Medicare Part D provides prescription drug coverage for beneficiaries to support their pharmacological treatment; however, the complex deductible structure within benefit plans creates a major coverage gap and unexpected consequences. Some evidence has demonstrated reduced adherence resulting from the coverage gap; however, little research has evaluated the effect on healthcare resource use (HRU) and cost, and no studies have been conducted for beneficiaries with chronic obstructive pulmonary disease (COPD). The three discrete research projects which make up this dissertation provide empirical evidence on the impact of, and adoption trends of, health information technology (HIT) as it pertains to cancer care. The findings of these studies are important as they provide insight into the potential of HIT to improve the quality of cancer care in the US. The results of this dissertation suggest that based on the current literature, HIT interventions seem to be more successful when targeting physicians, care in the prevention phase of the cancer continuum, and/or decision making. Physician-Parent Interactions In Pediatric End-Of-Life Care: Implications For Integrative Communication, Decision-Making And Ethics Training*, Lori Brand Bateman

In this dissertation, I utilized a primarily qualitative mixed methods longitudinal process to explore physician-parent pediatric end-of-life communication. Between August 2011 and March 2012, participant pediatric physicians (N=21) from emergency medicine (PEM) and critical care (PCC) were presented with a standardized case of an 8 year-old child in respiratory distress who had a degenerative neurological disorder and was nearing the end-of-life. Participants were observed in a timed simulation experience and given a physician-parent participatory communication score. All simulations were followed by a post-simulation debriefing interview. Introduction: Patients commonly complain of subjective side effects of opioid use, including forgetfulness, slowed processing speed, confusion, and sedation - which have raised concerns about potential cognitive impairment. However, research on the effects of opioids on objective neuropsychological performance is thus far equivocal and, unfortunately, many of the studies cited as support for opioid-related impairment do not fully account for the neuropsychological effects of chronic pain, which has been shown to affect many of the same neuropsychological domains purported to be affected by opioids. Introduction The purpose of this study was to examine the association of social determinants of health with 30-day rehospitalization among elderly patients who were discharged from an acute care hospital with an initial discharge destination of home with home health care support and to explore the social determinants' ability to accurately predict the odds of rehospitalization. Methods A secondary data analysis was performed on data obtained from the home healthcare agency. A total of 4,717 unique patients found in the data set. Utilization And Effectiveness Of Osteoporosis Medications Among Medicare Beneficiaries, Huifeng Yun

This dissertation contains three papers evaluating the utilization and effectiveness of osteoporosis medications using data on a 5% sample of Medicare beneficiaries in 2006-2009. The first paper assessed the completeness of Medicare Part D data on generic alendronate. The second paper sought to identify anti-osteoporosis medication utilization patterns and factors associated with medication discontinuation. The third paper examined the comparative effectiveness of osteoporosis medications for risk of fractures and mortality, with a focus on the recently introduced infusion and injection medications. In the first study, we identified beneficiaries highly adherent with branded alendronate during 1/1/06-2/6/07 ("2007 cohort") and highly adherent …

The Effect Of Optimism On The Relationship Between Religiosity And Spirituality Among Elderly Christians, Matthew West

Older Americans represent one of the most devout groups of religious people in America, yet research is consistently scarce on how to use this characteristic in mental health interventions. This study uses Ordinary Least Squares regression of a 2004, national sample of older Christian Americans to assess the relationship between private prayer, church attendance, prayer group participation, religious meaning, optimism, and depressive symptomology. Previous work has found a strong positive relationship between religion and health outcomes. The present endeavor goes beyond examining the main effects of such relationships to look at a possible moderating mechanism--optimism. Controlling for demographic variables and …
