Medication deprescribing is a targeted strategy to reduce inappropriate polypharmacy. Patients in primary care settings have reported high willingness to discontinue medications, but this phenomenon is less well studied in hospital settings. In this explorative post hoc analysis of two previously described clinical trials, we assessed patients' attitudes towards deprescribing in two hospital-based cohorts: acutely admitted older medical patients (≥ 65 years) in the emergency department at Copenhagen University Hospital Hvidovre (October 2018–April 2021) and multimorbid patients (≥ 18 years) referred to an outpatient clinic at Copenhagen University Hospital Amager (March 2019–December 2019). Among 104 and 72 patients in each …

Multi-Jurisdictional Hazard Mitigation Plan For Landslides For The Kentucky River Area Development District, Kentucky, Hudson J. Koch, Matt M. Crawford, Jason M. Dortch Apr 2026

Multi-Jurisdictional Hazard Mitigation Plan For Landslides For The Kentucky River Area Development District, Kentucky, Hudson J. Koch, Matt M. Crawford, Jason M. Dortch

Contract Reports--KGS

Landslides are a widespread and complex geologic hazard that affects people and infrastructure. The Kentucky River Area Development District, Kentucky, sustains relatively high landslide activity that impacts several municipalities in eastern Kentucky. This hazard mitigation planning project outlines landslide hazard and risk assessments for the Kentucky River Area Development District, with information needed for each community in the region to develop mitigation strategies that will support infrastructure and development needs, land-use planning, situational awareness, and response and recovery actions for the communities.

Animal ecologists frequently quantify variance in hierarchically structured traits in wild populations. Importantly, phenotypic plasticity within the period of measurement can modify the trait of interest in response to various unmeasured, temporally or spatially changeable, environmental conditions. Non-random sampling among units of the random effect (e.g. individuals) regarding the environment at issue may lead to estimates of the variance among ((Formula presented.)) or within ((Formula presented.)) such units that conflate several types of processes. This mixing of underlying biology can affect interpretations of the random effect variance. Here, we explore the conditions leading to this situation and assess potential solutions …

Anterior cruciate ligament reconstruction (ACLR) leads to profound muscle atrophy and weakness that remain resistant to rehabilitation. Although early recovery typically involves a brief period of limb unloading, the degree to which disuse alone accounts for muscle pathology after ACLR remains unclear. Here, we leveraged publicly available RNA-seq datasets of muscle biopsies from vastus lateralis obtained 7 days after ACLR or 10 days after unilateral lower limb suspension (ULLS), each with matched control limbs, to directly compare disuse-driven and ACLR-specific early transcriptional responses. Despite similar periods of reduced loading, substantial transcriptomic divergence was identified using both intersection and interaction bioinformatic …

INTRODUCTION: This study evaluates plasma-based proteomic profiles for predicting amyloid positivity in adults with Down syndrome (DS) and examines the impact of apolipoprotein E ε4 (APOE ε4) on test performance. METHODS: Cross-sectional data from 290 adults with DS were analyzed using single molecule array (SIMOA) technology to measure plasma amyloid beta (Aβ)42, Aβ40, neurofilament light chain (NfL), glial fibrillary acidic protein (GFAP), tau phosphorylated at threonine 181, and total tau. Amyloid burden was quantified using Pittsburgh Compound B and (18)F-florbetapir Aβ positron emission tomography. Support vector machine analyses were conducted with biomarkers as predictors and age, sex, and APOE ε4 …

Background: Almost all correlation measures currently available are unable to directly handle missing values. Typically, missing values are either ignored completely by removing them or are imputed and used in the calculation of the correlation coefficient. In either case, the correlation value will be impacted based on the perspective that the missing data represents no useful information. However, missing values occur in real datasets for a variety of reasons. In metabolomics datasets a major reason for missing values is that a specific measurable phenomenon falls below the detection limits of the analytical instrumentation (left-censored values). These missing data are not …

Background/Objectives: Neurodegenerative diseases like Alzheimer’s, Parkinson’s, and Amyotrophic lateral sclerosis (ALS) share common molecular pathways, including neuroinflammation and oxidative stress, which complicate the effectiveness of single-target treatments. Garcinia mangostana L. (mangosteen) has shown neuroprotective properties, but previous studies focused on lipophilic xanthones, which have poor bioavailability and uncertain blood–brain barrier permeability. Methods: In the current study, polar metabolites from G. mangostana peel aqueous extract (GMPE) were assessed for potential multi-target interactions via UHPLC-QTOF-MS-based metabolomics, systems pharmacology, and molecular docking analysis. Further, in silico ADMET screening and network-based analyses assessed for overlap between GMPE compounds and genes associated with neurodegeneration (AD, …

Chronic ethanol (EtOH) consumption is a major contributor to multi-organ dysfunction, yet its systemic effects remain incompletely understood. To address this, we utilized a physiologically relevant long-term mouse model, administering 20% EtOH in drinking water for 60 weeks, to investigate the integrated consequences of chronic exposure. EtOH-consuming mice (0.4–0.5 mL/day) exhibited > 30% reductions in chow and fluid intake, resulting in a 12% decrease in total caloric intake compared to controls (p < 0.001). Body mass remained similar until Week 52, after which EtOH-treated mice had lower body mass due to reductions in both lean and fat mass (p ≤ 0.004). Functional assessments revealed impaired treadmill endurance (−17%) and grip strength (−11%) (p ≤ 0.037), while motor coordination remained unaffected (p = 0.203). Chronic EtOH exposure significantly altered gut microbiota composition, reducing Lactobacillus and enriching Faecalibaculum, Clostridium, and Bifidobacterium at the genus level. These changes were accompanied by marked depletion of short-chain fatty acids (p ≤ 0.05). Indirect markers of gut permeability (serum LPS & zonulin) and liver injury (serum ALT & AST, hepatic amyloid content) were elevated, alongside increased total cholesterol and > 62% upregulation of hepatic TNFα, IL-6 & serum amyloid A (p ≤ 0.046). EtOH also induced dyslipidemia and glucose intolerance (p ≤ 0.041), although transcriptomic changes in white adipose …

Aim This study aimed to assess the oral health status of individuals with schizophrenia and explore its association with behavioural and pharmacological factors, with particular focus on long-term antipsychotic treatment and cumulative anticholinergic burden. Methods A total of 153 adults with schizophrenia (18–65 years) from the Mental Health Network of Bizkaia (Spain), all under antipsychotic treatment for ≥12 months, were evaluated and compared with 153 controls from the general population. Data on socio-demographic variables, tobacco use and oral hygiene habits were collected. Oral health was assessed using the Decayed, Missing and Filled Teeth (DMFT) index and the Community Periodontal Index …

Generative Models And Synthetic Data In Clinical Prediction Models, Anthony A. Mangino, Taha Ahmed, Vincent L. Sorrell

Radiology Faculty Publications

Introduction: Reproducibility, consistency, and transparency are essential to responsible and ethical scientific inquiry, though practices supporting these qualities are often neglected. However, in many cases data are confidential or otherwise unable to be shared publicly. This tutorial describes a method utilizing generative adversarial networks (GANs) to create synthetic data that are sufficiently similar to the original dataset in such cases where the source data cannot be shared or where the source data are too sparse as to internally validate results. Methods: Utilizing an exemplar study that aimed to create a clinical prediction model employing a novel echocardiographic measurement to differentiate …
