Male circumcision reduces female-to-male HIV transmission. Hypothesized mechanisms for this protective effect include decreased HIV target cell recruitment and activation due to changes in the penis microbiome. We compared the coronal sulcus microbiota of men from a group of uncircumcised controls (n= 77) and from a circumcised intervention group (n = 79) at enrollment and year 1 follow-up in a randomized circumcision trial in Rakai, Uganda. We characterized microbiota using16S rRNA gene-based quantitative PCR (qPCR) and pyrosequencing, log response ratio (LRR), Bayesian classification, nonmetric multidimensional scaling (nMDS), and permutational multivariate analysis of variance (PerMANOVA). We explored the contribution of nitrosamine metabolism to lung cancer in a pilot investigation of genetic variation in CYP2B6, a high-affinity enzymatic activator of tobacco-specific nitrosamines with a negligible role in nicotine metabolism. Previously we found that variation in CYP2A6 and CHRNA5-CHRNA3-CHRNB4 combined to increase lung cancer risk in a case-control study in European American ever-smokers (n = 860). However, these genes are involved in the pharmacology of both nicotine, through which they alter smoking behaviours, and carcinogenic nitrosamines. A fundamental challenge in the treatment planning process of multi-fractional external-beam radiation therapy (EBRT) is the tradeoff between tumor control and normal tissue sparing in the presence of geometric uncertainties (GUs). To accommodate GUs, the conventional way is to use an empirical planning treatment volume (PTV) margin on the treatment target. However, it is difficult to determine a near-optimal PTV margin to ensure specified target coverage with as much normal tissue protection as achievable. Coverage optimized planning (COP) avoids this problem by optimizing dose in possible virtual treatment courses with GU models directly incorporated. A near-optimal dosimetric margin generated by …

The objective of this dissertation is to develop treatment planning techniques that have the potential to improve radiation therapy of time-dependent (4D) anatomy. Specifically, this study examines dose estimation, dose evaluation, and decision making in the context of optimizing lung cancer radiation therapy. Two methods of dose estimation are compared in patients with locally advanced and early stage lung cancer: dose computed on a single image (3D-dose) and deformably registered, accumulated dose (or 4D-dose). The results indicate that differences between 3D- and 4D- dose are not significant in organs at risk (OARs), however, 4D-dose to a moving lung cancer target …

Neglected tropical diseases (NTD) impair physical and cognitive development. This impairment makes it difficult to earn a living, restricts quality of life, and limits productivity in the work force. As a result, neglected tropical diseases trap the poor in a cycle of disease and poverty. Lymphatic filariasis (LF), also known as elephantiasis, is an NTD. It starts when microscopic, thread-like parasitic worms (Wuchereria bancrofti, Brugia timori, and Brugia malayi) are transmitted by mosquitoes. These worms infect and block the lymph nodes in the body, causing edema. Blockage in the lymphatic system and build-up of tissue and blood flow often occur …

Low birthweight increases the risk of infant mortality, morbidity and poor development. Maternal nutrition and stress influence birth size, but their combined effect is not known.We hypothesised that an early-invitation time to start a prenatal food supplementation programme could reduce the negative influence of prenatal maternal stress on birth size, and that effect would differ by infant sex. A cohort of 1041 pregnant women, who had delivered an infant, June 2003–March 2004, was sampled from among 3267 in the randomised controlled trial, Maternal Infant Nutritional Interventions Matlab, conducted in Matlab, Bangladesh. Introduction: Sexually Transmitted Diseases, including HIV/AIDS, continue to be a major burden in the United States. Sexual partnering behaviors contribute to the spread of STDs. Sexual concurrency has been shown to exponentially increase STD prevalence in populations. Serial monogamy with short periods between sexual partners also introduces risk. Methods: We identified sexually active men and women from the 2006-2010 National Survey of Family Growth (NSFG) and used sub sets for each particular study. Sexual partnering was defined as being concurrent, serially monogamous or monogamous in the previous year. Background: Studies suggest that ambient sunlight plays an important role in the pathogenesis of non-melanoma skin cancers (NMSC). However, there is ongoing controversy regarding the relevance of occupational exposure to natural and artificial ultraviolet radiation (UV) radiation.
Objectives: We investigated potential associations between natural and artificial UV radiation exposure at work with NMSC in a case-control study conducted in Hungary, Romania, and Slovakia. Methods: Occupational exposures were classified by expert assessment for 527 controls and 618 NMSC cases (515 basal cell carcinoma, BCC). Background: Cholera outbreaks result in significant morbidity worldwide. Transmission of the disease may be influenced by weather fluctuations, such as El Niño-Southern Oscillation (ENSO) and excessive rainfall, and socioeconomic status (SES), including income and education. El Niño events may influence many aspects of the climate, including the amount of rainfall occurring in a given year and surface water temperature. This study aimed to assess the influences of extreme weather and SES factors on cholera incidence in India during the seventh cholera pandemic (1961-2008).

Introduction: Community-based distribution (CBD) has been successfully applied to family planning (FP) services worldwide. It forms the basis for the large lady health worker (LHW) programme in Pakistan which serves a limited number of women with contraception services. Thus, the concept has seen limited application in Pakistan. We present the outcomes of a CBD model that was implemented in 49 districts across Pakistan by a non-government organization (NGO).

Environmentalists, social scientists, and economists have long critiqued the enduring impacts of the Green Revolution’s diffusion of agricultural technologies throughout the Global South. However, largely missing from the myriad analyses is the relationship between those technologies, namely pesticides, and health outcomes. This thesis explores the social and biological mechanisms through which excessive pesticide use culminated into adverse reproductive health outcomes for rural women in the Global South. Lead poisoning is a significant public health problem with paint from old housing exposing thousands of children and leading to negative health and social outcomes. Identifying the highest risk children exposed to lead is important to public health agencies. The purpose of this study was to evaluate and assess the efficacy of a new geographically-based lead risk model that when combined with a child's physical address, predicts the extent of a child's risk of lead poisoning on a numeric risk scale. The purpose of this paper is to investigate the hypothesis that outdoor daily walking, as an exercise, has an effect on the rate of mortality among those elderly people in the Iowa 65+ Rural Health Study (RHS). RHS is a prospective longitudinal cohort study of 8 years follow-up from 1981 to 1989. It consists of a random sample of 3,673 individuals (1,420 men and 2,253 women) aged 65 or older living in Washington and Iowa counties of the State of Iowa. Vitamin D is a steroid hormone, a group of fat-soluble pro-hormones, which encourages the absorption and metabolism of calcium and phosphorous. The requirement for the nutrient is high at particular stages of the lifecycle especially during pregnancy and infancy. Severe vitamin D deficiency during pregnancy may increase the risk of developing preeclampsia and gestational diabetes in pregnancy and rickets in the child. While vitamin D supplementation has been a policy recommendation in Ireland since 2007, at the time of this study, it was not a policy recommendation in Saudi Arabia. Pregnancy is, for each individual woman, situated in a particular historical, social, and cultural understanding of her own body; the experience cannot be divorced from these components that inform what is deemed unhealthy or inappropriate. Most social research regarding pregnancy and childbirth focuses on economic and political implications. This study explored personal narratives of Samoan women’s experience with pregnancy and childbirth to shed light on cultural aspects of events. This paper discusses how Philip Morris invoked strong American values to defend its own corporate speech as well as “smoker’s rights.” In particular, it examines Philip Morris Company’s Bill of Rights advertising campaign of the early 1990s, especially focusing on an advertisement featuring Everett Alvarez, a prisoner of war during Vietnam. This strategy reveals how Philip Morris was able to manipulate the public conversation about smoking from one about health and disease to one about human rights and freedoms.

According to the World Health Organization in 2013, obesity is the fifth leading risk for deaths around the world. Past research suggests that senior citizens often face challenges related to deteriorating physical and men- tal health, and the quality of their lives may suffer as a result. Past research also suggests that volunteering can improve the health and quality of life for seniors. In the present study, 451 volunteers enrolled in the Retired and Senior Volunteer Program (RSVP) completed surveys including questions regarding their volunteer experiences and how these experiences have affected their health and quality of life. The objective of our research project is to discover the perceptions about herbal remedies as alternative medicine among Cal Poly students. Specifically we ask, why are they chosen and how students gauge their effectiveness.
