Despite the design freedom that additive manufacturing (AM) processes provide, there are still challenges in using some AM processes for end-use products. Fused filament fabrication (FFF), also known as material extrusion, is one of the AM technologies that has been used mainly for prototyping due to the low cost of machines, raw material, and ease of operation. Another advantage of FFF is its ease of integration with other additive or subtractive technologies. Falling particle receivers (FPRs) such as the one at Sandia National Labs, represent the state-of-the-art Concentrating Solar Power (CSP) technology for energy harvesting. The FPR operates by creating a gravity-driven particle curtain in a receiver that is irradiated by concentrated sunlight from a field of concentrators. The particles are used directly as the heat transfer and storage media for the concentrated energy absorbed. However, during operation, particles can egress through the open aperture of the receiver cavity, resulting in particle-inventory and heat losses from the system. The particle plumes egressing from the cavity present a unique challenge to metrology due …

Before 2016 in southern New Mexico, the closest trauma center for patients was El Paso, Texas. Las Cruces, New Mexico, in Doña Ana County began developing its first trauma center in 2016 and became designated in 2017. The development of this trauma center changed trauma triage destination protocols for trauma patients. The improved collaboration and teamwork with prehospital personnel, surrounding hospitals, and existing trauma centers led to the development of trauma triage and transport guidelines. These protocols categorized trauma patients and constituted a guide for emergency personnel. Developing a trauma center in Doña Ana County improved the New Mexico Trauma …

Fast detection and isolation of faults in a DC microgrid is of particular importance. Fast tripping protection (i) increases the lifetime of power electronics (PE) switches by avoiding high fault current magnitudes and (ii) enhances the controllability of PE converters. This thesis proposes a traveling wave (TW) based scheme for fast tripping protection of DC microgrids. The proposed scheme utilizes a discrete wavelet transform (DWT) to calculate the high-frequency components of DC fault currents. Multiresolution analysis (MRA) using DWT is utilized to detect TW components for different frequency ranges. The Parseval energy calculated from the MRA coefficients are then used …

This report describes methods for decomposing student credits into three categories: applied, unusable, and excess where applied credits count towards the student's degree requirements, unusable credits do not count towards any degree requirement, and excess credits exceed one or more degree requirement specifications. These methods enable multiple types of analysis including student progress assessment, student planning (i.e. helping the student determine the best path forward), credit efficiency analysis (i.e. quantifying the number of applied versus unusable and excess credits), transfer credit analysis (i.e. determining the utility and efficiency of transfer credits), and requirement analysis (i.e. determining the types of requirements …

Aperiodic stacks of dielectric low- and high-index films can be designed to enhance third-harmonic generation (THG) in reflection of near infrared laser pulses using computer optimization. Numerical and analytical results suggest that the TH energy increases rapidly with increasing number of films and the ratio of the high and low index.
Our optical matrix based THG model that takes into account the full pulse bandwidth predicts conversion efficiencies of about 7% for transform-limited Gaussian pulse bandwidths of 16 nm for mirrors with 45 layers, which exceed those expected from periodic designs. ALK-positive histiocytosis is a rare histiocytic proliferative disorder most often seen in infants and young children. 1,2 We describe a patient with a novel presentation of this condition characterized by a palatal mass that was responsive to treatment with first-generation ALK inhibitor, crizotinib. The unique immunophenotypic and molecular characteristics of ALK-positive histiocytosis provide an opportunity for targeted therapy with ALK inhibitors and additional research should be focused on further characterization of this condition and potential treatment options. Additionally, we support the suggestion that ALK-positive histiocytosis should be considered a distinct entity from other histiocytic proliferative disorders 1,2, such as Langerhans …

Purpose: Delayed diagnosis of pediatric sepsis increases morbidity, mortality and healthcare costs. Specific interventions, such as a sepsis screening tool, sepsis huddles, and sepsis management pathway and continuous quality improvement (QI)-based reviews have been shown to decrease time to treatment and mortality. When this project was designed, UNMH did not have any of these interventions in place. This QI project aims to: 1) implement a sepsis screening huddle within 20 minutes when pediatric patients meet a clinical trigger based on vital signs; 2) ensure patients who screen positive for sepsis receive antibiotics within 3 hours; and 3) ensure that patients …

In the United States there has been many disagreements concerning the place of dance within Protestant Christianity. Some denominations have banned dance entirely while other utilize dance as an essential element of worship. At the center of this argument is the understanding, treatment, and use of the physical body. Beginning in the sixteenth century through current times, I analyze specific Protestant denominations including the Puritans, Evangelical Fundamentalists, Southern Baptists, the Shakers, certain African American denominations, and Pentecostals. Aim: To report a case of a teenager with congenital dyserythropoietic anemia (CDA). This is a 16y/o female who been noted to have mild persistent jaundice since early childhood. No history of abdominal pain, nausea or vomiting. No bleeding symptoms. Initial work up by PCP showed mild macrocytic anemia. Mom has history of jaundice of unclear etiology. O/E she has evidence of scleral icterus, no pallor and no evidence of hepatosplenomegaly. Background: Primary immunodeficiency is common diagnosis among patients with autoimmune cytopenia. Autoimmunity and immune dysregulation causing immune cytopenias might be an initial presentation in some patients. Case Presentation: We report 2 patients who presented with single lineage cytopenia. Patient 1: 19y/o female who was admitted 4 years ago with severe anemia. Work up showed H/H 6.7/19, retic 8%. Childhood obesity is a growing epidemic in the United States and New Mexico. Excess weight early in life progressively increases risks of severe health complications in adulthood. Healthy and Fit Children’s Clinic (H&FCC) at Carrie Tingley Outpatient Children’s Clinic treats children ages 2-17 years old with elevated body mass index (BMI; ≥85th percentile). Providers work to lower children’s BMI and risks for chronic cardiometabolic comorbid conditions through lifestyle changes. Successful weight management requires a comprehensive treatment plan including social determinants of health (SDoH) assessment. One key SDoH is food insecurity. H&FCC serves vulnerable populations. We report the complex case of a nine-year-old male who was adopted from Ghana approximately four years ago with a past medical history significant for congenital HIV on combination antiretroviral therapy and treated latent tuberculosis who presented with persistent fevers of unknown origin and who was ultimately diagnosed with multisystem hemophagocytic lymphohistiocytosis (HLH). As seen with our patient, the variable clinical presentations of HLH make obtaining a diagnosis challenging.1 Hemophagocytosis is a multisystem inflammatory syndrome that occurs when regulatory pathways responsible for the natural termination of immune and inflammatory responses are disrupted or overwhelmed.2 This regulatory failure results in excessive …

MM is a 12-month-old female with history of chronic constipation, megacystitis, muscle weakness and failure to thrive who presented to the pediatric ED for massive abdominal distention in the setting of decreased oral intake. CT revealed severely distended bladder compressing the colon. She was admitted for concerns of dysmotility and chronic intestinal pseudo-obstruction. Prior traditional constipation therapies, celiac panel, milk-protein allergy, and Hirschsprung’s workup were negative. Neurologic workup was negative. An upper GI with small bowel follow-through revealed delayed transit constipation. Background: The quality of interactions between children and their caregivers significantly impacts a child’s development. We explored the relationship between maternal interactive behaviors during play, parental well-being and cognitive and language development in children aged 18 months old. Methods: Healthy term infants were recruited in Albuquerque, NM between 2010-2012 as part of a longitudinal study (n=40). BSID-III was used as an objective measure of cognitive and language development. Background: Nutrition for the preterm infant following hospital discharge from the NICU varies greatly among institutions, however studies have shown that a high percentage of preterm infants globally have growth values infants, studies have shown that late preterm infants have higher mineral requirements than term infants and nutritional requirements of late preterm infants is also greater than that of term infants. While human milk and breastfeeding is recommended for the first six months of life, preterm and late preterm infants are likely to require fortification to promote growth and development after leaving the hospital.

Background: Near-infrared spectroscopy (NIRS) is a non-invasive technique for monitoring regional oxygenation of underlying tissues and has been well validated in neonates. Cerebral oxygenation may be a better predictor for risk of brain hypoperfusion and subsequent adverse neurodevelopmental outcomes than standard hemodynamic monitoring parameters. Low or fluctuating cerebral oxygenation has been shown to be associated with increased risk of intraventricular hemorrhage (IVH), adverse neurodevelopmental outcomes, and death in preterm infants. Touchscreen Operant Platform Paradigms Reveal Differences in Attention and Inhibition but not in Flexibility After Moderate Prenatal Alcohol Exposure and Placental Insufficiency Prenatal Alcohol Exposure (PAE) can result in fetal alcohol spectrum disorder (FASD), with individuals demonstrating abnormalities in frontal cortical development with a range of deficits in attention, executive functioning, and response inhibition. A higher incidence of placental insufficiency (PI), which can lead to neurodevelopmental delays, occurs in the setting of PAE. Background: Continuing education is imperative for all medical providers particularly those who deliver care to specialized populations such as transport of critically ill pediatric patients. Currently at the University of New Mexico, there is no designated pediatric transport team in which members have specialized training. Illnesses caused by coronaviruses have previously been shown to affect maternal and neonatal morbidity and mortality. Coronavirus disease 2019 (COVID-19) has been shown to affect vasculature including placental changes. As significant remodeling of the placenta occurs at the end of the first trimester into the second trimester, insults during this crucial period could affect placental functionality, with impact on fetal growth and well-being. Thus, we explored the relationship between antenatally-acquired maternal COVID-19 infection and birth characteristics.

Background: Skin-to-skin care (the intermittent placement of a diaper-clad infant on a caretaker’s chest to enhance attachment and parental self-esteem), is a common occurrence in most newborn intensive care units (NICUs). Skin-to-skin care has been shown to be safe for preterm infants, independent of respiratory support. Introduction:
Foreign body (FB) ingestion is a clinical challenge in children. Most are accidental and occur in children under 6 years. Guidelines published in 2015 by the North American Society of Pediatric Gastroenterology, Hepatology, and Nutrition (NASPGHAN) aid in the management decision-making process. We present cases of pediatric FB ingestion at UNMH between January and March 2022 and management for each case.
Cases:
Case 1: A 9-year-old boy swallowed a heart-shaped pendant. Two days after being found in the stomach, it passed to the colon without intervention.

Background: Pediatric patients with thromboembolic events (TEEs) often undergo testing for inherited thrombophilia disorders,1 including anti-thrombin III deficiency, Factor V Leiden mutation, prothrombin 20210 mutation, protein C deficiency, and protein S deficiency. While some data suggests testing for these disorders in all pediatric patients with a confirmed thrombosis,2 there is increasing evidence that this often does not result in meaningful changes in management1,3,4 and may not be cost effective.1 Another group suggests a stepwise testing algorithm to optimize diagnosis while preventing excessive testing.5 Our study will evaluate the rate of testing that results in diagnosis and management changes and explore …

Objective: The purpose of this pilot study was to determine feasibility and usefulness of integrating a fitness tracker (device for self-monitoring physical activity) coupled with a software program viewable by the clinician into virtual clinical interactions.
Methods: Adolescents with obesity (BMI >95th percentile) treated at a weight management clinic used a fitness tracker and associated phone application (app) to self-monitor physical activity for 4 weeks. We developed a parent guide with instructions for use of the device and app, and assisted participants with device set-up and app installation.
