Pediatric chronic osteomyelitis is a rare, debilitating condition lacking management guidelines. In a national survey of 162 pediatric infectious disease physicians through the Emerging Infections Network, tremendous variability in diagnostic approaches and management were noted, highlighting a need for prospective study to better define the spectrum of pathogens and disease.

A coronal mass ejection or a high-altitude nuclear explosion can produce an artificial radiation belt containing high-energy electrons (~1MeV) in the Earth's magnetosphere. Some electrons become trapped in the Earth's magnetic field and have enough energy to severely damage or destroy nearly all lower-earth orbit (LEO) satellites. Over the years, there has been much interest in using a space-borne dipole antenna to inject very low frequency (VLF) whistler waves (3-30kHz) along the Earth's magnetic field lines. The whistler waves precipitate the electrons from the magnetosphere through a wave-particle interaction called pitch-angle scattering. However, because the magnetosphere is composed of plasma, …

BACKGROUND: The financial hardships and social isolation experienced during the COVID-19 pandemic have been found to adversely affect children's developmental outcomes. While many studies thus far have focused on school-aged children and the pandemic-related impacts on their academic skills and behavior problems, relatively less is known about pandemic hardships and associations with children's development during their early years. Using a racially and economically diverse sample, we examined whether hardships experienced during the pandemic were associated with children's development with a particular focus on communication and socioemotional development.

The purpose of this amendment to the White Mountain Apache Tribe Water Rights Quantification Act of 2010 is to extend the enforceability date for the deadline for publication of the statement of findings to December 30, 2027 with a repeal date of December 31, 2027 of Title III of the Act for failure to meet the revised deadline. Certain provisions include clarifying funding, cost indexing, cost overrun subaccount (increase in authorized appropriations and prohibition), use of funds (expenditures), oversight and accounting, and other purposes.

Although droughts have long plagued the western United States, rapid population growth and climate change are making the American West increasingly water insecure. In some western states, including Arizona, Colorado, and California, decisionmakers are responding to these changes with innovative water conservation-focused land use policies. In other states, however, water and land use policies are lagging decades behind. Improving water security in western cities is an enormous task, requiring extensive social, legal, and policy reform. Additional federal funding for western water security initiatives could do much to drive that reform, but state and local governments should also play a leading …

Wild cards are improbable, high impact events. Such events, like the COVID-19 pandemic, have the ability to shape policy by upending entire systems and paradigms. Unfortunately, long-term planning and policymaking rarely account for wild card events. Instead, planning and policymaking typically assume that existing trends will continue without major disruption. Moreover, adequately planning for potential wild card events is challenging because they are inherently difficult to predict. However, planning for wild card events in natural resources and environmental policymaking is vital as global developments, such as climate change, create more extreme conditions and challenges. Renewable energy is increasingly scapegoated as the primary cause of weather-related power outages and other grid failures, despite substantial evidence to the contrary. Disinformation campaigns framing renewables as unreliable are driven by two factors: the increasing frequency of power outages and the growing pressures facing fossil fuel energy stakeholders. Over the past decade, power outages in the United States have doubled, primarily due to increased extreme weather, aging energy infrastructure, and a rapidly changing resource mix. At the same time, the energy transition is placing unprecedented competitive pressure on the coal and gas industry and on the nation’s utilities. These …

Most often, when law enforcement agencies or governments use a person’s DNA or genetic information, they do so in the pursuit of justice. That said, there are privacy concerns that arise when the government or police have open access to any person’s DNA or genetic information, especially through the Internet. This article attempts to present an argument for a reasonable expectation of privacy (REP) in a person’s genetic information or DNA as well as other forms of advancing technology. Under traditional common law, a plaintiff could recover damages for libel if she could prove that the defendant had published a factual statement about the plaintiff that tended to injure the plaintiff’s reputation. The plaintiff, at most, was required to show negligence to recover damages for libel. While the amount of money that any given plaintiff could recover in damages was uncertain, one thing was clear: the First Amendment would not protect libel. In 1964, in New York Times Co. v. Sullivan, the Supreme Court radically upended this received view of libel as unprotected speech. According to Sullivan, …

Cannabinoid use in all populations is increasing as legalization across the United States continues. Concerningly, there is a lack of caution provided by medical providers to pregnant individuals as to the impact the use of cannabinoids could have on the developing fetus. Research continues in both the preclinical and clinical areas, and is severely needed, as the potency of delta-9-tetrahydrocannabinol (THC), the primary psychoactive component of cannabis, has increased dramatically since the initial studies were completed. Systemic sclerosis (SSc) is a rare orphan disease, characterized by skin thickening, vascular insufficiency, and fibrosis of internal organs. SSc affects about 100,000 people in the United States. This study explored perceived benefits and challenges of patient partners and stakeholders, who were team members on a project to revise and test a self-management program (Taking Charge of Systemic Sclerosis). Five patient partners, 1 stakeholder from the Scleroderma Foundation and 1 stakeholder from a state chapter of the Scleroderma Foundation were interviewed. Conversations were audio recorded and transcribed verbatim and analyzed. Four themes emerged from the analysis with corresponding subthemes: contributions …

The resident-led micro-mentorship curriculum enhanced the surgical learning environment by fostering goal-oriented mentorship, improving feedback, and increasing student integration into the surgical team. While overall satisfaction and achievement of learning objectives were high, targeted improvements in technical skill feedback and history and physical exam observation are needed to optimize the program’s impact.

Our study demonstrates that a formal communication training initiative can boost faculty self-confidence in conflict resolution skills within the operating room. Using simulations tailored to the unique challenges of an OR setting equip surgeons with immediately transferable skills.

Background and study aims We assessed sessile serrated lesion detection rate (SSLDR) at a large academic medical center from 2008 to 2020 and modeled a local, aspirational target SSLDR. We also assessed SSLDRs among all gastroenterology fellows to better understand the relationship between SSLDRs and total colonoscopies performed.
Patients and methods SSL-positive pathology results were flagged from a dataset composed of all screening colonoscopies for average-risk patients from 2008 to 2020. Unadjusted SSLDRs were calculated for individual endoscopists by year. More information is needed about the impact of outpatient nutrition care from a registered dietitian nutritionist (RDN) on patient outcomes. This study aimed to assess the feasibility of a cohort study design to evaluate impact of RDN nutrition care on patient outcomes, describe clinic malnutrition screening practices, and estimate statistical parameters for a larger study. Seventy-seven patients with lung, esophageal, colon, rectal, or pancreatic cancer from six facilities were included (41 received RDN care and 36 did not). OBJECTIVE: In the management of diabetic ketoacidosis (DKA), the standard of care is to administer insulin glargine after ketoacidosis has resolved and the patient is transitioning from intravenous (IV) insulin to subcutaneous insulin; however, there is evidence to suggest that earlier administration of insulin glargine may accelerate resolution of ketoacidosis. The objective of this research is to determine the efficacy of early subcutaneous insulin glargine on time to resolution of ketoacidosis in children with moderate to severe DKA.
