In this review article, we have described the clinical approach in detecting and confirming myofascial trigger points (MTrPs) that elicit clinical complaints. In the last decade, the pathophysiology of MTrPs has been much clarified based on clinical and basic science research work. An MTrP is a hyperirritable spot in a taut band of skeletal muscle fibers with characteristics related to central sensitization in the spinal cord. The pathological diagnosis should include: (1) the electromyographic (EMG) recording and ultrasound imaging of local twitch responses (LTRs), (2) the EMG recording of spontaneous electrical activity (endplate noise) at multiple active loci in the …

To evaluate the immediate effects of acupuncture therapy in myofascial pain syndrome (MPS), 13 subjects (11 females and 2 males), aged from 34 to 59 years old were recruited and all had suffered MPS anywhere from 1 to 180 months. The oblique insertion of an acupuncture needle was performed at an Ah's point (trigger point, TrP) and through the associated taut band without any twirling. The needle was removed anywhere from several seconds to 2 to 3 minutes after insertion whenever the needle sensation disappeared. Despite advances in neuroimaging technology and neurosurgical treatment, spinal epidural abscess remains a challenging problem. Early diagnosis is difficult and thus treatment is often too late to prevent severe neuro- logical sequelae.A 46 years old male was admitted to our emergency department due to fever and general weakness. Under the impression of sepsis due to Staphylococcus aureus, he was admitted for further antibiotic treatment. Fever subsided one day after parenteral antibiotic usage. Unfortunately, fever recurred three weeks later accompaning with neck pain, cough, dypsnea and skin rash. Chest roentgenogram revealed rapid progressive cardiomegaly. Pronator syndrome as a compression of median nerve at proximal forearm level is uncommon in all entrapment neuropathies. Only one case with bilateral pronator syndrome has been reported in the literature. A 51 year-old female suffered from numbness over bilateral hands for years, then she received neurolysis of median nerves at wrist level under the impression of bilateral carpal tunnel syndrome. Four months after operation, she still suffered from grip weakness and numbness over forearms and bilateral radial 3 and half fingers. Preliminary Validation Of A Mobility Obstacle Course For Persons With Mobility Impairment, Stephen F. Figoni, Christian Thompson, A. Katherine Froehlich, Dot E. Nary, Janet Marquis, Glen W. White

Kinesiology (Formerly Exercise and Sport Science)

The purpose of this study was to assess the reliability and validity of a mobility obstacle course for women with mobility impairments. Participants included 72 adult women with permanent physical disabilities including arthritis, orthopedic conditions, paraplegia, and others. The 60-m course consisted of carpeted runways and turns, ramps, a doorway, a transfer, and object manipulation. Participants completed two trials, walking or wheeling through the course as quickly as possible, safely and without running. Total course time and peak heart rate data were correlated with SF-36 health survey subscales. Overall, peak heart rate was significantly (p < .05) correlated with physical functioning (r = -.328), limitations due to physical functioning (r = .261), and pain (r = .296). Total course time was significantly correlated with physical functioning. These findings indicate very high reliability and preliminary evidence of validity.

Knee injuries are among the most common injuries seen in a physical therapy clinic. Nearly all rehabilitation programs following a knee injury involve quadriceps muscle strengthening. One popular and safe method of quadriceps strengthening often used is the wall slide. There is, however, debate and little research indicating whether or not changing the foot position used during wall slides alters the muscle activity during this exercise.

Backward walking is a common intervention in the rehabilitation of lower extremity injuries. Despite its popularity, there is limited research available on the EMG activity during backward walking at an incline when compared to forward walking. In this study, we recorded EMG activity in four muscles of the lower extremity and utilized motion analysis to evaluate the knee range of motion when walking forward and backward on a treadmill at 0 and 15 percent grade inclines.

The purposes of this study are to assess whether University of North Dakota Physical Therapy (UND-PT) graduates desire a transitional Doctorate of Physical Therapy (t-DPT) degree and to determine their preferences regarding this proposed educational program.

The survey was sent to 1136 alumni of the UND-PT program spanning from its first graduating class in 1970 to the class of 2002. It consisted of 8 demographic questions and 12 questions regarding areas such as curriculum content, program implementation, and personal views about the Doctorate of Physical Therapy degree. Numerous studies have been done on the Plyo Press, with focus on the concentric phase of the exercise. However, for athletes eccentric training has always been a topic of interest. Most leg press machines have a concentric and an eccentric phase involved with the exercise. However, during the eccentric phase of the leg press the muscles are underloaded because of the force-velocity relationship of the human skeletal muscle and the fact that eccentric contractions require fewer motor units for the same force and workload. The objective of this study was to compare knee angles and moments of healthy subjects (n/20) and subjects that were anterior cruciate ligament deficient (ACLD) (n/16) during stepping and crossover cutting activities. Subjects that were ACLD were separated into high (n/7) and low (n/9) functioning groups based on knee functioning ratings. Knee angles and moments were estimated using three dimensional motion tracking and force plate data. The results suggest that knee angle and moment data were associated with level of functioning of ACLD subjects. Primarily knee frontal and transverse plane moments distinguished the stepping and crossover cut activities. Only some …

Study Design: Case report.

Objective: To describe the knee kinematics and moments of a giving-way trial of a subject with an anterior-cruciate-ligament– (ACL) deficient knee relative to his non–giving-way trials and to healthy subjects during a step-down task.

Background: Episodes of giving way are believed to damage joint structures, therefore treatments aim to prevent giving-way episodes, yet few studies document giving-way events.

Comparison between successful and failed sit-to-stand trials of a patient after traumatic brain injury.

Objective: To compare the peak whole-body center of mass (COM) velocities and joint angular contributions in successful and unsuccessful sit-to-stand (STS) trials in a subject with traumatic brain injury (TBI).

Design: Single-case study.

Setting: Motion research laboratory.

Participant: A 24-year-old man who was 3.5 years post-TBI.

Interventions: Not applicable.

Main Outcome Measures: Peak horizontal and vertical velocities of the whole-body COM and peak angular velocities of the ankle, knee, hip, and shoulder joints.

The purpose of this research was to explore the nature of equine-human bonding and its therapeutic value in recovery from trauma. The study sample consisted of six participants who identified that their pre-existing relationships with horses were therapeutic in recovering from various types of trauma (i.e. car accidents, horse-related accidents, work accidents and health trauma). Using a phenomenological perspective and an emotionalist lens, though the use of semi-structured interviews and video-tapes of horse-rider interaction, the study sought to describe the nature of the equine human bond and how it is useful in trauma recovery.
