The immunological approach to neoplastic disease is gaining favor because malignant tumors are associated with immunodeficient states. Mesenchymal tumors have been seen in renal allograft recipients. The discovery of carcinoembryonic antigen, alfafetoprotein and other protein antigens associated with malignant diseases are other factors. Spontaneous regression of malignant neoplasm has been attributed to an effective immunologic process in the host. Identification of tumor specific antigens may explain these spontaneous tumor regressions. It may not be too optimistic to expect tumor vaccine of some sort within the next few years. B.C.G. and other nonspecific immunostimulants are under investigation for treatment of malignant …

Productivity Of The Roll-Streak Method To Perform Anaerobic Bacteriology In The Routine Clinical Laboratory, Thomas R. Neblett

Data presented are derived from one year of performing routine clinical anaerobic bacteriology by the roll-streak tube method. 826 specimens from 1508 (54.8%) contained anaerobes. 162 specimens were polymicrobic (19.6%) and contained 528 strains of a total of 1051 isolated (50.2 %). 479 blood specimens contained 525 strains of anaerobes. Propionibacterium, considered a contaminant, occurred 209/210 times in blood as the sole isolate. Bacteroides occurred alone or polymicrobically 308 times and comprised 54.7% of the blood isolates and 32.5% of the total anaerobic genera isolated. Clostridium, Peptococcus and Fusobacterium were the next most frequently occurring anaerobes. The roll-streak method is …

A convenience sample of nineteen patients composing group A, hospitalized for initial placement of permanent cardiac pacemakers, was studied. A programmed instructional test was prepared by the investiga- tors for this population. The question under study was, does programmed instruction for pacemaker patients affect immediate and delayed recall of information? Patients were pre-tested, presented with the programmed instruction package and given a post-test immediately upon completion of the package. They were retested later to evaluate retention level. An IQ test was administered at the same time as the pre-test. Statistical analysis using t-tests indicated significant differences between pre-test scores and …

The relationship between the number of persons per room in the home and the relative occurrence of stress-related morbidities of hospitalized patients is investigated. The social factors of age, social class, and level of social support also are examined as they relate to both density and morbidity type occurrence. The population under study is composed of all Kaiser-Permanente Medical Care Program members twenty years of age and older, discharged from Bess Kaiser Hospital during 1974, and whose diagnosed morbidity was included in specific morbidity categories. This study was designed to test the hypotheses that ambulatory cardiac patients given a special teaching program would have increased understanding about their medication, make fewer medication errors, and know more about undesirable effects and how to handle them than patients not included in the special teaching program. Forty-one ambulatory cardiac patients were, after a random start, alternately assigned to an experimental or to a control group. All the patients were taking one or more of three different medications before hospitalization, during their hospital stay, and after their discharge. Cushing's Disease: Spontaneous Remission Following Severe Headache, Raymond C. Mellinger

A young woman was found to have persistent Cushing's syndrome one year after surgical removal of both hyperplastic adrenal glands. Improvement followed pituitary irradiation butshe developed skin pigmentation. Remission of the adrenal hyperfunction occurred only after several months of therapy with o,p'-DDD. Within 15 months, the recurrent disorder required a second course of DDD with resulting adrenal insufficiency. Eighteen months later, however, steroid supplements were no longer required and increasing Cortisol levels indicated still another imminent relapse. There was no enlargement of the sella turcica. The patient had received no treatment whatever for nearly nine months when she experienced a …

Retrograde endoscopic pancreatic duct cannulation for evaluation of patients with chronic pancreatitis is reviewed over the last two years. Of 40 patients that were endoscoped, 30 had a successful pancreatogram. The X-ray findings fell into three groups: 1) single and multiple stenosis, 2) diffuse narrowing, and 3) apparently normal duct. This procedure proved of value in ruling out or delaying surgical operation in 10 persons. Twenty-one patients underwent operations. Thirteen had a 95% pancreatectomy, one had drainage of a necrotic collection thought to be a pseudocyst and one had a pseudocyst resected. One had a gastrojejunostomy and vagotomy bypassing a …

A case is reported of multiple myeloma in a 23-year-old man, including extensive clinical studies and postmortem examination. Multiple myeloma for this age group is considered to be extremely rare.

Multiple isolations of T. Cutaneum from the blood stream of two patients enabled the authors to characterize the morphology, biochemical reactions and antibiotic susceptibility of T. Cutaneum. This organism appears to be an opportunistic pathogen and responds promptly to intravenous Amphotericin B or oral 5-Fluorocytosine.

Bupivacaine As A Local Anesthetic For Hernia Repair, Joseph L. Ponka, James A. Sapala

Bupivacaine hydrochloride is a safe, useful local anesthetic agent for repair of groin hernias. Prolonged anesthesia, lasting six hours or longer, was observed In 94 of WO patients. No toxic effects involving the central nervous system were observed using a 0.25% solution with epinephrine. Hypotension and cardiac arrhythmia occurred in one patient. Wound complications, such as hematomas and ecchymosis, were more common with Bupivacaine than when Chlorprocaine was used. However, painstaking hemostasis, which required ligation of even the tiniest blood vessels, reduced the wound complications. As experience is gained in the use of Bupivacaine, complications will be reduced to a …

A 6-week study was made in the summer of 1971 as an initial effort to determine the extent of pollution that the three sewage disposal plants in Jessamine County, Kentucky, are contributing to its streams. With the rapid population increase in Lexington and nearby municipalities, this study should furnish a basis of comparison for future investigations. Eighteen collecting stations were established in riffle areas of Hickman and Jessamine Creeks, and coliform bacteria, macro-invertebrate populations, fish populations and chemical water quality of each riffle area were studied.

Comparative Analysis Of Hospital Utilization In Urban And Rural Counties, James R. Dingels

Journal of the Minnesota Academy of Science

Hospital utilization, when measured as patient days per population, exhibited generally insignificant relationships with factors concerning population characteristics, hospital resources, financial assistance, and degree of urbanization. Adjusted multivariate linear regression results indicated that number of physicians per population did possess significant effects, while other independent variables remained significant. Additional research, as well as refinement of present data, appear paramount to further validation procedures.

A school-based dental program was established at the Mary Hooker Elementary School under a grant from the Robert Wood Johnson Foundation. Administration was under the auspices of the University of Connecticut, Department of Pediatrics. The author participated in the organization of the program. Pertinent literature on evaluation of health care and dental care in particular are reviewed. An evaluation system for this program is developed, which can be extended to other dental care facilities.

We describe a method for measuring the release of fatty acids from endogenous substrates of human platelet homogenates and membranes. The method depends on the availability of lipids whose fatty acids are odd-chained and therefore suitable as internal reference compounds that, at the time of lipid extraction, can be added to an incubation to permit subsequent quantification of the content of free fatty acids or fatty acids esterified to specific lipids. We found four types of lipolytic activities in human platelets. In homogenates at pH 4.0 a triglyceride lipase operated as shown by the synchrony of triglyceride degradation and release …

Pedal lymphangiography was performed In 30 patients who subsequently underwent radical retropubic prostatectomy, and radical pelvic lymphadenectomy. In 24 of 30 patients, lymphangiograms correctly predicted the presence or absence of nodal Involvement. In spite of certain limitations of the technique. It is considered an essential diagnostic study in patients considered for possible radical prostatic surgery, or in patients who may be candidates for external megavoltage radiotherapy to the prostate gland and its lymphatic drainage.

Palliative Urinary Diversion In Adenocarcinoma Of The Prostate, R. C. Youngman, Joseph C. Cerny

Ten patients with inoperable prostatic carcinoma, producing ureteral obstruction and azotemia, underwent palliative urinary diversion. Good quality survivals of two months to three years were obtained. These results compare favorably with the good results seer) In gynecologic neoplasm of the bladder, colon and breast. In selected cases of carcinoma of the prostate, palliative urinary diversion is indicated when the prospects exist for further benefit from chemotherapy or radiotherapy.

Radical Retropubic Prostatectomy And Pelvic Lymphadenectomy In Carcinoma Of The Prostate, Riad N. Farah, Joseph C. Cerny

Henry Ford Hospital Medical Journal

An operative technique Is described which combines regional pelvic lymphadenectomy with radical prostatectomy. The technique has been used on 30 patients with no operative mortality. Morbidity has been within acceptable limits. Thus far, only one patient has shown recurrence of the neoplasm.
