Purpose: As the second of two papers addressing challenges in applying best evidence to physiotherapy practice (see volume 4 number 3), the purpose of this paper is to explore health and clinical reasoning models that can facilitate evidence-based practice. Summary of Key Points: The challenge and importance of using skilled clinical reasoning in applying research evidence and managing patients that fall outside the available evidence are discussed. Aerial photographs from 1937–2000 of Bahía Salina del Sur on vieques, Puerto Rico were analyzed to detect and describe spatial changes in the areal cover of sea-grass beds in Bahía Salina del Sur. The images were pre-processed to minimize noise and unsupervised classification was used to detect areas colonized by seagrass. The number of individual seagrass patches, direction, and characteristic of growth were quantified and described. Seagrass coverage increased by 85.8% over the 64-yr period and this increase was best described by a 2nd order polynomial function (r2 = 0.91). Between 1937 and 2000, the spatial expression of the …

The leading genetic cause of infant mortality is spinal muscular atrophy (SMA), a clinically and genetically heterogeneous group of disorders. Previously we described a domestic cat model of autosomal recessive, juvenile-onset SMA similar to human SMA type III. Here we report results of a whole-genome scan for linkage in the feline SMA pedigree using recently developed species-specific and comparative mapping resources. We identified a novel SMA gene candidate, LIX1, in an ~140-kb deletion on feline chromosome A1q in a region of conserved synteny to human chromosome 5q15. The United States Military Academy has offered gymnastics instruction since 1838. Gymnastics continues to be an integral component of the physical education curriculum. The purpose of this study was to investigate cadets' perceptions of their experiences in a required gymnastics course using the critical incident technique. Students described experiences in the gymnastics class that they believed had a positive or negative influence on their development as cadets and future Army officers. Key elements of their responses were classified into 16 positively perceived and 11 negatively perceived categories. The purpose of this paper is to discuss the development of research questions in mixed methods studies. First, we discuss the ways that the goal of the study, the research objective(s), and the research purpose shape the formation of research questions. Second, we compare and contrast quantitative research questions and qualitative research questions. Third, we describe how to write mixed methods research questions, which we define as questions that embed quantitative and qualitative research questions. Finally, we provide a framework for linking research questions to mixed methods data analysis techniques. A major goal of our framework is to illustrate that …

Insuring The Uninsured: Reducing The Barriers To Public Insurance, Cynthia M. Saunders

Health insurance is one of the essential enabling resources to gain access to medical care and ultimately increase health status. Over 11 million or one quarter of the nation’s uninsured individuals are eligible for Medicaid or the State Children’s Health Insurance Program (SCHIP), but are not enrolled. Interviews with 368 individuals from 1999 through 2003 identify eight primary barriers to enrollment in public insurance. These include: economic aspects of qualifying, lack of knowledge, benefit design of public programs, poor experiences and stigma, complexity and literacy, immigration status, poor customer service, and fear of fraud. There is a sharp divide between quantitative and qualitative methodologies in the social sciences. We investigate an innovative way to bridge this gap that incorporates quantitative techniques into a qualitative method, the “quanti-qualitative method” (QQM). Specifically, our research utilized small survey questionnaires and experiment-like activities as part of the question rout e in a series of five focused group interviews on nutrition education. We show how these quantitative-type activities fit naturally with our question route and contributed to testing the hypotheses within the context of the five important characteristics of focused group interviews. The compound genotype KIR3DS1/HLA-B Bw4-80I, which presumably favors natural killer cell activation, has been implicated in protection against HIV disease. We show that this genotype confers dual protection over the course of HIV disease; early direct containment of HIV viral load, and late specific defense against opportunistic infections, but not AIDS-related malignancies. The double protection of KIR3DS1/Bw4-80I in an etiologically complex disease such as AIDS, along with the disease specificity of its effects is conceptually novel and underscores the intricacy of host immunogenetics against HIV/AIDS.

Filter samples of fine aerosols collected in the Southeastern United States in June 2004 were analyzed for the characterization of polar organic components. Four analytical techniques, liquid chromatography –mass spectrometry, ion trap mass spectrometry, laser desorption ionization mass spectrometry, and high-resolution mass spectrometry, were used for identification and quantification. Forty distinct species were detected, comprising on average 7.2% and 1.1% of the total particulate organic mass at three inland sites and a coastal site, respectively. The relative abundance of these species displays a rather consistent distribution pattern in the inland region, whereas a different pattern is found at the coastal …

Tumor susceptibility gene 101 (TSG101) encodes a host cellular protein that is appropriated by human immunodeficiency virus type 1 (HIV-1) in the budding process of viral particles from infected cells. Variation in the coding or noncoding regions of the gene could potentially affect the degree of TSG101-mediated release of viral particles. While the coding regions of the gene were found to lack nonsynonymous variants, two polymorphic sites in the TSG101 5' area were identified that were associated with the rate of AIDS progression among Caucasians. This study was survey of disease distribution and abundance relating to coral diseases present in Broward County. Data of the 1330 scleractinian coral individuals found, 88 showed signs of disease. 19 coral species, 3 diseases and bleaching were represented. Diseases affected 9 of the species. Bleaching was the most common disease noted in this survey, followed by dark spot, red band, and yellow band.
Disease distribution appears to be scattered on the reef system. No apparent patterns were found when grouped by reef or corridor locations. MDS cluster analysis revealed a clumping of disease, but this was not correlated with …

Purpose: The purpose of this study was to examine what processes facilitate, temper, or impede occupational therapy identity development in a community of practice. Methods: A multiple case design organized data collected from five in-depth interviews with occupational therapy students on level II fieldwork. A cross-case analysis was used to arrive at multiple case themes. Results: Themes emerged as responses to participation in a community of practice: a) professional relationships; b) supervision types; and c) responsibility for professional identity development. Results suggest that communities of practice have unique characteristics that either inhibit students from adopting professional identity or draw them …

Purpose: As the first of two papers addressing challenges in applying best evidence to physiotherapy practice, this paper highlights the role of qualitative research and challenges researchers face in producing sound evidence for practice.Summary of Key Points: The challenge of what constitutes acceptable evidence to inform evidence-based practice is addressed in this paper with critique of the current over-reliance on quantitative methodologies that excludes a substantial body of valuable qualitative evidence to support sound practice.
