Manipulation Of External Tachodynamometer By The Patient: Case Report, Chang Y. Lee, Michael P. H. Lau

External tachodynamometer has frequently been used to evaluate premature labor. In three cases, the patient was apparently able to record uterine contraction-like curves by means of abdominal pressure. In two of these cases, the patient was evidently using this manipulation to gain attention and support from her husband and/or family members. In the third case, described in this paper, because false curves were recorded, an emergency Caesarean section was performed under the impression that abruptio placenta had occurred.

Methodological Issues In Studying Treatment Effects In Patients With Cerebrovascular Disease, Mark W. Shatz

Evaluation of the neuropsychological effects of surgical treatment on cerebrovascular disease is beset by numerous methodological difficulties. These include problems specific to this patient population as well as others inherent in all retrospective studies. Five such problems are described: 1) nonrandomized subject selection; 2) dropout from follow-up; 3) natural history of cerebrovascular disease; 4) effects of hospitalization; and 5) the role of practice effects. This paper examines these methodological problems for their impact on our knowledge and proposes alternative research directions to address their shortcomings.

Carotid Artery Disease, Carotid Endarterectomy, And Behavior: A Critical Appraisal, Mark P. Kelly

We reviewed the recent literature on the relationship between carotid artery disease, carotid endarterectomy, and behavior. The methodological adequacy of each study and the complexities involved in interpreting behavioral changes in patients with carotid disease are emphasized. Experimental design issues qualify conclusions in all studies. The weight of the evidence suggests that carotid artery disease may result in cognitive impairment, and that carotid endarterectomy is followed by psychometrically determined cognitive improvement in some patients. Although characteristics of the subgroup of patients most likely to improve have been identified in preliminary investigations, the clinical significance of such improvement has not been …

The Effects Of Cerebrovascular Surgery On Behavior: What Has Been Demonstrated?, Laurence M. Binder

We studied the effect of the superficial temporal artery to middle cerebral artery bypass (STA-MCA) procedure on neuropsychological function. Nineteen patients with cerebrovascular disease of the anterior carotid circulation (12 on surgical and seven on medical therapy) received an extensive battery of neuropsychological tests; the average test-retest interval was eight weeks. Both groups improved on some measures, and no patients deteriorated. There was no evidence of greater improvement by the surgical group. The results are compatible with a spontaneous remission/practice effect interpretation. A review of the carotid endarterectomy and STA-MCA bypass literature suggests that the null hypothesis of no effect …

Longitudinal Pantothenic Acid Status Of Pregnant And Lactating Women, Won Oack Song

Pantothenic acid nutritional status was evaluated in a cohort of twenty-nine Caucasian pregnant women, aged 20-35, during their third trimester of pregnancy, at two-weeks postpartum and at three months postpartum. Seventeen non-pregnant, non-lactating women who had similar demographic backgrounds as the pregnant women were selected as a control group and followed the same experimental schedule as the pregnant women. A fasting blood sample, two days 24-h urine specimens and diet record were obtained for each of two days from each subject at each period.

The mean dietary pantothenic acid intakes of the women during pregnancy (5.30±1.74 mg/day) and during lactation …

Compression of the duodenum by the root of the mesentery is a rare pathological entity that has caused much controversy because it has been mistakenly identified as the "superior mesenteric artery syndrome" or the "vascular compression of the duodenum." We discuss the pathological anatomy and the diagnosis of the disease, and report an illustrative case. Some authors report many cases of the syndrome, while others doubt its existence. Before surgery is considered, the diagnosis should be established beyond doubt and a prolonged period of observation, medical treatment, and nutritional support should be provided. An Analysis Of A Sample Of Parents And University Freshmen In Regard To Their Attitudes Toward The Needs Of Sex Education In Community Schools, Linda C. Aleksa

This study attempted to investigate a sample of both parents' and college freshmens' attitudes toward sex education, and the content and timing of sex education in the Tidewater schools. Two Likert-type scales each containing ten statements were administered in order to survey attitudes. A statistically significant difference was found in scores on the attitude toward content and timing of sex education scale between parents and college freshmen. Parents demonstrated a more favorable attitude toward sex education as an integral component of the overall educational curriculum. Compulsory Treatment In Psychiatry: Some Reflections On Self-Determination, Patient Competency And Professional Expertise, Lawrence O. Gostin

Georgetown Law Faculty Publications and Other Works

In this article the author examines the rationale, in legal and policy terms, of the inextricable association traditionally formed between certification and incompetency. He argues that forming categories of people in which the law automatically dispenses with the requirement of seeking consent is fraught with conceptual inconsistencies and practical difficulties. He further argues that clinical judgments made without the consent of the patient should be made subject to an independent statutory review. Such a review procedure could also be adopted for treatments which are unusually hazardous, irreversible or not fully established even if the doctor purports to proceed with the …

The current study was designed: (1) to develop an educational tool on female masturbation capable of reaching a wide audience of women with accurate and complete information on female masturbation; (2) to provide initial validation for the further educational usefulness of this tool and (3) to assess its potential ability for improving overall sexual functioning in women by possibly altering negative attitudes toward masturbation and/or by dis-inhibiting masturbatory behavior.

Behcet's syndrome is a disease commonly found in Japan and in countries around the Mediterranean Sea. In the U.S., it is relatively uncommon. This report describes a 24-year-old Lebanese-American man with Behcet's syndrome who presented with symptoms four months after he developed arthritis and who had been initially diagnosed as having incomplete Reiter's syndrome. A literature review is Included.

Obstetric analgesia and anesthesia is an important concern today. The purpose of this study was to evaluate the effects of analgesia used in labor and delivery on the mother and newborn. The information obtained in this study evaluated the amount, time and types of analgesia and the effects on the mother and the newborn.

Previous research has indicated adverse effects of analgesia and anesthesia on both mother and newborn, but conclusive results have not been obtained. The present study was conducted as a service to ~e Norfolk Juvenile and Domestic Relations Court, to determine if the family counseling service administered by the Diversion Department, could significantly reduce the recidivism rate among juvenile status offenders (JSOs). A retrospective design was employed in order to examine the records of 74 JSOs, who came in contact with the Intake Department of the Norfolk Juvenile Court in 1978. The recidivism rate was recorded, and data analyzed from a two year intervention period. Systematic Treatment Of Chronic Pain With Antidepressants, Dietrich Blumer, Mary Heilbronn, Edgard Pedraza, Gerald Pope

Patients with chronic pain who do not have significant physical symptoms present a characteristic clinical-psychological profile and show a high incidence of depressive traits (inactivity, depressive mood, anhedonia, and insomnia). This syndrome is termed pain-prone disorder and represents a form of masked depression par excellence. A series of 129 patients with pain of 6.9 years' duration was divided into three treatment groups. Two psychiatrists prescribed antidepressants systematically by promptly increasing doses and by alternating antidepressants if indicated. Significant improvement or freedom from pain was achieved in most patients who complied with treatment.

The purpose of this study was to compare the proportion of women in the experimental group who had a successful breastfeeding experience following post-discharge nursing intervention with the proportion of women in the control group who had a successful breastfeeding experience without nursing intervention. Criteria defining success in breastfeeding were established before the study. Chi-square analysis was used to test for significance with the preset level of significance being α = 0.05.
