal changes were discussed.

Accounting Systems And Patien Receivables: Complementary Management Areas, Richard J. Vargo Jan 1972

Accounting Systems And Patien Receivables: Complementary Management Areas, Richard J. Vargo

MCV/Q, Medical College of Virginia Quarterly

The first segment of my discussion will deal with medical accounting systems and my evaluations of those systems. The second part of the paper will deal with the topic of improving the cash flow from patient receivables. Cash flow is in many ways dependent upon the effectiveness of a doctor’s accounting system, and if the system works well and produces timely and accurate information, the cash from patient receivable will flow more efficiently.

Radiation Cataract: Biomicroscopic Observations In Rabbit, Monkey, And Man, Walter J. Geeraets Jan 1972

Radiation Cataract: Biomicroscopic Observations In Rabbit, Monkey, And Man, Walter J. Geeraets

MCV/Q, Medical College of Virginia Quarterly

The type or configuration of lens changes in these two species was quite different. This can be understood on the basis of the structural differences of their crystalline lenses based on the location of the suture lines.

Cryopreserved Corneal Tissue: A Practical Guide To Its Use, Walter Mayer Jan 1972

Cryopreserved Corneal Tissue: A Practical Guide To Its Use, Walter Mayer

MCV/Q, Medical College of Virginia Quarterly

What we will explain in detail is the technique we have developed to actually use cryopreserved tissue in the operating room. Anyone who has followed the development of this type of transplant material has heard and read how important it is to follow exactly certain steps, but nowhere in the literature is there a guide as to how, in practice, the different steps are to be carried out. We know of no other procedure in ophthalmology where absolute adherence to the strictest set of guidelines is absolutely necessary.

Acuity Variations In Diabetes Mellitus, William W. Carroll Jan 1972

Acuity Variations In Diabetes Mellitus, William W. Carroll

MCV/Q, Medical College of Virginia Quarterly

The purpose of this paper is to briefly review the factors involved in acuity changes in diabetes mellitus and to report findings suggesting a new concept, namely, diurnal fluctuations in acuity that are not associated with hypoglycemic episodes.

An Optical Approach To Aid Cerebral Hemiplegics, Thomas W. Nooney Jr. Jan 1972

An Optical Approach To Aid Cerebral Hemiplegics, Thomas W. Nooney Jr.

MCV/Q, Medical College of Virginia Quarterly

Communication is the process by which information is received, interpreted, and retained; however, communication often breaks down as a result of brain injury. It has been estimated that there are 400,000 adults in the United States with some degree of communication loss from stroke, and each year another 20,000 people become similarly afflicted. The ability to transmit and/or receive information depends on visual and auditory comprehension and retention, reading comprehension, visual motor facility, and speech. These areas are so interrelated that a collapse in one often causes a breakdown in a closely connected ability.

Experimentally Induced Coloboma In The Golden Hamster: A Preliminary Report, Ragnit Geeraets Jan 1972

Experimentally Induced Coloboma In The Golden Hamster: A Preliminary Report, Ragnit Geeraets

MCV/Q, Medical College of Virginia Quarterly

The present study represents the second part of a more comprehensive investigation of the morphogenesis of the normal and abnormal embryonic fissure. The first part, an electron microscopic study of the normal closure, has already been completed. The second part consists of the attempt to induce colobomata by means of hypo- and hyper-vitaminosis A. If both attempts are successful, the resulting colobomata will then be compared with one another as well as with the sequence of events in the normal closure. It is hoped that this investigation will permit some insight into the cellular phenomena that lead to a congenital …

The Surgical Approach To Problems Of Sexual Identification, Howard W. Jones Jr. Jan 1972

The Surgical Approach To Problems Of Sexual Identification, Howard W. Jones Jr.

MCV/Q, Medical College of Virginia Quarterly

While transsexualism is a disorder which seems to affect both anatomical sexes, the gynecologist plays his principal role in the therapy of the male transsexual, i.e., the anatomical male who considers the difficulty to be that of a female psyche trapped within a male body. It is only with these male patients with a female gender orientation that this discussion will deal.

An Ounce Of Prevention, Nancy G. Witt Jan 1972

An Ounce Of Prevention, Nancy G. Witt

MCV/Q, Medical College of Virginia Quarterly

In conclusion, we have found that an educational and behavioral approach has been most effective in correcting maladaptive behavior in children, and complete success is only limited by our own creativity. We cannot advocate behavior modification for all children because neither we nor our children should adapt to an ineffective educational system. Improvement in programming in both academic and social skills must be of primary emphasis in prevention. We must concentrate our best minds and personnel in the preschool and primary school years. As the child learns to accept the consequences of his behavior, the necessary social skills to meet …

Current Knowledge Of Gonadotrophin Releasing Factor(S), Emanuel M. Bogdanove Jan 1972

Current Knowledge Of Gonadotrophin Releasing Factor(S), Emanuel M. Bogdanove

MCV/Q, Medical College of Virginia Quarterly

What I want to discuss are not the potential therapeutic or diagnostic uses of these newly available compounds, which are really very speculative, but certain features of the physiological control system in which these compounds are thought to play key roles. Any rational system of therapeutics or prophylaxis involving these releasing factors, or their derivatives, must be based on adequate understanding of (1) the physiological roles of these compounds, (2) how these roles may be altered by disease, and (3) the effects specific disturbances of the control system(s) in which they operate can be expected to have on reproductive processes.

Anesthesia, 1972, C. Ronald Stephen Jan 1972

Anesthesia, 1972, C. Ronald Stephen

MCV/Q, Medical College of Virginia Quarterly

Implied in a title such as this one is an obligation to determine the present "state of the art," or science, if you will, and to elucidate, in a manner of speaking, where we are, why we are, and in which direction we may be going. To perform such a function in perspective, we must return to the past. Then one wonders, how far into the past? How much of the past is really relevant to today? Our reflection will go back thirty years to 1942, in Montreal, Canada, to the sphere of McGill University.

Oxygen Toxicity And Anesthesia: A Ten-Year Review And Overview, John Q. Durfey Jan 1972

Oxygen Toxicity And Anesthesia: A Ten-Year Review And Overview, John Q. Durfey

MCV/Q, Medical College of Virginia Quarterly

Anesthesia personnel will become increasingly involved in situations related to such exposures to oxygen at high pressure (OHP) such as will be found in the new fields of industry and agriculture now being developed underwater, sometimes at great depths. It is the responsibility of the specialty to continue basic and clinical research in these areas and to expand clinical teaching to encompass the management of related problems.

Some Therapeutic Considerations In Diabetic Retinopathy, Walter J. Geeraets, William W. Carroll Jan 1972

Some Therapeutic Considerations In Diabetic Retinopathy, Walter J. Geeraets, William W. Carroll

MCV/Q, Medical College of Virginia Quarterly

With the increasing emphasis on photocoagulation therapy in diabetic retinopathy, its potentials and, most probably, the beneficial effects such therapeutic modality may have in arresting or even improving diabetic retinopathy, one should realize the limitations, the present lack of factual knowledge, and the theories underlying such treatment.

Myocardial Infarction After General Anesthesia, Sait Tarhan, Emerson A. Moffitt, William F. Taylor, Emilio R. Giuliani Jan 1972

Myocardial Infarction After General Anesthesia, Sait Tarhan, Emerson A. Moffitt, William F. Taylor, Emilio R. Giuliani

MCV/Q, Medical College of Virginia Quarterly

Few statistics are available about the incidence of primary or recurrent myocardial infarction after anesthesia. Mortality rates and their relation to age and sex, and to type and duration of anesthesia in existing reports, are based on a relatively small number of cases. This review attempts to provide additional data on these questions by analyzing a 2-year experience in a large anesthetic practice.

Child Psychiatry, Walter Draper Jan 1972

Child Psychiatry, Walter Draper

MCV/Q, Medical College of Virginia Quarterly

The first annual Spring Forum for Child Psychiatry was held in 1963. Although interest in the establishment of programs for children was running high in many civic groups, there was little knowledge about how these programs could be organized and what they could accomplish. The first Forums served the purpose of sharing with the public and with other mental health professionals the experience and knowledge being gathered at the Virginia Treatment Center for Children. Later Forums have provided a means whereby other groups having a vested interest in children and their welfare might express their views, proposals, or the results …

Problems In The Delivery Of Health Care, Kinloch Nelson Jan 1972

Problems In The Delivery Of Health Care, Kinloch Nelson

MCV/Q, Medical College of Virginia Quarterly

The news these days is generally bad. Much of it is concerned with the inadequacies in the provision of medical care: its great cost; the shortage of health care personnel; and the misdistribution of the personnel available. All are blamed on the present system, or "non-system."

The Relationship Of Motility Problems To Reading Problems, Keith Mcneer Jan 1972

The Relationship Of Motility Problems To Reading Problems, Keith Mcneer

MCV/Q, Medical College of Virginia Quarterly

In summary, the ophthalmologist's role in caring for dyslectic children is simply caring for ocular problems. It is rather easy to separate the ocular defects from the nebulous concepts of dyslexia or learning disability. It is also easy to make the decision when and to what degree ophthalmological care is indicated with a clear and attainable goal in sight. Eye exercises or so-called visual training have virtually no value. Practically, the ophthalmologist's most valuable tool in caring for dyslexia is honesty about his results.
