Patient expectations have been shown to be a major predictor of outcomes. Furthermore, fulfilled expectations have been linked to increased patient satisfaction and rehabilitation adherence. Expectations may be influenced by a variety of factors, including patient characteristics, pre-operative function, or disease characteristics. However, it is currently unknown what factors and to what degree they may influence patient expectations prior to knee surgery. Furthermore, understanding the importance and values of those expectations for recovery using qualitative methods has not previously been conducted in this patient population.

Diabetes has reached epidemic levels, to the currently estimated 29 million individuals who are living with diabetes. Those with diabetes must manage their disease through a combination of medication, physical activity recommendations, and nutritional guidelines. The consequences of non-adherence to recommendations include cardiovascular disease, kidney failure, vision loss, or ultimately, death. Despite the risks of non-adherence, individuals often do not adhere to recommended treatment. Researchers have attempted to identify strategies to promote diabetes self-management adherence, thereby decreasing complications related to the disease.

The purpose of this research was to identify the composition of soils on firefighter turnout gear from the Philadelphia Fire Department and to determine which soils contain hazardous materials. The objective of the research was to identify the composition of soils remaining on the firefighter turnout gear to enable the industry to design an effective cleaning procedure for removal of soils. Cervical stenotic myelopathy (CSM) is an important musculoskeletal and neurologic disease of the horse. Clinical disease occurs due to malformations of the vertebrae in the neck causing stenosis of the cervical vertebral canal and subsequent spinal cord compression. The disease is multifactorial in nature, therefore a clearer understanding of the etiology and pathogenesis of CSM will allow for improved management and therapeutic practices. This thesis examines issues of equine CSM diagnosis, skeletal tissue pathology, and inherited genetic determinants utilizing advances in biomedical imaging technologies and equine genomics. Bacterial endosymbionts can have profound impacts on their host’s ecology. Notably, endosymbionts can protect their hosts against natural enemies and influence host plant interactions. The endosymbiont Candidatus Arsenophonus infects a wide taxonomic range of arthropod hosts, and is suspected of an uncharacterized mutualistic role in hemipterous insects. In the soybean aphid, Aphis glycines, an introduced pest of soybeans in the United States, Arsenophonus is the sole facultative endosymbiont. The focus of this dissertation is to characterize the role of Arsenophonus in the aphid, with an overall emphasis on its impact on aphid management strategies.

Hemlock woolly adelgid is an invasive herbivore causing extensive mortality of eastern hemlock, an important foundation species that provides stable conditions influencing biological communities. Hemlock is often found in riparian areas and following its decline, broadleaved species, including birch, beech, and rhododendron, will replace it. These plants differ from hemlock in patterns of canopy cover and leaf properties, which influence conditions and resources within streams.

According to optimal foraging theory, generalist predators, such as spiders, are thought to feed indiscriminately on prey according to its availability, especially when food is scarce. In contrast, generalists can display selective feeding decisions under regimes of high prey abundance, but few studies have tracked changes in prey choice on a seasonal basis under open field conditions. Additionally, adaptations to surviving winter have been largely ignored in the research of foraging behavior. Current policies and interventions have aimed to improve intake by improving access to fruits and vegetables. Gestational diabetes mellitus (GDM) is an increasing problem in the U.S. Many comorbidities are associated with GDM: increased risk for type 2 diabetes, neonatal hypoglycemia and fetal malformation. Healthcare organizations develop GDM educational programs to provide women with knowledge and skills to manage GDM and reduce health risks. While there are significant benefits to attending GDM educational programs, attendance rates are low. Little research has been conducted to determine reasons for low attendance in GDM educational programs. Vital statistics data were statistically analyzed to determine who is breastfeeding in central Kentucky and if factors differ between those in a WIC and non-WIC population. The sample consisted of 479 postpartum women aged 18-44 in central Kentucky. Participants in the study were analyzed as a whole, then divided by their participation in the WIC program (n=304) or non-participation in the WIC program (n=175). Variables recorded in the vital statistics form were analyzed. The purpose of this study is to gain a deeper understanding of the dynamic of perceived benefits and barriers associated with cooking meals at home. The findings will be used to design a social marketing campaign promoting healthy home-cooked meals. Participants included 64 SNAP-eligible mothers throughout the state of Kentucky aged 21-49 years with young children. Eight focus groups were conducted in four metro and four non-metro counties. Nutrition knowledge, dietary intake, body composition and perceived sport performance were measured before and after an eight week nutrition intervention. The sample consisted of eleven male high school football athletes aged 14-18 years old. Baseline nutrition knowledge was higher than anticipated but fruit and vegetable intake was low. As a result of the nutrition intervention, vegetable intake improved from 0.94 servings per day to 2.02 servings per day (p=0.02). Of the eleven subjects in the study, eleven conceded to both performing and feeling better as a result of the nutrition intervention. The present study suggests that a nutrition intervention can …

Obesity is a complex issue; stress, depression, and food addiction, are several psychological conditions that can accompany an obesity diagnosis. Emotional Brain Training (EBT) was evaluated as a new approach to these conditions. Stress, depression, food addiction, and weight were assessed at baseline, after a seven week active intervention, and after a seven week no contact period. The final sample consisted of 26 obese adults. At seven week assessments, EBT participants experienced significant weight loss (p-value = 0.05) and decreased perceived stress (p-value = 0.035). Food addiction also decreased from 50% to 8.3% (or one participant). At fourteen week assessments, …

While traditional behavioral weight loss programs have been successful at producing weight loss, weight maintenance is difficult to achieve. Addressing stress, one major contributing factor in weight gain, may be a potential solution. The purpose of this study was to test a stress-management program (EBT) for changes in weight and blood pressure, and maintenance of those changes. Obese adults (N=33) were randomized to two weight loss groups for a seven-week intervention and a seven-week follow-up period: an intuitive eating group or a stress management group. This paper is an examination of the determinants of Family Support employee allocation. Specifically, related to Kentucky, the goal of this research is to determine if public assistance caseloads are equitable across nine geographic regions. A review of relevant literature provides a larger context to this topic. Literature primarily suggests three mechanisms by which Family Support resources might be allocated: legal regulations, postcode lottery, and Tiebout migration.

Cancer patient mortality is caused by the ability of tumor cells to invade the extracellular matrix and metastasize. Our lab was the first to identify the role of Abl family of non-receptor tyrosine kinases (c-Abl and Arg) in the progression of solid tumor cancers. In our previous studies, we showed that high c-Abl/Arg activity promotes proliferation, invasion, and metastasis in melanoma and breast cancer cells lines. Here, we demonstrate that our previous findings are clinically relevant by showing increased c-Abl/Arg kinase activity in primary melanoma tumor tissue in comparison to low activity as compared to benign nevi. The purpose of this study was to examine the effect of cooperative learning on preservice elementary teachers’ interest in, and the application of music into, core academic subject lesson plans. Participants (N = 59) were preservice elementary teachers enrolled in four class sections of a music method course designed for elementary education majors at a large southern university. All members participating in the study were placed by section for eight weeks in one of two groups-an individualistic learning group or cooperative learning group.

The Ryan White CARE Act of 1991 implemented a federal grant program to help individuals in the US with HIV/AIDS get access to healthcare and diminish barriers to care. INTRODUCTION: Vascular dementia is the second most common cause of dementia after Alzheimer's disease (AD). In addition, it is estimated that almost half of all AD patients have significant cerebrovascular disease comorbid with their AD pathology. We hypothesized that cerebrovascular disease significantly impacts AD pathological progression.
METHODS: We used a dietary model of cerebrovascular disease that relies on the induction of hyperhomocysteinemia (HHcy). HHcy is a significant clinical risk factor for stroke, cardiovascular disease and type 2 diabetes. In the present study, we induced HHcy in APP/PS1 transgenic mice.

Recent reports detail a rise in the practice of home food preservation in the United States due to economic woes, nutritional concerns, and increasing devotion to local food production.Home food preservation is the processing of foods in order to extend its shelf-life. Current common approaches to preserving foods at home include pressure canning, freezing, drying, water bath canning, and cellaring/storing. Local food production in four Kentucky counties were examined through in-depth qualitative interviews with home food preservation practitioners to yield a rural/urban comparison. Innovative plant breeding and technology transfer fostered the Green Revolution (GR), which transformed agriculture worldwide by increasing grain yields in developing countries. The GR temporarily alleviated world hunger, but also reduced biodiversity, nutrient cycling, and carbon (C) sequestration that agricultural lands can provide. Meanwhile, economic disparity and food insecurity within and among countries continues. Disease progression begins with insulin resistance in peripheral tissues followed by pancreatic beta-cell failure. The mechanisms behind disease progression are not completely understood. Childhood overweight/obesity is a critical public health problem with devastating consequences. Many factors have combined to create the epidemic, and primary care providers can play an important role in addressing the problem by identifying overweight/obese children and encouraging them to engage in healthy behaviors. Purpose: The purpose of this evidence-based education project was (i) to evaluate baseline nursing knowledge of anemia, blood conservation and transfusion medicine, (ii) to identify current blood conservation practice and the nurse’s prior exposure to blood conservation education (iii) to determine the potential impact of evidence-based education on nursing knowledge, and (iv) to determine the impact of education on the nurse’s attitudes towards future implementation of blood conservation practices based on the nurse’s self report.

Ergovaline is produced by the endophyte Neotyphodium coenophialum (Morgan-Jones and Gams) in tall fescue (Schedonorus arundinacea (Schreb.) Dumort. = Festuca arundinacea Schreb.) and is blamed for a multitude of costly livestock disorders. Testing of pastures is common in both research and on farm situations. Since ergovaline is known to be unstable and affected by many variables, the objective of this study was to determine the effect of sample handling and storage on the stability of this compound. Switchgrass (Panicum virgatum L.) is an important native warm-season grass for biomass and forage production in the U.S. This research determined the effect of fertilizer type (conventional, manure, and biosolids) and rate on switchgrass biomass yield and forage quality. Fertilizers were added at 0, 33, 67, 100, and 134 kg N ha-1 on established stands of ‘Kanlow’ switchgrass in three northeastern Kentucky counties. Soils across sites ranged from recently cleared forestland (low pH, P, and K) to productive cropland (high pH, P and K).
