Despite advances in healthcare, cardiovascular disease (CVD) remains the leading cause of death in the United States. Elevated levels of plasma cholesterol are highly predictive of CVD and stroke and are the principal driver of atherosclerosis. Unfortunately, current cholesterol lowering agents, such as statins, are not known to reverse atherosclerotic disease once it has been established. In preclinical models, agonists of nuclear receptor, LXR, have been shown to reduce and reverse atherosclerosis. Phytosterols are bioactive non-cholesterol sterols that act as LXR agonists and regulate cholesterol metabolism and transport. We hypothesize that stigmasterol would act as an LXR agonist and alter …

Utilizing data collected by the University of Kentucky Lung Cancer Research Initiative (LCRI), this study investigated potential mining-related sources for the elevated levels of cadmium in Harlan and Letcher counties. Statistical analyses for this study were conducted utilizing SAS. A number of linear regression models and logarithmic models were used to evaluate the significance of the data. The linear regression models consisted of both simple and multivariate types, with the simple models seeking to establish significance between the potential sources and urine cadmium levels and the multivariate models seeking both to identify any statistically significant linear relationships between source types …

The purpose of this pilot investigation was to assess the inter-rater reliability of an expert-validated pediatric feeding assessment, designed for clinician use with children who have severe disabilities. The tool would ideally allow for standardization of the clinical mealtime assessment process.
A comprehensive review of available feeding instruments revealed that there is a need for an assessment tool for children with motor and developmentally based feeding problems.In response to this need, a new clinician administered instrument called the Mealtime Assessment Tool for Children (MATCH) was developed in order to guide the assessment of a child during a clinical meal.

Vitamin D Works Through The Lipid Droplet Protein Plin2 To Augment Mitochondrial Function In Skeletal Muscle, David M. Schnell

Vitamin D has been connected with increased intramyocellular lipid (IMCL) mitochondrial function in skeletal muscle. It is also shown to prevent lipotoxicity in several tissues, but this has not yet been examined in skeletal muscle. Perilipin 2 (PLIN2), a lipid droplet protein upregulated with vitamin D treatment, is integral to managing IMCL capacity and lipid oxidation in skeletal muscle. Increased lipid storage and oxidation is associated with increased tolerance to a hyperlipidic environment and resistance to lipotoxicity. Therefore, I hypothesized that vitamin D increases β-oxidation and lipid turnover though a PLIN2 mediated mechanism, thereby preventing lipotoxicity.

Exposure to environmental pollutants poses numerous risk factors for human health, including increasing incidence of cardiovascular disease and diabetes. Persistent organic pollutants, such as polychlorinated biphenyls (PCBs) have been strongly linked to the development of these chronic inflammatory diseases and the primary route of exposure is through consumption of contaminated food products. Thus, the gastrointestinal tract is susceptible to the greatest levels of these pollutants and is an important facet to study.

Alzheimer’s disease (AD) is the most common form of dementia and a growing disease burden that has seen pharmacological interventions primarily fail. Instead, it has been suggested that preventative measures such as a healthy diet may be the best way in preventing AD. Prebiotics are one such potential measure and are fermented into metabolites by the gut microbiota and acting as gut-brain axis components, beneficially impact the brain. However, the impact of prebiotics in AD prevention is unknown. Abdominal aortic aneurysms (AAAs), a permanent dilation in the abdominal region of the aorta, is a highly sexually dimorphic disease. AAAs prevalence is ranging from 4-10 fold higher in males than females. Defining the mechanistic basis for reduced (in females) or increased (in males) AAA formation and progression may uncover potential therapeutic targets. The majority of studies examining sexual dimorphism focus on the role of sex hormones. However, genes residing on sex chromosomes, in addition to sex hormones, may contribute to sexual dimorphism of AAAs. Consumption of endophyte-infected (E+) grasses has long been associated with health problems in animals. In cattle E+ tall fescue consumption leads to fescue toxicosis, and in horses it leads reproductive problems. The health-related issues associated with endophyte consumption have been attributed to the effects caused by the ergot alkaloids produced by the fungus. These ergot alkaloids are considered D2-like receptor agonists, and 5-HT2 serotonin and α-adrenergic receptor partial agonists. Many studies in humans, swine, cattle, and horses have identified that ergopeptines cause a decrease in prolactin production due to their dopaminergic activities. Conventional Appalachian surface-mine reclamation techniques repress natural forest regeneration, and tree plantings are often necessary for reforestation. Reclaimed Appalachian surface mines harbor a suite of mammal herbivores that forage on recently planted seedlings. Anecdotal reports across Appalachia have implicated herbivory in the hindrance and failure of reforestation efforts, yet empirical evaluation of herbivory impacts on planted seedling vitality in this region remains relatively uninitiated. Many insects form close relationships with microbial symbionts. Insect symbionts can provide novel phenotypes to their hosts, including influencing dietary breadth. In the polyphagous cowpea aphid, Aphis craccivora, the facultative symbiont Arsenophonus improves aphid performance on one host plant (locust), but decreases performance on other plants. The goal of my thesis was to investigate the mechanism by which Arsenophonus facilitates use of locust. First, I assembled an Aphis craccivora-Arsenophonus-Buchnera reference transcriptome to conduct RNAseq analysis, comparing gene expression in aphids feeding on locust and fava, with and without Arsenophonus infection. Cucurbits (i.e. squash, melons, pumpkins, gourds) are high value crops of global importance. Insect pests in these systems are often controlled by chemical insecticides, which are not always effective and can be damaging to the environment. Many integrated pest management (IPM) techniques have been developed for the control of pests in these systems, with a goal of improving system stability and reducing chemical inputs. The overarching goal of my research was to investigate the impact of select IPM techniques on arthropod populations and yield in organic and conventional cucurbit systems.

Public awareness of declining pollinator populations has increased interest in creating “bee–friendly” urban landscapes. I quantified bee visitation and assemblages of 72 species of flowering woody plants common in urban landscapes. I found strong plant species effects and variation in seasonal activity of particular bee taxa but no overall differences in bee visitation or genus diversity between native versus nonnative species or trees versus shrubs. Analysis of pollen from a subset of these plants revealed small but statistically significant differences in total and essential amino acids between native and nonnative species and trees and shrubs, although each group had species …

The goal of this study is to determine issues rural Appalachian residents consider most important, their perceived environmental health risk, and how community engagement can potentially improve those issues. The University of Kentucky Superfund Research Center held the Appalachian Community Health and Well-being Forum at the Letcher County Cooperative Extension Office in Eastern Kentucky. A four-member panel consisted of two local health officials, a nutrition expert, and a federal scientist; answered questions from community members. The expert panel and audience members shared concerns, success stories, and highlighted efforts to promote health in the region. Community members completed a questionnaire collecting …

Due to socioeconomic disparities and geographic isolation, rural Kentucky residents bear a greater burden of poor health compared to national averages. Specifically, rural Kentucky residents are at greater risk of becoming obese and suffering from comorbidities of obesity such as type 2 diabetes, hypertension and cardiovascular disease. The purpose of the present study was two-fold. First, we the barriers to nutrition and physical activity unique to rural Kentucky counties which high proportions of obese adults (< 40%) were examined and extrapolated. Second, this information was used to develop a health and wellness app tailored to rural Kentucky counties. The objective was met via a formative assessment regarding causes for obesity in three rural Kentucky counties using focus groups. From these discussions, two major themes arose: barriers to good nutrition and physical activity, and desired web‐app features. From this assessment, FitFaceoff was developed and released into the same counties. Usage and user interaction were assessed using GoogleAnalytics™ software. Analysis revealed poor user liking and unsuccessful implementation of FitFaceoff, however further qualitative research is needed to evaluate poor user reception of FitFaceoff and possible future directions.

Patients with liver disease have an increased risk for malnutrition because of side effects of the disease. The Nutrition Focused Physical Exam (NFPE) was developed for nutrition professionals to aid physicians in a nutrition-based diagnosis of malnutrition. The purpose of this study was to examine the NFPE for its validity in liver disease patients being evaluated for transplant. The purpose of this mixed methods study was to learn how cultural sensitivity was integrated and perceived in community nutrition education of an existing program offered by the University of Kentucky Cooperative Extension Service Nutrition Education Program. Data collection consisted of a quantitative cultural competence assessment survey tool used with nutrition education paraprofessionals (n=74), qualitative in-depth interviews of nutrition education paraprofessionals (n=8) and focus groups of Hispanic/Latino nutrition education program participants (n=39). Consumers have a variety of ice cream available for purchase through retail stores, including: standard commercial, specialty store, organic, and premium commercial options. This study used a sensory analysis to solicit responses from a group of adults to determine consumer preference and acceptance of four vanilla ice creams based on taste. Consumers preferred (i) organic ice cream to standard, commercial ice cream and (ii) specialty store ice cream to standard, commercial ice cream. Hypertension disproportionately affects Africans Americans more than their White counterparts. Counseling African Americans to Control Hypertension (CAATCH) is an intervention program that is targeted at African American adults to decrease uncontrolled hypertension. The CAATCH intervention has three components, which includes 1) computerized hypertension education, 2) at home blood pressure monitoring, and 3) behavioral counseling. BACKGROUND: Shoulder to Shoulder Global (STSG) recognizes anemia as a cause of morbidity among patients in Santo Domingo, Ecuador. Little research has been done to assess targetable anemia risk factors to serve as a foundation for future pharmacotherapeutic interventions. This study sought to identify risk factors for anemia in this population.
METHODS: Data obtained from existing patient records from January 1, 2010 to August 31, 2016 included hemoglobin/hematocrit, age, sex, pregnancy status, and presenting community. This review focuses on the role of Aβ in AD pathogenesis in Down syndrome and current approaches for imaging Aβ in vivo. We will describe how Aβ deposits with age, the posttranslational modifications that can occur, and detection in biofluids. Three unique case studies describing partial trisomy 21 cases without APP triplication, and the occurrences of low level mosaic trisomy 21 in an early onset AD patient are presented. Brain imaging for Aβ includes those by positron emission tomography and ligands (Pittsburgh Compound B, Florbetapir, and FDDNP) that bind Aβ have been published and are summarized here. Health inequalities take many forms related to race, gender, socioeconomic status, ethnic, language and many other axes throughout communities around the world. Type two diabetes, high blood pressure, and high cholesterol are examples of conditions (among many others) that disproportionately affect Latino@s in the U.S.. The research of this dissertation is based on fieldwork conducted throughout several predominantly Latin@ neighborhoods in Chicago, IL. This dissertation examines how Latin@s in Chicago navigate health and well-being, and how they engage in agentive strategies in the face of chronic disease. Arterial blood pressure (BP) undergoes a 24-hour oscillation that peaks in the active day and reaches a nadir at night during sleep in humans. Reduced nocturnal BP fall (also known as non-dipper) is the most common disruption of BP circadian rhythm and is associated with increased risk of untoward cardiovascular events and target organ injury. Modern maize hybrids exhibit higher yields, increased biomass production, stress tolerance and greater nitrogen (N) use efficiency. Increased biomass accumulation can influence nutrient uptake and lead to increased nutrient removal. Hybrids were tested at seeding rates (SR) of 74000 (low) and 148000 (supraoptimal) plants ha-1 and at N rates of 0 (deficient) and 390 (non-limiting) kg N ha-1. Plants were sampled at V7, V14, R3, R5 and R6 and separated into vegetative and reproductive fractions for determination of dry matter and N accumulation. Grain yield was harvested at R6.

This experiment examined the forage potential of hemp (Cannabis sativa) and kenaf (Hibiscus cannabinus). The objectives were to evaluate yield and forage nutritive value (i.e. NDF, ADF, ADL, IVTD, and CP) fluctuations over the course of a growing season based on planting date, morphological composition, and management. Three types of hemp (grain, fiber, and a dual- purpose type) and kenaf were planted on two dates and were sampled approximately every two weeks throughout the growing season at the University of Kentucky (UK) Research Farm in Lexington, KY. A multidimensional poverty index for the United States is designed, evaluated, and defended as a useful measurement tool for policymakers to evaluate poverty. Chapter 1 presents a normative case for the index. Chapter 2 reviews the literature on poverty measures. Chapter 3 constructs the proposed index. Chapter 4 is a statistical examination of the internal structure of the index. Chapter 5 explores the index across states, over time, and among population subgroups, as well as presenting policy applications.

The primary objective of this dissertation is to study the management of public and nonprofit resources and financial risk. Governments will be able to use its findings to continue to provide public services in collaboration with other sectors, including the nonprofit sector. Nonprofit financial self-sufficiency and sub-award grant mechanisms between the government and nonprofit sectors are two primary areas to be examined. This dissertation consists of three essays. As urban areas have come to increasing dominate the social landscape, rurality is often defined in negative ways such as being backwards, simple, or even deviant. Urbanormativity is a theoretical approach developed to capture the normative and structural impacts and implications of privileging the urban. The result is not only the construction of urban as correct and positive and rurality as abnormal and backwards, the cultural ideology impacts the structural flow of resources which negatively impacts and results in a marginalization of rural areas.
