BACKGROUND: Postoperative mortality rate is one of six surgical indicators identified by the Lancet Commission on Global Surgery for monitoring access to high-quality surgical care. The primary aim of this study was to measure the postoperative mortality rate in Tanzania's Lake Zone to provide a baseline for surgical strengthening efforts. The secondary aim was to measure the effect of Safe Surgery 2020, a multi-component intervention to improve surgical quality, on postoperative mortality after 10 months.

RATIONALE & OBJECTIVE: Nutrition management can slow the progression of chronic kidney disease (CKD) and help manage complications of CKD, but few individuals with CKD receive medical nutrition therapy before initiating dialysis. This study aimed to identify knowledge, attitudes, experiences, and practices regarding medical nutrition therapy and barriers and facilitators to medical nutrition therapy access for individuals with CKD stages G1-G5 from the perspective of patients and providers.

The opioid epidemic in the United States has led to a significant increase in the incidence of neonatal opioid withdrawal syndrome (NOWS); however, the understanding of long-term consequences of NOWS is limited. The objective of this study was to evaluate post-discharge healthcare utilization in infants with NOWS and examine the association between NOWS severity and healthcare utilization. A retrospective cohort design was used to ascertain healthcare utilization in the first year after birth-related discharge using the CERNER Health Facts® database. Hereditary deficiencies of protein S (PS) increase the risk of venous thrombosis; however, assessing the plasma levels of PS can be difficult because of its complex physiological interactions in plasma, sample-related preanalytical variables, and numerous acquired disease processes. Reliable laboratory assays are essential for accurate evaluation of PS when diagnosing a congenital deficiency based on the plasma phenotype alone. This report presents the current evidence-based recommendations for clinical PS assays as well as when to test for PS abnormalities.

The purpose of this dissertation is to measure and explain age- and sex-related differences in energy expenditure, physical activity, and patterns of horticultural work, in the Tsimane’, a subsistence forager-horticulturalist population from the Bolivian Amazon. Changes in the way energy is used with advancing age has important implications for the tradeoffs negotiated within the aging body as it partitions increasingly limited energy among competing life functions. Biomphalaria snails serve as vectors for Schistosoma mansoni, a trematode causing human schistosomiasis. Control of Schistosoma mansoni involves chemotherapy of affected people, but new control methods built on improved understanding of schistosome-snail interactions are needed. My dissertation applies molecular and bioinformatics approaches to understand such interactions. Chapter 1 shows that significant differences exist among Biomphalaria species in Africa with respect to their ability to support schistosome development. Chapters 2 and 3 reveal the transcriptional responses of Biomphalaria glabrata susceptible (SUS) or resistant (RES) to Schistosoma mansoni. The objectives of this study were to identify the lactate threshold (LT) and the first ventilatory threshold (VT) during resistance exercise in a discontinuous incremental test, determine if both thresholds were observed at the same relative workload, and to analyze the acute cardiorespiratory and metabolic responses during a separate constant load resistance exercise protocol. Twelve healthy, trained women (n = 6) and men (n = 6) performed a maximal running test on a treadmill followed by three protocols using the hexagonal bar deadlift (HBDL) exercise. The purpose of this study was to explore the meaning of and experiences living with “borderline diabetes” among Mexican American adults residing in or around Amarillo, Texas. Sixteen Mexican Americans who self-identified as having “borderline diabetes” participated. Research questions focused on how participants understood the condition, self-care practices, and recommendations for improving prediabetes healthcare and education. An interpretive description approach was used to analyze data from phone interviews. Principal findings revolved around issues of disclosure vs. secrecy, information seeking, and challenges or “struggle” to change lifestyle, particularly diet. However, information is limited on care cascade outcomes in persons experiencing homelessness now that highly effective medications offer the potential for cure.

This study examined mothers ’reports on types of childcare arrangement for a preschool-age child, levels of mothers’, fathers’, and relatives’ involvement in childcare at home, maternal satisfaction with childcare arrangement, the influence of socioeconomic status and social support on mothers ’satisfaction with childcare arrangement, and the nature of mothers ’affection, responsiveness, encouragement, and teaching behaviors during mother-child play sessions. The sample consisted of 130 New Mexico mothers from two-parent families with a preschool child. Introduction: Digital breast tomosynthesis (DBT) may decrease recall rates (RRs) and improve positive predictive values (PPVs) and cancer detection rates (CDRs) versus full-field digital mammography (FFDM). The value of DBT has not been assessed in New Mexico's rural and minority population. Objectives of this study were to compare RRs, CDRs, and PPVs using FFDM+DBT versus FFDM in screening mammograms at the University of New Mexico between 2013 and 2016 and to qualitatively evaluate patient decision-making regarding DBT. Due to the nursing shortage in Sri Lanka, patients need informal caregivers, known as bystanders, to help provide patient care. This study described bystanders’ perspectives on informal, hospital-based care to bedridden patients with cancer in Sri Lanka. In this qualitative descriptive study, 17 bystanders at Apeksa/Cancer Hospital, Sri Lanka, were recruited using snowballing sampling. Data were collected through telephone interviews and analyzed using content and thematic analyses. Findings showed that paid and unpaid bystanders cover all caregiving tasks with minimum interaction with nurses. Unpaid bystanders described the need for same-sex bystanders and additional help with patients’ mobilization and specialized care. …

Purpose:

This study evaluated the oral health literacy OHL of school staff in Southern Minnesota. OHL is the degree to which individuals can understand health information. Dental caries is the most common chronic disease in school aged children, and school staff members spend a large portion of the day with children. Therefore, it is important for all school staff members to have a moderate or higher OHL status.

Diarrhea is a leading cause of death worldwide because a lack thereof in household sanitization exposes humans to high concentrations of pathogenic Escherichia coli. In 2016, the University of New Mexico’s Nepal Study Center collected cross-sectional survey data using proportional random sampling on three communities in Western Nepal. Structural and parameter learning estimation and approximate inference of Bayesian networks studied diarrheagenic E. coli exposure while incorporating participation in sanitary behaviors, access to sanitary built-in environments, and other human characteristics. Prevalence of Type 2 Diabetes (T2DM) in New Mexico (NM) has steadily increased to 12.5% in 2019.1Registered Dietitian Nutritionists (RDNs) play a critical role in the T2DM healthcare team. A web-based survey was distributed to NM clinicians [Medical Doctors (MD), Doctors of Osteopathic Medicine (DO), Certified Nurse Practitioners (CNP), and Certified Physician Assistants (PA-C)] investigating barriers preventing referral of patients with T2DM to an RDN.

In total, 132 clinicians completed the survey (average age=52 years; 54.5% CNPs, 32.6% MDs, 9.1% PA-Cs, 3.9% DOs; 44% female). Most (67.4%) did not have an RDN employed in their practice. The majority …

Few studies have examined inoculum potential of ectomycorrhizal fungi (EMF) in the absence of plant hosts, yet persistence of these fungi may be paramount to resilience of Pinus edulis and other mycorrhizal plant species. We conducted a study in which seven sites were selected in northwestern New Mexico with known dates of P. edulis extirpation and a lack of regeneration. Age classes included: two sites extirpated 10-20 years ago, two extirpated 55-65 years ago, two extirpated 500+ years ago, and one extirpated 11,000+ years ago. The objective of this quasi-experimental study was to assess whether instructor-prepared mind maps increased knowledge, exam scores and final grades in undergraduate students enrolled in an introductory human nutrition course. Two sections of the same class (one intervention, one control) were taught by the same student instructor/investigator. Mind maps were posted online for the intervention class following each chapter lecture. Students tracked their study methods, including whether they used the mind maps. Chronic health conditions, such as asthma and food allergies, affect a substantial proportion of school-aged children in New Mexico (NM). School health services, policies, programs, and practices can affect daily management of chronic pediatric health conditions. Purpose of Study:

Only a third of US mothers achieve three months of exclusive breastfeeding (BF) despite the American Academy of Pediatrics (AAP) recommendation to exclusively BF for six months. National experts have recently published evidence-based guidelines for outpatient clinics to support longer durations of BF. Our pilot study sought to evaluate the feasibility of an intervention to improve support for BF in outpatient primary care clinics.

Introduction:

The American Academy of Pediatrics recommends food insecurity (FI) screening at all pediatric visits. Despite an existing EMR screening tool, how our pediatric providers screen FI and housing insecurity (HI) is unknown. Our objective was to collect formative data on FI and HI screening practices of UNMH pediatric providers to allow for the design of a quality improvement (QI) project.

Methods:

We performed a baseline cross-sectional survey with UNMH pediatric residents and attendings in May 2020.

Results:

Background:

Malnutrition is often underdiagnosed, and consequently undertreated, in hospitalized patients. A nationwide study is being conducted to validate indicators (the Malnutrition Clinical Characteristics [MCC]) to diagnose malnutrition in hospitalized patients.

Methods:

For the full study, sixty pediatric hospitals will collect patient medical history, patient STRONGKids malnutrition screening score, and nutrition intervention data. Six hundred pediatric patients will be randomly selected from the cohort to be assessed for the MCC and the Nutrition Focused Physical Exam (NFPE). BACKGROUND: Older cancer survivors, faced with both age- and treatment-related morbidity, are at increased and premature risk for physical function limitations. Physical performance is an important predictor of disability, quality of life, and premature mortality, and thus is considered an important target of interventions designed to prevent, delay, or attenuate the physical functional decline. Currently, low-cost, valid, and reliable methods to remotely assess physical performance tests that are self-administered by older adults in the home-setting do not exist, thus limiting the reach, scalability, and dissemination of interventions.

Ankyloglossia, which is characterized by a short, tight or thick lingual frenulum, can negatively impact breastfeeding. It has been recognized as a problem for centuries. Frenotomies have traditionally been performed to remove the lingual frenulum when indicated. The use of frenotomies and a focus on ankyloglossia fell out of favor in the twentieth century with renewed attention in the past several decades. Limited high-quality studies have been undertaken on ankyloglossia. There remains no standardized diagnosis or management recommendations.
