Drylands cover 45% of the terrestrial surface and are expanding rapidly due to anthropogenic drivers. Altered precipitation regimes, atmospheric nitrogen deposition, and wildfire will likely have significant consequences in these regions where ecological processes are limited by water and nitrogen. In this dissertation, I explored temporal dynamics of net primary production (NPP) and related above- and belowground processes under several environmental change drivers in the Sevilleta National Wildlife Refuge, central New Mexico, USA. Methylation of host and viral genes is promising for triage of women with high-risk human papillomavirus infections (hrHPV). Using a population-based sample of hrHPV positive women with cervical biopsies within 12 months after cervical screening, the clinical value of the S5 methylation classifier (S5), HPV genotyping and cytology were compared as potential triage tests, for outcomes of cervical intraepithelial neoplasia (CIN) grade 3 or greater (CIN3+), CIN2+ and CIN2, and the area under the curve (AUC) calculated. Rare cases of human herpesvirus 8 (HHV8)-negative effusion-based large B-cell lymphoma (EB-LBCL) occur in body cavities without antecedent or concurrent solid mass formation. In contrast to HHV8 + primary effusion lymphoma (PEL), EB-LBCL has no known association with HIV or HHV8 infection. However, the small sample sizes of case reports and series worldwide, especially from non-Japanese regions, have precluded diagnostic uniformity. Therefore, we conducted a retrospective, multi-institutional study of 55 cases of EB-LBCL and performed a comprehensive review of an additional 147 cases from the literature to identify distinct clinicopathologic characteristics. IMPORTANCE: Although classic psychedelic medications have shown promise in the treatment of alcohol use disorder (AUD), the efficacy of psilocybin remains unknown.

OBJECTIVE: To evaluate whether 2 administrations of high-dose psilocybin improve the percentage of heavy drinking days in patients with AUD undergoing psychotherapy relative to outcomes observed with active placebo medication and psychotherapy.

DESIGN, SETTING, AND PARTICIPANTS: In this double-blind randomized clinical trial, participants were offered 12 weeks of manualized psychotherapy and were randomly assigned to receive psilocybin vs diphenhydramine during 2 day-long medication sessions at weeks 4 and 8. The use of therapeutic monoclonal antibodies is constrained because single antigen targets often do not provide sufficient selectivity to distinguish diseased from healthy tissues. We present HexElect, an approach to enhance the functional selectivity of therapeutic antibodies by making their activity dependent on clustering after binding to two different antigens expressed on the same target cell. lmmunoglobulin G (lgG)-mediated clustering of membrane receptors naturally occurs on cell surfaces to trigger complement- or cell-mediated effector functions or to initiate intracellular signaling. The classification of myeloid neoplasms and acute leukemias was last updated in 2016 within a collaboration between the World Health Organization (WHO), the Society for Hematopathology, and the European Association for Haematopathology. This collaboration was primarily based on input from a clinical advisory committees (CACs) composed of pathologists, hematologists, oncologists, geneticists, and bioinformaticians from around the world. The recent advances in our understanding of the biology of hematologic malignancies, the experience with the use of the 2016 WHO classification in clinical practice, and the results of clinical trials have indicated the need for further revising and updating the classification. The high-affinity immunoglobulin E (IgE) receptor, FcεRI, is the primary immune receptor found on mast cells and basophils. Signal initiation is classically attributed to phosphorylation of FcεRI β- and γ-subunits by the Src family kinase (SFK) Lyn, followed by the recruitment and activation of the tyrosine kinase Syk. FcεRI signaling is tuned by the balance between Syk-driven positive signaling and the engagement of inhibitory molecules, including SHIP1. Here, we investigate the mechanistic contributions of Lyn, Syk, and SHIP1 to the formation of the FcεRI signalosome. DESCRIPTION: Coronavirus disease 2019 convalescent plasma (CCP) has emerged as a potential treatment of COVID-19. However, meta-analysis data and recommendations are limited. The Association for the Advancement of Blood and Biotherapies (AABB) developed clinical practice guidelines for the appropriate use of CCP.

METHODS: These guidelines are based on 2 living systematic reviews of randomized controlled trials (RCTs) evaluating CCP from 1 January 2019 to 26 January 2022. There were 33 RCTs assessing 21 916 participants. The results were summarized using the GRADE (Grading of Recommendations Assessment, Development and Evaluation) method. PURPOSE: This is the first multisite investigation of the validity of scores from the current version of the Medical College Admission Test (MCAT) in clerkship and licensure contexts. It examined the predictive validity of MCAT scores and undergraduate grade point averages (UGPAs) for performance in preclerkship and clerkship courses and on the United States Medical Licensing Examination Step 1 and Step 2 Clinical Knowledge examinations. It also studied students' progress in medical school.

Acute lymphoblastic leukemia (ALL) is the most common childhood cancer. Here, using whole-genome, exome and transcriptome sequencing of 2,754 childhood patients with ALL, we find that, despite a generally low mutation burden, ALL cases harbor a median of four putative somatic driver alterations per sample, with 376 putative driver genes identified varying in prevalence across ALL subtypes. Most samples harbor at least one rare gene alteration, including 70 putative cancer driver genes associated with ubiquitination, SUMOylation, noncoding transcripts and other functions. In hyperdiploid B-ALL, chromosomal gains are acquired early and synchronously before ultraviolet-induced mutation. By contrast, ultraviolet-induced mutations precede chromosomal …

Cervical cancer is the fourth most common malignancy in women worldwide. Although infection from human papillomavirus (HPV) has been the leading cause of cervical cancer, HPV-negative cervical cancer accounts for approximately 3-8% of all cases. Previous research studies on cervical cancer have focused on HPV-positive cervical cancer due to its prevalence, resulting in HPV-negative cervical cancer receiving considerably less attention. As a result, HPV-negative cervical cancer is poorly understood. Its etiology remains elusive mainly due to limitations in research methodology such as lack of defined markers and model systems. Intramedullary nailing is an effective treatment of femoral shaft fractures. However, this procedure has several common complications and technical problems that can interfere with full recovery in some patients. The purpose of this review article is to expand on the literature with the authors’ own experiences to summarize the following common complications of femoral nailing: missed injuries, suboptimal surgical timing, malreduction, reaming errors, implant insertion errors, pain-management problems, and implant removal errors. Strategies are also identified to help avoid and manage these complications.

Reaming for intramedullary implants has biomechanical and biologic benefits. However, there are multiple potential pitfalls in the reaming process. Common errors in reaming for intramedullary implants include: over-reaming or under-reaming the intramedullary canal, reamer incarceration, using dull or narrow-fluted reamers, and poor technique. Being aware of these potential problems with reaming helps surgeons avoid intraoperative issues altogether or diagnose their causes as they arise, decrease complications, and improve efficiency in the operating room.

Background: Tranexamic acid (TXA) reduces blood loss by inhibiting plasminogen and preventing subsequent fibrinolysis (stabilizing fibrin clots). Topical TXA has been suggested for use in unicompartmental joint arthroplasty and other procedures with residual viable cartilage. However, the effects on human cartilage have not been thoroughly investigated. The purpose of this study is to determine the viability of human chondrocytes in the setting of various concentrations of topical TXA. Introduction: Various factors including patient demographics, fracture types, and fixation methods can affect the outcomes of distal femur fractures. This study aimed to analyze the risk factors for reoperation after operative fixation of these fractures. Methods: This was an institional review board (IRB) approved retrospective study of extra-articuilar, partial articular, and complete articular distal femur fractures treated with operative fixation at a Level I trauma center between 2008 and 2018. Nursing burnout continues to rise as the COVID-19 pandemic amplifies environmental stressors in the workplace. This paper explores the application of Callista Roy's adaptation model in preventing nursing burnout during the pandemic using four modes of adaptation: rolefunction, interdependence, physiological/physical, and self-concept or group identity. The presented solution to alleviate nursing burnout examines the implementation of Roy's adaptation model through proposed resiliency initiatives, leadership and management modifications, and state legislature regulation. These interventions aim to reduce anxiety, moral distress, and compassion fatigue, along with increasing support for nurses.

This study compared the performance of machine learning models in classifying COVID-19 patients using exhaled breath signals and simulated datasets. Ground truth classification was determined by the gold standard Polymerase Chain Reaction (PCR) test results. A residual bootstrapped method generated the simulated datasets by fitting signal data to Autoregressive Moving Average (ARMA) models. Classification models included neural networks, k-nearest neighbors, naïve Bayes, random forest, and support vector machines. Hydrogen getter materials are used to irreversibly absorb hydrogen gas in sealed environments. The DEB based getter system, despite finding widespread use in this application, still posses challenges with respect to experimental performance analysis and fundamental understanding of reaction mechanism. The second chapter of this report addresses the experimental challenges of this getter system by demonstrating the utility and experimental strengths of a custom magnetic suspension balance (MSB) based gravimetric sorption system. This apparatus enables performance evaluation in process relevant conditions including dilute mixed gas environments which our results indicate is necessary for accurate performance evaluation of getter systems.
