Objectives

The healthcare industry continues to have a high number of reported injuries. The purpose of this study was to 1) characterize healthcare industry injuries by patient contact status; 2) identify the occupations associated with healthcare injuries by patient contact status; and 3) determine healthcare injury rates by occupation, to gain a better understanding of healthcare industry compensated injuries , and better target healthcare industry safety practices and programs.

The purpose of this study was to develop and evaluate the flow rate, spray pattern, and droplet spectra characteristics of an actively controlled variable-orifice nozzle at constant carrier pressures. A commercially available variable-orifice nozzle (VariTarget) was modified to allow for direct electromechanical control of the metering stem. The modified system was tested at five carrier pressures ranging from 138 to 414 kPa and five metering stem (and thus orifice) positions. The metering stem position range was chosen because it provided a linear response in flow rate at each carrier pressure. Flow rate testing indicated a turndown ratio of 2.4:1 at …

Breast cancer is the most common cancer diagnosis of women, with an estimated 232,670 new cases in 2014. With 89.2% of breast cancer patients surviving five years or longer, studies are needed to investigate the long-term impact of breast cancer on women and families (National Cancer Institute, 2014).

The purpose of this study was to examine, using a mixed methods approach, the impact of chemotherapy on mothering occupations for patients diagnosed with breast cancer. This cross sectional study examined problem solving by typically developing children on the Rapid Assessment of Problem Solving test (RAPS). The RAPS, a modification of Mosher and Hornsby’s 20Q task, requires the examinee to solve three problems. Each problem involves asking yes/no questions to identify a target picture from a 32-picture array with as few questions as possible. Participants were 73 young (ages 7-9), 79 early adolescent (ages 10-13) and 77 adolescent (ages 14-17) children residing in Kentucky. Autism Spectrum Disorder (ASD) is a low-incidence disorder with high impacts on individuals, families, and society. School-based speech-language pathologists (SLPs) have tremendous responsibilities toward individuals with ASD, but pre-service SLPs are not adequately trained to fulfill these expectations. In order to reduce the widespread financial and social impact of ASD, school-based SLPs need to complete effective training to prepare them for the selection of established social-communication practices. Darwin’s evolutionary theory provided, for some atheist and agnostic authors in Victorian England, a theory of kinship and community, of investment in the world, that had been missing before. Without a “creation” story that could match the Biblical version, those who stood outside the dominant Christian paradigm rarely had the words or concepts to construct their own visions of how humans fit into the existence of other species, into landscapes, and into a world that, if unfallen, seemed resistant to other explanations. Those who did construct alternate mythologies usually reared them on a Christian base.

Shelled yellow corn is commonly stored in concrete or corrugated steel bins. Granular materials compact under their own weight, primarily due to particle rearrangement, leading to an increase in bulk density and a change in volume when stored. Reliable grain pack factors are needed to estimate storage capacities and to accurately monitor grain inventories. Three Patients Needing High Doses Of Valproic Acid To Get Therapeutic Concentrations, James Jackson, Betsy Mccollum, Judy Ognibene, Francisco J. Diaz, Jose De Leon

Valproic acid (VPA) can autoinduce its own metabolism. Cases requiring VPA doses >4000 mg/day to obtain therapeutic plasma concentrations, such as these 3 cases, have never been published. Case 1 received VPA for seizures and schizophrenia and had >50 VPA concentrations in 4 years. A high dose of 5,250 mg/day of VPA concentrate was prescribed for years but this dose led to an intoxication when switched to the enterocoated divalproex sodium formulation, requiring a normal dose of 2000 mg/day. VPA metabolic capacity was significantly higher (t = -9.6; df = 6.3, p < 0.001) during the VPA concentrate therapy, possibly due to autoinduction in that formulation. Case 2 had VPA for schizoaffective psychosis with 10 VPA concentrations during an 8-week admission. To maintain a VPA level ≥50 μg/mL, VPA doses increased from 1500 …

Maintaining alertness is a well-known benefit and in addition, the cardiovascular (CV) effects of the active compounds, which include polyphenols and caffeine, must be considered. This work describes the fascinating interplay between two tumor suppressors Prostate apoptosis response-4 (Par-4) and p53. The guardian of the genome, p53, is frequently mutated in human cancers, and may contribute to therapeutic resistance. However, p53 is intact and functional in normal tissues, and we observed that specific activation of p53 in normal fibroblasts could induce apoptosis selectively in p53-deficient cancer cells. This paracrine apoptotic effect was executed by Par-4 secreted in response to p53 activation. Accordingly, activation of p53 in wild-type mice, but not in p53-/- or Par-4-/- mice, caused systemic elevation of Par-4 that induced apoptosis …

INTRO Breast cancer is the most common type of malignancy among women in the United States (U.S.) and the second leading killer. Despite the availability of effective breast screening tools, a disparity has formed. Specifically, white women over the age of 40 experienced a higher incidence rate of breast cancer, while African American women experience higher related mortality rate. Purpose: According to CDC statistics, prior to pertussis vaccine development in the 1940’s, about 200,000 children contracted pertussis annually. Today, although this number has decreased drastically, pertussis cases began trending upward again in the 1980’s. There are several proposed contributory factors, including vaccination exemption. In Kentucky, there are both medical and religious exemptions to mandatory school vaccines. The purpose of this study was to examine possible correlations between reported school vaccination exemptions and pertussis incidence in Kentucky counties between the years 2004-2012.

This study examines the demographic disparities in children with externalizing behavior disorders, such as oppositional defiant disorder and conduct disorder, utilizing secondary data from the 2011/2012 National Survey of Children’s Health (NSCH). Current literature suggests that children with externalizing behavior disorders are more likely to be male, live in a low-­‐income family, receive lower grades in school, and to develop later conditions such as depression. Background: Young workers have been more vulnerable to fatal and nonfatal work injuries. To gain a comprehensive understanding of the injuries that led to fatalities in younger workers, the goal of this study was to compare characteristics of young worker fatal injuries in those aged 16-24 to all workers aged 25 and older in Kentucky using a descriptive analysis.

Methods: A retrospective analysis of fatalities among workers under the age of 25 compared to workers aged 25 and older was performed over a ten-year period from 2005-2014 using Kentucky Fatality Assessment and Control Evaluation (FACE) program data.

Background/Objectives

Evidence suggests working in horse barns is associated with adverse respiratory effects among Latino thoroughbred workers, yet no studies to date have measured pulmonary function. We examined the prevalence of self-reported respiratory symptoms, abnormal pulmonary function, and associated occupational factors among Latino thoroughbred farmworkers.

Methods

Participants were recruited via a community-based, purposive sampling strategy and participated in an interview-administered survey and spirometer test. Surveys and spirometry tests were administered to 80 participants by two trained health promoters (Promotoras) in July-September 2014. Objectives. This study investigated the effects of an injury prevention program, specific to work related musculoskeletal disorders (WMSDs), which placed employees into jobs based on their physical abilities compared to the physical demands of their prospective jobs.

Methods. Employee injury data (N=3550) from a large auto manufacturer in the U.S. was analyzed to examine changes in injury rates for employees that were hired pre versus post-strategic placement. Bivariate and multivariate analyses were used to determine dependence between placement, injury within 120 days of hire and gender.

As part of the EPA’s initiative to reduce the hypoxic zone in the Gulf of Mexico, a feasibility study for a potential water quality trading (WQT) program in the Kentucky River Watershed (KRW) was conducted. The objectives of these studies was to evaluate the effects of nucleotide (NT) supplementation of sow diets during late gestation and lactation and in nursery diets to elucidate the potential value of nucleotides for a variety of biological responses.

The studies conducted in the university research facilities showed that supplying nucleotides to sow diets during late gestation and throughout lactation had no effect on routinely measured reproductive responses or piglet growth postweaning. Nucleotides in the nursery diet had no effects on growth performance and intestinal morphology. However, the LPS-challenged pigs that consumed NT diets tended to lose less weight and …

Reproductive success depends on a functional oviduct for gamete storage, maturation, fertilization, and early-conceptus development. The ovarian-derived sex steroids estradiol and progesterone are known to affect functionality of the oviduct. Advances in microarray and NanoString technology allow for gene expression analysis to increase understanding of processes critical for fertility. Studies were conducted to investigate mechanisms regulating oviductal function in cattle and mice by using the Bovine Gene 1.0 ST array and the Mouse Gene 430-2.0 arrays (Affymetrix Inc., CA), respectively.

Acute phase responses (APR) and circadian rhythm of body temperature (CRT) changes to a minor, chronic stressor (endophyte) and major, acute stressor, lipopolysaccharide (LPS) were evaluated in two experiments (E1:22 heifers; E2:24 heifers). Vaginal (E1 and E2) and rectal (E2) temperature probes recorded body temperature. Temperature was analyzed by cosinor analysis in each of 4 Phases: pre-endophyte (P1), endophyte exposure, pre-LPS (P2), post-LPS spike 1 (P3), post-LPS spike 2 (P4). During P1 of E1, amplitude (P=0.05), but not mean temperatures (P>0.10), differed across days (P=0.05). Endophyte affected E2 vaginal temperature amplitudes in P2 (P=0.07) and P4 (P=0.08) without influencing …

Effects of dietary supplementation with low levels of organic sources of trace minerals in place of normal levels of their inorganic salts and phytase on growth performance and mineral metabolism were evaluated in two studies using pullets of white and brown shell laying strains. The organic sources were proteinates of copper, iron, manganese and zinc and selenium yeast. A corn-soybean meal diet was fed alone, plus inorganic minerals or plus organic minerals, and with or without phytase in a 3 x 2 factorial arrangement. Objective: To examine the association between stressful life events during pregnancy and change of maternal smoking behaviors.

Methods: We used data from the CDC’s Pregnancy Risk Assessment Monitoring System (PRAMS) to conduct the research. Our data included 24 states from 2011-2012. We considered 13 stressful life events and grouped them into four categories: emotional, partner-related, financial and traumatic stressors. Change of Maternal Smoking during pregnancy was defined by smoking at the last trimester of pregnancy versus smoking at 3 months prior to smoking. The United States health care system is currently experiencing an increase in demand for services due to an aging population. Rheumatology, an Internal Medicine sub-specialty, treats those patients with arthritic conditions as well as other autoimmune and inflammatory conditions. A shortage of rheumatologists is projected by 2025, adding additional stress to an already burdened clinical area.This means patients can face substantial wait times when requesting consultation with a rheumatology provider.This capstone project will present three manuscripts that explore the referral process from primary to specialty care and strategies to reduce wait times.
