Department of Urology Faculty Papers
We report our experience performing a robot-assisted dismembered pyeloplasty on a patient with a ureteropelvic junction obstruction in a horseshoe kidney and a prior history of endopyelotomy. We provide 18-month follow-up demonstrating that robotic pyeloplasty is a reasonable second treatment option for patients with horseshoe kidneys with failed prior endourological management.

Implementing A Practice Doctorate Program At A Distance Through An Urban-Rural Partnership, Beth Ann Swan, Anthony J. Frisby, Lori S. Lauver, Margaret M. West, Mary Powell, Ksenia Zukowsky, Alexis Marsella
The Selected Works of Anthony J. Frisby, PhD
The purposes of this poster presentation are to 1) describe the implementation of a doctor of nursing practice (DNP) program by providing access to rigorous distance education to students living in rural Pennsylvania; 2) discuss building a critical mass of doctorally prepared advanced practice nurse experts in both urban and rural communities; and 3) share formative and summative evaluation information.
Through funding from the U.S. Department of Health and Human Services, Health Resources and Services Administration, Bureau of Health Professions, Division of Nursing, the Jefferson School of Nursing (JSN) expanded its DNP program currently offered at the urban Philadelphia campus …

Children's participation in everyday activities and routines in home and community settings is an important focus of services for infants and young children with disabilities. Data indicate that assistive technology (AT) is not widely used nor do early intervention service providers report frequent use of AT devices with infant-toddlers. Adaptation interventions combine environmental accommodations and AT in ways that promote children's participation in activities and routines and provide functional skill-building learning opportunities. A decision-making process for planning and implementing adaptation interventions is outlined with examples of strategies and formats that service providers can use to create successful interventions for infants …

The spread of metastatic tumors to different organs is associated with poor prognosis. The metastatic process requires migration and cellular invasion. The protooncogene c-jun encodes the founding member of the activator protein-1 family and is required for cellular proliferation and DNA synthesis in response to oncogenic signals and plays an essential role in chemical carcinogenesis. The role of c-Jun in cellular invasion remains to be defined. Genetic deletion of c-Jun in transgenic mice is embryonic lethal; therefore, transgenic mice encoding a c-Jun gene flanked by LoxP sites (c-jun(f/f)) were used. c-jun gene deletion reduced c-Src expression, hyperactivated ROCK II signaling, …

Retinal Pigment Epithelial Tears After Intravitreal Bevacizumab Injection For Exudative Age-Related Macular Degeneration., Sunir Garg, Roy Brod, David Kim, R Gary Lane, Joseph Maguire, David Fischer
Wills Eye Hospital Papers
PURPOSE: To determine the incidence of and the risk factors for the development of retinal pigment epithelial (RPE) tears after intravitreal bevacizumab (Avastin) injection for the treatment of exudative age-related macular degeneration (AMD).
METHODS: A retrospective, multicentre, consecutive interventional case series of all patients with subfoveal exudative AMD treated with intravitreal bevacizumab between August 2005 and April 2007. The main outcome measures were pre- and post-RPE tear visual acuity and choroidal neovascular membrane lesion types, incidence of tears and time from first injection until development of the tear.
RESULTS: A total of 920 eyes with exudative AMD were treated with …

Metastatic Disease To The Pancreas And Spleen, Shayna L. Showalter, Eric Hager, Charles J. Yeo
Department of Surgery Faculty Papers
Isolated metastases to the pancreas and spleen are a rare occurrence. When they are diagnosed, pancreatic metastases are most often from renal cell carcinoma, lung cancer, and breast cancer. The most common source of splenic metastases is gynecological in origin; the overwhelming majority is ovarian. If extensive staging studies reveal these metastases to be isolated, then curative resection may be warranted. This review will demonstrate that long-term survival may be achieved in patients with isolated metastases and a prolonged disease-free interval.

Department of Medicine Faculty Papers
Serous cystadenomas (SC) of the pancreas are uncommon benign cystic neoplasms that were first differentiated from mucinous cystadenomas in 1978 by Compagno and Oertel (1-3). Serous cystadenomas characteristically have a spongy gross appearance and are composed of innumerable tiny cysts lined by flattened cuboidal or polygonal cells with clear to pale eosinophilic cytoplasm atop of thin fibrous septae. Since 1978, five unique SC subtypes have been identified (4-5). Here we report a case of a solid serous adenoma, a rare serous cystadenoma subtype.

Challenges And Benefits In Using Productivity Data From Clinical Trials For Vbp Decisions, Thomas J. Bunz, Pharm.D., Laura T. Pizzi
Value-Based Purchasing Newsletter
Making value based purchasing decisions in healthcare requires balancing data from several sources. Information on effectiveness of interventions often comes from clinical trials, with less reliance on observational studies. Historically, the opposite has been true of information on the cost of interventions. More recently, providers of healthcare interventions such as drugs, devices, and disease management programs have begun to understand and appreciate the importance of costs in healthcare decision making. Because of this, the clinical trials used to demonstrate effectiveness have begun to gather data on costs. This article describes some benefits and detriments of collecting cost data as a …

The Emerging Role Of Electronic Medical Records - What Do They Mean To Quality And Value?, Richard Jacoby
Value-Based Purchasing Newsletter
President Bush identified electronic medical records (EMRs), also known as electronic health records (EHRs), as one of the 4 cornerstones in his value driven health care initiative. EMRs represent a “disruptive technology” insofar as they change the way we capture, store, retrieve, share, and use health information. Because they have the ability to transform and enhance virtually all communications, transactions, and analyses involving healthcare information, implementation of EMRs will have a profound effect on patients, providers, and payers. The transformation likely will parallel the one which occurred as information technology enhanced knowledge and productivity in the non-healthcare segments of the …
