Purpose: This presentation will discuss the necessary transitions in the role of the Hospital-Based Clinical Nurse Scientist to work successfully in a fully enculturated evidence-based practice environment. The role of the Hospital-Based Nurse Scientist is relatively new. Nurse Scientists who are employed in hospital- based settings are educated in the traditional research paradigm and many have very limited exposure to evidence-based practice.

Purpose: To evaluate if the use of transitional care services through home care services reduced hospital readmissions as compared to self-care at home for heart failure patients. In addition, the four key processes of the Institute of Healthcare Improvement (IHI) Transforming Care at the Bedside How to Guide for Patients with Heart Failure were evaluated.

Purpose: Changing an organization's culture is challenging and requires comprehensive assessment and purposefully deploying organizational tools. This paper will present a model of engaging a process to ramp up a research culture in a College of Nursing where teaching was heretofore the primary focus.
Methods: To purposefully engage in transformational change, an organizational consultant was utilized to conduct a systematic assessment of the college's work culture and climate. Purpose: Anticipating barriers when introducing a new form to nurses for incorporation into their daily routine, Kurt Lewin's action research model for change was used as a framework for this study. The purpose was to determine usability and acceptability of the Modified Early Warning Scoring (MEWS) tool by healthcare providers from the hospital units where the tool will be implemented.
