Background: Background Chronic Obstructive Pulmonary Disease (COPD) is a common chronic condition causing significant morbidity and mortality. Patients with COPD report sudden episodes of deteriorating respiratory symptoms called exacerbations, often presumed to be precipitated by infections. COPD is in the top 5 causes of death globally. COPD exacerbations drive a large proportion of these deaths. Despite this, there is no standardised model of care for the sickest patients who need hospitalisations for exacerbations. Meanwhile, treatment for COPD exacerbations continues to be guided by symptoms, instead of underlying biology. Most patients are treated with unnecessary and harmful systemic corticosteroids. As an increasing number of individuals flee persecution on the basis of sexual orientation and gender identity expression (SOGIE), Canada has become a prominent destination for SOGIE refugees. The chronic stressors encountered by this marginalized population remain insufficiently studied, particularly in relation to physical health outcomes. This qualitative study investigates the cumulative health impacts of chronic stress among SOGIE refugees through the application of a cell-to-society framework, underpinned by Minority Stress Theory (MST). This dissertation explores the impact of the 2021 New York State Gender Recognition Act (GRA) on mental health outcomes and service utilization among non-binary adults in the public mental health system. The GRA introduced pivotal legal reforms—including the option for an “X” gender marker and self-attestation for gender identity changes on state-issued documents—designed to affirm gender diversity and reduce structural barriers for transgender and non-binary populations. Guided by gender affirmation theory and minority stress frameworks, this study investigates how these legal shifts relate to changes in mental health service engagement and psychological well-being among non-binary individuals.

Background: People living with diagnosed HIV (PLWDH) have a higher chance of co-infections and co-morbidities than the general population. When SARS-CoV-2 was first reported in China in December 2019 and later declared a pandemic, it was speculated that PLWDH might be more prone to SARS-CoV-2 infection and severe disease outcomes given their increased vulnerability to other respiratory infections like influenza, pneumococcal pneumonia and tuberculosis. For the heathcare field to be impactful in positive ways, providers should emerge from multifaceted academic backgrounds, equipped with an array of knowledge and skills to address the complex needs of different patients. Central to this is the idea that all aspects of an individual's identity and the intersectionality of these identities should be considered when providing care. BACKGROUND: The BCG vaccine induces trained immunity, an epigenetic-mediated increase in innate immune responsiveness. Therefore, this clinical trial evaluated if BCG-induced trained immunity could decrease coronavirus disease 2019 (COVID-19)-related frequency or severity.

METHODS: A double-blind, placebo-controlled clinical trial of healthcare workers randomized participants to vaccination with BCG TICE or placebo (saline). Enrollment included 529 healthcare workers randomized to receive BCG or placebo. Primary analysis evaluated COVID-19 disease frequency, while secondary analysis evaluated coronavirus immunity in a subset of participants. Study enrollment ceased early in December 2020 following introduction of COVID-19-specific vaccines.

Introduction: HIV integrates its proviral DNA into the host genome to establish persistent infection. To promote HIV clearance, we have designed an approach for selective elimination of host cells harboring replication-competent HIV (SECH), through inhibition of autophagy and anti-apoptotic molecules during viral reactivation. SECH approach can clear HIV-infected cells in approximately 50% humanized mice. However, the mechanisms for the resistance of reservoirs to depletion in mice with failure in HIV clearance are unclear.

Objective: Parotid gland neoplasms account for a large proportion of benign salivary gland tumors, with pleomorphic adenomas (PA) being the most common in the civilian population, followed by Warthin's tumor (WT). However, the distinct risk profiles of Veterans significantly influence the incidence and prevalence of salivary gland neoplasms. We investigated the diagnostic and management algorithms for benign parotid gland neoplasms in Veterans.

All-terrain vehicles (ATVs) pose significant injury risks, particularly among youth. These powerful machines are widely used across recreational and rural settings. West Virginia consistently ranks among the top five states for ATV-related deaths and injuries. Safety equipment and requirements are often not followed nor enforced, causing children and adolescents to be the most vulnerable group because they tend to not wear safety equipment such as helmets, ride on roadways, carry passengers, and operate oversized ATVS. Gestational age as a risk factor has a significant impact on fetal outcomes in pregnancies affected by preeclampsia. This research explores the interplay among noise levels, thermal conditions, and lighting intensity in an intensive care unit (ICU), focusing on preserving circadian rhythm and promoting nighttime sleep to advance patient-centric care. This investigation assesses lighting levels (Lux), natural versus artificial light ratios, ICU room temperature, and correlations with acoustic data during a field research period and examines the collective impact of patient exposure to sleep linked to delirium and health outcomes, addressing critical gaps in understanding. The objective of the study is to conduct a systematic review and methodological quality appraisal on studies reporting the development of health and wellbeing outcome measures for Māori (Indigenous people of New Zealand), identify common features and processes, and critically appraise the measures using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN). Key databases were searched using key terms in May 2022 without date limiters. Teaching food and health may be challenging, especially in classrooms where young people from different cultures, ethnicities, and religions meet. This article explores teachers’ navigation of ethical complexities when teaching food and health in classrooms with immigrant students. Five didactical strategies were identified: (1) The educare strategy, (2) The tiptoeing strategy, (3) The collegial support strategy, (4) The intercultural strategy, and (5) The pluralistic strategy. These didactical strategies are embedded in various educational settings. Current understanding of viral dynamics of SARS-CoV-2 and host responses driving the pathogenic mechanisms in COVID-19 is rapidly evolving. Here, we conducted a longitudinal study to investigate gene expression patterns during acute SARS-CoV-2 illness. Cases included SARS-CoV-2 infected individuals with extremely high viral loads early in their illness, individuals having low SARS-CoV-2 viral loads early in their infection, and individuals testing negative for SARS-CoV-2. We could identify widespread transcriptional host responses to SARS-CoV-2 infection that were initially most strongly manifested in patients with extremely high initial viral loads, then attenuating within the patient over time as viral loads decreased. BACKGROUND: Optimal management of fetuses diagnosed as small for gestational age based on an estimated fetal weight ofHowever, maternal bed rest has not been recommended based on the results of a randomized clinical trial that showed that maternal rest does not improve fetal growth in small-for-gestational-age fetuses. This study was conducted to revisit this question.

Human pluripotent stem cells (hPSCs) exist in multiple, transcriptionally distinct states and serve as powerful models for studying human development. Despite their significance, the molecular determinants and pathways governing these pluripotent states remain incompletely understood. Here, we demonstrate that transposable elements act as sensitive indicators of distinct pluripotent cell states. We engineered hPSCs with fluorescent reporters to capture the temporal expression dynamics of two state-specific transposable elements, LTR5_Hs, and MER51B. This dual reporter system enables real-time monitoring and isolation of stem cells transitioning from naïve to primed pluripotency and further towards differentiation, serving as a more accurate readout of pluripotency …

Background and objectives: We previously demonstrated that a telehealth-supported community health worker (CHW) intervention significantly improved clinical outcomes in diabetes care. However, the extent to which these benefits vary across different age groups remains unclear. This study evaluated the effectiveness of a CHW-led multidimensional diabetes intervention in reducing HbA1c across age groups.

Whole transcriptome sequencing (WTS/ RNA-Seq) is a ubiquitous tool for investigating cancer biology. RNA isolated from frozen sources limits possible studies for analysis of associations with phenotypes or clinical variables requiring long-term follow-up. Although good correlations are reported in RNA-Seq data from paired frozen and formalin fixed paraffin embedded (FFPE) samples, uncertainties regarding RNA quality, methods of extraction, and data reliability are hurdles to utilization of archival samples. Background & aims: Although it is well-known that ribosomes are critical for cell function, and their synthesis (known as ribosome biogenesis [RiBi]) is energy-intensive, surprisingly little is known about RiBi in vivo in adult tissue.

Methods: Using a mouse model with conditional deletion of Nat10, an essential gene for RiBi and subsequent translation of mRNA, we investigated the effects of RiBi blockade in vivo, with a focus on pancreatic acinar cells during homeostasis and tumorigenesis.

Objective: Through the brain-gut-microbiome axis, myriad psychological functions that affect behavior share a dynamic, bidirectional relationship with the intestinal microbiome. Little is known about the relationship between personality-a stable construct that influences social- and health-related behaviors-and the bacterial ecosystem. The authors of this exploratory study examined the relationship between general and maladaptive personality traits and the composition of the gut microbiome.

Purpose: To examine the relationship between Hemoglobin A1c (HbA1c) and asthma outcomes in an urban cohort of children with asthma.

Methods: The AIRWEIGHS Study was a randomized controlled clinical trial of an air cleaner intervention testing the hypothesis that overweight/obese children would experience greater improvement in asthma control compared to normal weight children. The study enrolled 164 children with asthma from Baltimore, MD and assessed HbA1c levels and asthma outcomes during clinic visits at baseline and three months. HbA1c levels were analyzed as a continuous measure and categorized as either normal (< 5.7%) or consistent with pre-diabetes (≥5.7%). Asthma outcomes included standardized questionnaires, spirometry, and fractional exhaled nitric oxide (FeNO). Generalized Estimating Equation (GEE) regression models were used to analyze the association between the HbA1c and asthma outcomes.
