"This chapter is written in the spirit of optimism, suggesting that self-organization is just such a general theory that may assist in providing a deeper, scientifically grounded understanding of the complex biopsychosocial processes involved in psychotherapy. To provide some foundation to this rather grand suggestion, the following review rests on five more specific theoretical propositions stemming from self-organization theory... Background and Purpose: This case report describes the use of two different conservative treatment approaches on a patient suffering from chronic nonspecific low back pain. The case report illustrates the difference between aquatic therapy/core stabilization, which focuses on the biological aspect of the injury, and multi-disciplinary treatment, which treats the injury on a biopsychosocial level.

Objectives: To compare the effectiveness of a digital video disc (DVD) with that of a written workbook delivering falls prevention education to older hospital patients on self-perceived risk of falls, perception of falls epidemiology, knowledge of prevention strategies, and motivation and confidence to engage in self-protective strategies. To compare the effect of receiving either education approach versus no education on patients' perception of falls epidemiology.

Design: Randomized trial (DVD vs workbook) with additional quasi-experimental control group.

Background. With the increasing incidence of Alzheimer disease (AD), determining the validity and reliability Of Outcome measures for people with this disease is necessary.

Objective. The goals of this study were to assess test-retest reliability of data for the Timed "Up & Go" Test (TUG), the Six-Minute Walk Test (6MWT), and gait speed and to calculate minimal detectable change (MDC) scores for each outcome measure. Performance differences between groups With mild to moderate AD and moderately severe to severe AD (as determined by the Functional Assessment Staging [FAST] scale) were Studied.

The purpose of this research was to examine the outcomes prompting hiking along the Appalachian Trail (AT). By using means-end theory, linkages between attributes, consequences, and values of the AT hiking experience were made. The researchers conducted forty-three interviews of AT hikers. Self-fulfillment, self-reliance, fun and enjoyment of life, and warm relationships with others were some of the values that emerged. Specifically, strong links existed between hiking and exercise, exercise and health, health and fun and enjoyment of life. This handbook is the culmination of a multiphase, multidisciplinary research project that used grounded theory and participatory action research to illuminate ways that healthcare providers can work sensitively (in a trauma-informed way) with adult survivors of childhood sexual abuse.

The research identified nine Principles of Sensitive Practice: respect, rapport, taking time, sharing information and control, respecting boundaries, fostering mutual learning, understanding non-linear healing and demonstrating an understanding of trauma to patients. Specific guidelines were developed for a wide variety of issues pertinent to clinical practice such as, removal of clothing, touch, responding to disclosures of abuse, managing triggers among others. …

Among baseball athletes, joint range of motion (ROM) is considered an important physical characteristic with respect to injury prevention and performance. Professional and intercollegiate programs employ various methods of flexibility training; however, to date, no literature exists with regard to the effect of a yoga-based training program. The purpose of this investigation was to determine the effects of a sport-specific yoga program among NCAA Division I intercollegiate baseball players. Subjects (N=30, age 19.42 ±1.37 years) were assessed for shoulder joint (SH), hamstring (HS), and groin (GR) ROM and subsequently followed a 12-week, 2x/week yoga intervention. Background and Purpose: This case report follows a 20 year old male through his five week out patient rehabilitation for an open reduction internal fixation (ORIF) pinning of his right lunate and subsequent hardware removal following a motor vehicle accident. The patient presented with pain, decreased strength and range of motion of his right wrist. A deficit was also identified in his right shoulder stemming from decreased use of his right upper extremity. The symptoms of arthritis can be detrimental to a patient's ability to participate in their daily functional activities. Historically through today, patients report that these symptoms increase with changes in weather conditions.

Carpal tunnel syndrome (CTS) is common among supermarket employees with a high prevalence among supermarket cashiers. In a study by Davis et. aI., (2001) it was found that supermarkets had the highest number of workman's compensation claims regarding CTS in Massachusetts. Of 247 reported cases of CTS in supermarkets 80 involved cashiers. Supermarket cashiers show a 19-60% CTS prevalence rate (Bonfiglioli et aI. , 2007). No prevention guide designed for supermarket employees was found in the literature; this scholarly project addresses the need for prevention of CTS among supermarket employees.

Children in elementary school classrooms are expected to participate in fine motor activities up to 60% of the day. These fine motor activities include writing, cutting, gluing, coloring, and self care tasks such as buttoning and tying shoes (McHale & Cermak, 1992). In a literature review conducted by Feder and Majnemer (2007) it was found that poor handwriting has a negative impact on academic and social performance into adolescence. Consequences of poor handwriting may be avoidance of fine motor activities (Jackman & Stagnitti, 2007) and decreased academic performance which may lead to poor self esteem (McHale & Cermak, 1992).

Background/Purpose: This case study describes a 28-year-old woman’s six physical therapy visits over a period of four weeks for treatment of right pes anserine bursitis. The purpose of this case study is to describe the treatment of an individual with pes anserine bursitis using hamstring stretching, quadriceps strengthening, and iontophoresis and to describe the outcomes.

Case Description: Following a fall down eight steps in her home with her right knee underneath her. The patient presented with increased pain, swelling, loss of range of motion, and tenderness at the pes anserine. Treatment consisted of iontophoresis, hamstring stretches, and quadriceps strengthening.

Contracts with Industry, implemented in the 1990s as the Natural Supports Initiative, is a program option that allows the Oklahoma Department of Human Services (OKDHS) Developmental Disabilities Services Division (DDSD) to contract directly with businesses to provide job coaching supports that become a part of the natural workplace. Individuals participating in this program are employed by a business and are paid minimum wage or better. The name change to Contracts with Industry was an effort to distinguish the program from other DDSD employment options that involve employment service providers and paid job coaches.

King County’s program to employ people with disabilities in county jobs is an example of Washington’s commitment to the use of innovative approaches to increase integrated employment. In 1989, a training resource funded by Washington State and the county Division of Developmental Disabilities, O’Neill and Associates, submitted a grant application to the Rehabilitation Services Administration to develop public sector jobs for people with developmental disabilities within the state. In 2006, a new Maine law mandated the creation of a waiver program that promotes the expansion of supported employment programs for people with intellectual and developmental disabilities (ID/DD). As a result, state funding for sheltered workshops was reduced for seven workshops and approximately 220 individuals throughout Maine. These individuals received funding under the new waiver program for community supports and supported employment services. As a result, Maine’s Department of Health and Human Service’s (DHHS) Office of Adults with Cognitive and Physical Disabilities (OACPD) developed a comprehensive work plan. Tennessee implemented its Employment First initiative in 2002 with a goal of making employment the first day service option for adults receiving supports from the Department of Mental Retardation Services. As part of the Employment First initiative, the state requires a periodic community-based work assessment for all individuals not currently employed in the community. State-contracted and independent individualized support coordinators ensure that individuals who are not in integrated employment participate in a community-based work assessment at least every three years. Since December 2005, employment providers can be trained in conducting situational job assessments and the importance of providing these opportunities.

The state of Florida has implemented a five-year employment initiative for people with ID/DD. The goal is to enable at least 50% of adults (ages 18 to 55) receiving APD-funded day services as of July 1, 2004, to achieve community employment by July 1, 2009. APD-funded services include adult day training, supported employment, and non-residential supports and services. Florida is specifically targeting a total of 25% of individuals who were in Adult Day Training (ADT) on July 1, 2004, to be employed by July 1, 2009. In North Carolina, counties have been consolidated into Local Management Entities (LMEs). These entities contract for services with community providers and provide oversight on access, utilization, best practices, and community collaborations. The Mecklenberg County LME established the Best Practices Community Committee, comprising service providers, individuals and family members, advocacy agencies, community partners, interested community volunteers, and LME staff. Sub-committees addressed several areas, including employment. Based on their recommendations, a pilot project that uses an outcome-based funding model for follow-along employment services was developed.
