Costs, Treatment Approaches, And Access To Care For Stroke Survivors, Corey Morrow
Costs, Treatment Approaches, And Access To Care For Stroke Survivors, Corey Morrow
MUSC Theses and Dissertations
Barriers to rehabilitation for stroke survivors exist despite evidence supporting improvements in independence and quality of life. Telerehabilitation is emerging as a viable option to improve costs and access. While there is growing evidence for telerehabilitation efficacy, the associated costs are unknown. Additionally, to inspire changes in reimbursement and policy, more evidence of access disparities is needed. The overall objective of this project is to provide depth to the costs, treatment approaches, and access to care for stroke survivors. We completed three separate projects to address our objective. Our time-driven, activity-based costing analysis compared marginal costs in the care delivery …

Comparison Of Accuracy And Efficiency Of Robotic-Assisted Haptic Guidance Versus Freehand Access Preparation In Calcified Teeth, Roshanak Momen
Comparison Of Accuracy And Efficiency Of Robotic-Assisted Haptic Guidance Versus Freehand Access Preparation In Calcified Teeth, Roshanak Momen
MUSC Theses and Dissertations
Introduction: The initial step of root canal therapy consists of access preparation. Access preparation can be difficult, due to pulpal calcifications, misaligned teeth, and full coverage restorations that obscure the canal morphology. Recent trends involve static or dynamic guides to help the operator to locate the canals. Both systems have limitations. A robot with haptic guidance has been developed for implant surgeries, however, no studies have evaluated its possible use in endodontics. The purpose of this study was to determine whether the volume of tooth structure removed, and time required for access preparation in calcified anterior teeth differed between a …

Purpose: Social factors have been implicated in the development of peri-implant pathologies, including implant failure. This study aims to investigate whether alcohol consumption affects late dental implant failures. Methods: A retrospective cohort study evaluated implants placed between 2006 and 2012 at the Philadelphia Veterans Affairs Medical Center. The primary predictor variable was alcohol consumption, measured as nonconsumption and mild, moderate, and heavy consumption. The primary outcome variable was late dental implant failure. Univariate, bivariate, and multivariate statistics were applied, with P <.05 used to define statistical significance. Results: Our cohort consisted of 103 patients and 295 implants with a 5-year minimum follow-up. Most patients were male (93%) with an average age of 60 at the time of implant placement. Late implant failure was associated with 30 implants. Compared to nonconsumption, mild consumption was associated with a 75% decrease in late implant failure (P =.0494), moderate consumption was associated with a 60% decrease (P =.3826), and heavy consumption was associated with a 200% increase (P <.1782). Compared to mild consumption, heavy consumption was associated with an 847% increase in late failure (P =.0135). Conclusions: Results from this retrospective cohort analysis suggest that mild alcohol consumption is associated with a decrease in late dental implant failures.

High fluoride concentrations in drinking water affect millions of people around the world; however, fluoride can come from several sources. The World Health Organization recommends a fluoride concentration in drinking water of no more than 1.5 mg/L; fluoride above this concentration can cause long-term problems known as fluorosis, such as mottled teeth and increased risk of dental caries, or skeletal deformities. Rural communities near Arusha, Tanzania have high fluoride concentrations in their water. Adults and children of the Arusha Region rely heavily on tea for daily water consumption, which has the benefit of disinfection by boiling. Researchers investigated water quality, …

The integrity of the basal stem cell layer is critical for epithelial homoeostasis. In this paper, we review the expression of oral mucosal stem cell markers (OM-SCMs) in oral submucous fibrosis (OSF), oral potentially malignant disorders (OPMDs) and oral squamous cell carcinoma (OSCC) to understand the role of basal cells in potentiating cancer stem cell behaviour in OSF. While the loss of basal cell clonogenicity triggers epithelial atrophy in OSF, the transition of the epithelium from atrophic to hyperplastic and eventually neoplastic involves the reactivation of basal stemness. The vacillating expression patterns of OM-SCMs confirm the role of keratins 5, …

Epithelial-mesenchymal transition (EMT) is a critical process that occurs during the embryonic development, wound healing, organ fibrosis and the onset of malignancy. Emerging evidence suggests that the EMT is involved in the invasion and metastasis of cancers. The inflammatory reaction antecedent to fibrosis in the onset of oral submucous fibrosis (OSF) and the role of EMT in its malignant transformation indicates a hitherto unexplored involvement of EMT. This review focuses on the role of EMT markers which are regulators of the EMT mediated complex network of molecular mechanisms involved in the pathogenesis of OSF and OSCC. Further the gene enrichment …

MicroRNAs (miRNAs) are a family of small non-coding (18–22 nucleotide) RNA molecules. These molecules regulate gene expression by either inhibiting mRNA translation or by degrading mRNA. A single miRNA can control the expression of target genes, and the expression of a target gene can be regulated by multiple miRNAs. They are key regulators of various biological and pathological processes. These include cell proliferation, development and tumorigenesis. Novel studies have discovered definite signature miRNAs in the initiation and progression of cancers. Interestingly, miRNAs have also been found in fragile genomic sites that are associated with increased cancer risk. These micro RNAs …

Amalgam in dentistry has been in use for over 150 years and still been widely used because of its low cost, easy application, durability, bacteriostatic effect, and strength. Whenever aesthetics isn’t the prime concern it is possibly used with no age bar, in areas of stress, as a foundation for cast-metals, ceramic restorations, and below-par oral hygiene conditions. Its advantages such as long-lasting and being less technique sensitive among all restorative materials, there has been growing concern about amalgam that it causes mercury toxicity. Mercury (Hg) has been historically popular as one of the extremely toxic nonradioactive materials. Therefore, the …
