Context.—: Autopsy pathologists, including medical examiners, provide valuable public health support for infectious disease deaths through surveillance for deaths of public health concern including emerging infections, identifying causative organisms for unexplained deaths, and providing insights into the pathology and pathogenesis of novel or unusual infections. However, autopsy poses biosafety risks to workers within and outside the laboratory. The highest rates of laboratory-acquired infections occur in autopsy workers.
Objective.—: To design and construct an appropriately biosafe autopsy laboratory.

BACKGROUND: Convalescent plasma is being used widely as a treatment for coronavirus disease 2019 (COVID-19). However, the clinical efficacy of COVID-19 convalescent plasma is unclear.
METHODS: The Passive Immunity Trial for Our Nation (PassITON) is a multicenter, placebo-controlled, blinded, randomized clinical trial being conducted in the USA to provide high-quality evidence on the efficacy of COVID-19 convalescent plasma as a treatment for adults hospitalized with symptomatic disease. Adults hospitalized with COVID-19 with respiratory symptoms for less than 14 days are eligible. Enrolled patients are randomized in a 1:1 ratio to 1 unit (200-399 mL) of COVID-19 convalescent plasma that has …

Compassion fatigue (CF), its components of burnout (BO) and secondary traumatic stress (STS), and compassion satisfaction (CS) affect healthcare providers and staff and can dramatically influence patient care and access to primary care services. Resilience is a tool which serves as a protective factor that can mitigate CF and enhance CS. There is a gap in research regarding the relationship between compassion fatigue, compassion satisfaction, and resilience among primary care providers and frontline staff. The 2019 novel coronavirus disease (COVID-19) has spread worldwide, infiltrating, infecting, and devastating communities in all locations of varying demographics. An overwhelming majority of published literature on the pathologic findings associated with COVID-19 is either from living clinical cohorts or from autopsy findings of those who died in a medical care setting, which can confound pure disease pathology. A relatively low initial infection rate paired with a high biosafety level enabled the New Mexico Office of the Medical Investigator to conduct full autopsy examinations on suspected COVID-19-related deaths. In order to achieve the global elimination of cervical cancer as a public health problem, close surveillance of progress in public health and clinical activities and outcomes across the three pillars of vaccination, screening and treatment will be required. Surveillance should ideally occur within an integrated system that is planned, funded, and regularly evaluated to ensure it is providing timely, accurate and relevant feedback for action. In this paper, we conceptualise the main public health surveillance objectives as process and outcome measures in each of the three pillars. Automation in clinical microbiology is starting to become more commonplace and reportedly offers several advantages over the manual laboratory. Most studies have reported on the rapid turnaround times for culture results, including times for identification of pathogens and their respective antimicrobial susceptibilities, but few have studied the benefits from a laboratory efficiency point of view. This is the first large, multicenter study in North America to report on the benefits derived from automation measured in full-time equivalents (FTE), FTE reallocation, productivity, cost per specimen, and cost avoidance. BACKGROUND: Suicides by any method, plus 'nonsuicide' fatalities from drug self-intoxication (estimated from selected forensically undetermined and 'accidental' deaths), together represent self-injury mortality (SIM)-fatalities due to mental disorders or distress. SIM is especially important to examine given frequent undercounting of suicides amongst drug overdose deaths. We report suicide and SIM trends in the United States of America (US) during 1999-2018, portray interstate rate trends, and examine spatiotemporal (spacetime) diffusion or spread of the drug self-intoxication component of SIM, with attention to potential for differential suicide misclassification.

Clinicians in medically underserved communities encounter both community-wide health issues and community strengths. Together these raise the opportunities and responsibilities for clinicians to take action outside the clinical exam room to improve health in the community. Rural and Urban Underserved Program (RUUP) Scholars develop skills through engagement with communities in: 1) Health related advocacy, 2) Volunteer clinical care, 3) Health education pipeline support, 4) Non-clinical public health based community work, and/or 5) Community health education. The AAMC recommended in March 2020 that medical schools suspend medical student involvement in clinical activities due to CoVid. In response, UNM SOM implemented a 4-week virtual course on COVID-19 for second- and third-year medical students and PA students. The course addressed the need for students to learn about public health, infectious disease, and social determinants of health. In addition, the new course provided ample opportunity for community service. The objectives of the course were lofty, but critical at this point in the students’ medical education. The U.S. Census Bureau predicts that Hispanic, Black and other minority groups will constitute more than half of the U.S. population by 2044. It is therefore problematic that only about 10% of images in dermatology textbooks illustrate dermatologic conditions in dark skin. This paucity of adequate resources has affected physicians’ education and patient outcomes. A study several years ago found 47% of dermatologists and residents reported that their medical training on pathologies in Black skin was insufficient, and that there was a need for more training materials exhibiting Black skin. U.S. HPV vaccine uptake remains below the Healthy People 2030 goal of 80% series completion. Parental concerns and misinformation about the efficacy and safety of the Human Papillomavirus (HPV) vaccine remain, and may be addressed by digital interventions tailored to their concerns. Reported here are results from a small scale randomized trial testing a mobile web app for parents and their adolescent daughters (ages 11-14 years) encouraging HPV vaccination in New Mexico, an ethnically-diverse U.S. state.

The opioid epidemic in the United States has led to a significant increase in the incidence of neonatal opioid withdrawal syndrome (NOWS); however, the understanding of long-term consequences of NOWS is limited. The objective of this study was to evaluate post-discharge healthcare utilization in infants with NOWS and examine the association between NOWS severity and healthcare utilization. A retrospective cohort design was used to ascertain healthcare utilization in the first year after birth-related discharge using the CERNER Health Facts® database. Many published accounts have shown that quality improvement (QI) initiatives within medical practice settings can increase vaccination rates. Project ECHO is a telementoring platform that uses video conferencing technology to educate and support healthcare professionals through case-based learning and brief lectures. This manuscript explores the results of a learning collaborative focused on combining QI and Project ECHO to increase human papillomavirus (HPV) vaccination rates within pediatric practices.

Coccidioidomycosis is endemic to the Southwestern United States as well as parts of Mexico, Central, and South America. Coccidioides infection is typically subclinical or mild and self-limited. Disseminated coccidioidomycosis is uncommon except in immunosuppressed, post-transplant, or pregnant patients. Disseminated coccidioides in immunocompetent individuals is rare. We report a case of a young, apparently immunocompetent, individual who developed disseminated coccidiomycosis diagnosed at the time of a bedside splenectomy for abdominal decompression. Clinical and histological findings are discussed with review of literature.

The purpose of this dissertation is to measure and explain age- and sex-related differences in energy expenditure, physical activity, and patterns of horticultural work, in the Tsimane’, a subsistence forager-horticulturalist population from the Bolivian Amazon. Changes in the way energy is used with advancing age has important implications for the tradeoffs negotiated within the aging body as it partitions increasingly limited energy among competing life functions. Biomphalaria snails serve as vectors for Schistosoma mansoni, a trematode causing human schistosomiasis. Control of Schistosoma mansoni involves chemotherapy of affected people, but new control methods built on improved understanding of schistosome-snail interactions are needed. My dissertation applies molecular and bioinformatics approaches to understand such interactions. Chapter 1 shows that significant differences exist among Biomphalaria species in Africa with respect to their ability to support schistosome development. Chapters 2 and 3 reveal the transcriptional responses of Biomphalaria glabrata susceptible (SUS) or resistant (RES) to Schistosoma mansoni. Group B Streptococcus (GBS) can be found to colonize about 25% of all healthy, adult women and is the leading infectious cause of early neonatal morbidity and mortality in the United States. This study evaluated the clinical performance of PhenoMatrix (PM) chromogenic detection module (CDM) digital imaging software in detection of GBS from LIM broth plated on ChromID Strepto B chromogenic medium (ChromID) using the WASP automated processor. The performance of the PM CDM was compared to manual culture review of the digital images and molecular detection of GBS. ChromID alone had a sensitivity and specificity of 84.5% and 94.7%, …

Background: Depression and anxiety disorders, believed to be caused by stress-related neuropeptides and pro-inflammatory molecules in the brain, are prevalent comorbidities of fetal alcohol spectrum disorders (FASD).
Methods: Brain tissue was collected following a 4-hour maternal separation stress during the early postnatal period, postnatal days 10 and 12, from mice subjected to 1st and 2nd trimester prenatal alcohol exposure (PAE).
Hypothesis: We hypothesized that PAE leads to glucocorticoid receptor insensitivity, stress molecule overexpression, and potentiated toll-like receptor 4 (TLR4) pathway inflammatory responses in the brain following early life stress.
