Each year, infections from major foodborne pathogens are responsible for an estimated 9.4 million illnesses, 56,000 hospitalizations, and 1,350 deaths in the United States (1). To evaluate progress toward prevention of enteric infections in the United States, the Foodborne Diseases Active Surveillance Network (FoodNet) conducts surveillance for laboratory-diagnosed infections caused by eight pathogens transmitted commonly through food at 10 U.S. sites. During 2020-2021, FoodNet detected decreases in many infections that were due to behavioral modifications, public health interventions, and changes in health care-seeking and testing practices during the COVID-19 pandemic. This report presents preliminary estimates of pathogen-specific annual incidences during …

Australia's cervical screening program transitioned from cytology to HPV-testing with genotyping for HPV16/18 in Dec'2017. We investigated whether program data could be used to monitor HPV vaccination program impact (commenced in 2007) on HPV16/18 prevalence and compared estimates with pre-vaccination benchmark prevalence. Pre-vaccination samples (2005-2008) (n = 1933; WHINURS), from 25 to 64-year-old women had been previously analysed with Linear Array (LA). Post-vaccination samples (2013-2014) (n = 2989; Compass pilot), from 25 to 64-year-old women, were analysed by cobas 4800 (cobas), and by LA for historical comparability. Age standardised pre-vaccination HPV16/18 prevalence was 4.85% (95%CI:3.81-5.89) by LA; post-vaccination estimates were …

Shiga toxin-producing Escherichia coli (STEC) causes acute diarrheal illness. To determine risk factors for non-O157 STEC infection, we enrolled 939 patients and 2,464 healthy controls in a case-control study conducted in 10 US sites. The highest population-attributable fractions for domestically acquired infections were for eating lettuce (39%), tomatoes (21%), or at a fast-food restaurant (23%). Exposures with 10%-19% population attributable fractions included eating at a table service restaurant, eating watermelon, eating chicken, pork, beef, or iceberg lettuce prepared in a restaurant, eating exotic fruit, taking acid-reducing medication, and living or working on or visiting a farm. Significant exposures with high …

BACKGROUND: Coronavirus disease 2019 (COVID-19) convalescent plasma (CCP) reduces hospitalizations among outpatients treated early after symptom onset. It is unknown whether CCP reduces time to symptom resolution among outpatients.

METHODS: We evaluated symptom resolution at day 14 by trial arm using an adjusted subdistribution hazard model, with hospitalization as a competing risk. We also assessed the prevalence of symptom clusters at day 14 between treatments. Clusters were defined based on biologic clustering, impact on ability to work, and an algorithm.

RESULTS: Among 1070 outpatients followed up after transfusion, 381 of 538 (70.8%) receiving CCP and 381 of 532 (71.6%) receiving …

Electronic cigarette (Ecig) use has become more common, gaining increasing acceptance as a safer alternative to tobacco smoking. However, the 2019 outbreak of Ecig and Vaping-Associated Lung Injury (EVALI) alerted the community to the potential for incorporation of deleterious ingredients such as vitamin E acetate into products without adequate safety testing. Understanding Ecig induced molecular changes in the lung and systemically can provide a path to safety assessment and protect consumers from unsafe formulations. While vitamin E acetate has been largely removed from commercial and illicit products, many Ecig products contain additives that remain largely uncharacterized. In this study, we …

Neuroinflammatory And Metabolomic Temporal Dynamics From Inhaled Wildfire Smoke With Attenuation Via Pharmacological Intervention, David Scieszka

Biomedical Sciences ETDs

Wildfire smoke is a global issue affecting nearly all humans on the surface of this Earth. Smoke dispersion from wildfires can span continents and cross oceans with amazing ease. Furthermore, total acres of land burned per year in the United States has roughly doubled since 1980, meaning this issue will persist for an indeterminate amount of time. Exposure to wildfire smoke is hazardous and promotes aging-associated events in the cardiopulmonary systems. Yet long- and short-term effects on the brain remain relatively unknown. From these, it is critically important to understand the neurological impacts from wildfire smoke exposure; to compare these …

Therapeutic plasma exchange (TPE) is a therapeutic intervention that separates plasma from blood cells to remove pathological factors or to replenish deficient factors. The use of TPE is increasing over the last decades. However, despite a good theoretical rationale and biological plausibility for TPE as a therapy for numerous diseases or syndromes associated with critical illness, TPE in the intensive care unit (ICU) setting has not been studied extensively. A group of eighteen experts around the globe from different clinical backgrounds used a modified Delphi method to phrase key research questions related to "TPE in the critically ill patient". These …

Saving The Microbiome One Phlegm At A Time: Sputum Collection In The Ed For Patients Admitted With Pneumonia, Joanna Watterson, Jessica Mitchell, Elyse Sheehan, Meghan Brett

Quality Improvement/Patient Safety Symposium

Problem Identification

Patients brought into the Emergency Department (ED) with pneumonia who require admission often receive broad-spectrum antibiotics, due to high acuity and desire to cover an array of possible pathogens. While this is reasonable, broad-spectrum antibiotics come with potential harms to both the individual patient and the community at large, and efforts should be made to narrow coverage as soon as possible, usually informed by culture data and sensitivities. To this end, IDSA guidelines for hospital acquired pneumonia currently recommend antibiotic treatment based on culture data, over empiric treatment[i]. However, culture data takes time. Therefore, antibiotic stewardship …

Improved Tracking And Referrals For Vision And Dental Care For Patients With Hiv, Kory Tillery, Michelle Iandiorio

Quality Improvement/Patient Safety Symposium

BACKGROUND

Regular vision and dental care are important components of healthcare for persons with HIV (PWH). HIV infection is associated with a wide array of complications for both ocular and oral health regardless of severity of HIV disease. Ocular disease can range from opportunistic or associated infections (i.e cytomegalovirus retinitis, ocular syphilis); to inflammatory or degenerative disease (i.e retinal vasculopathy, optic neuropathy), or drug effects. Oral disease similarly encompasses infectious and noninfectious etiologies, including opportunistic infections (i.e., candidiasis, oral hairy leukoplakia), comorbid infections (i.e. herpes simplex virus, human papilloma virus, periodontitis/gingivitis), or inflammatory conditions (i.e. salivary gland disease). The Health …

Examining The Relationship Between Anxiety And Depression Symptoms, Protective Behavioral Strategies And Opioid Use Outcomes, Melissa Rose Hatch

Psychology ETDs

Protective behavioral strategies (PBS), behaviors individuals use to reduce negative consequences of substance use, have been shown to mediate and moderate the relationship between mental health and substance use outcomes. The present study aimed to examine the relationships between anxiety and depression symptoms and opioid PBS on risky opioid use and opioid-related consequences. Participants in this secondary data analysis were non-treatment seeking individuals who reported past-month opioid use (n=257). Structural equation modeling was used to examine 1) PBS as a mediator of the relationship between anxiety/depression symptoms and opioid use outcomes, and 2) anxiety/depression symptoms as a moderator …

Genomic Sequencing Of An Atypical Outbreak Of Human Metapneumovirus In A Pediatric Population Reveals The Co-Circulation Of Multiple Genetic Lineages, Joshua Kennedy, Jesse Young, Kurt Schwalm, Daryl Domman, Darrell Dinwiddie

2023 Pediatric Research Forum Session

Human metapneumovirus (HMPV) is a common cause of respiratory infections in children, accounting for 5-15% of all acute respiratory infections in this population. Although HMPV is closely related to respiratory syncytial virus (RSV) its incidence, epidemiology, and genetic diversity are poorly understood. In the United States, HMPV infections typically occur annually in the winter and early spring months. Similar to other common respiratory viruses, during the first two years of the COVID-19 pandemic, cases of HMPV were nearly absent. However, in the summer and early fall of 2022 an atypical and dramatic increase of HMPV cases across the United States …

BACKGROUND: Estimating the economic costs of self-injury mortality (SIM) can inform health planning and clinical and public health interventions, serve as a basis for their evaluation, and provide the foundation for broadly disseminating evidence-based policies and practices. SIM is operationalized as a composite of all registered suicides at any age, and 80% of drug overdose (intoxication) deaths medicolegally classified as 'accidents,' and 90% of corresponding undetermined (intent) deaths in the age group 15 years and older. It is the long-term practice of the United States (US) Centers for Disease Control and Prevention (CDC) to subsume poisoning (drug and nondrug) deaths …

Care Through Connection: Patient Mutuality And Working Alliance Curriculum, N Mariam Salas, Kathryn Lenberg

HSC Education Days

Background: There is clear evidence in the psychology literature that beyond “people skills,” providers who give the patient an active role in developing the treatment plan can increase the adherence to the medical plan by 70%. Though this is utilized extensively in psychology, medical trainees are not routinely taught these communication skills. To arm our fellows with these necessary skills, we worked with our clinical psychologist, Dr. Kathryn Lenberg, to develop an integrated curriculum over the 2-year period of the fellowship. Implementation: We began a phased roll out of the curriculum in 2020. During elective rotations, fellows spent 1 hour …

The Human Gut Microbiome, Tammy J. Ladue

InterProfessional Education Research and Scholarship

Although the gut microbiome has been known for many years, it wasn’t until the mid-1970’s that a relationship was hypothesized in medical journals. With the advent of the Human Genome Project in 2001 (Adak & Khan, 2019) further investigation into the types and relationship of the microbes in the GI tract were seriously studied.

Utilizing DNA & RNA tracking and culture techniques found during the Genome Project, more information was found on the Gut Microbiome. It was as of 2017 that the knowledge on the Microbiome began to show promise. It has been speculated that the GI tract has approximately …

Wild Cards In Natural Resource Planning: High Impact Surprises That Shape The Future, Dr. David Bengston Jan 2023

Wild Cards In Natural Resource Planning: High Impact Surprises That Shape The Future, Dr. David Bengston

Natural Resources Journal

Wild cards are improbable, high impact events. Such events, like the COVID-19 pandemic, have the ability to shape policy by upending entire systems and paradigms. Unfortunately, long-term planning and policymaking rarely account for wild card events. Instead, planning and policymaking typically assume that existing trends will continue without major disruption. Moreover, adequately planning for potential wild card events is challenging because they are inherently difficult to predict. However, planning for wild card events in natural resources and environmental policymaking is vital as global developments, such as climate change, create more extreme conditions and challenges. This review, (1) defines and characterizes …
