Imaging, Screening, Artificial Intelligence, And The Diagnostic Dilemma: An Epidemiologist's Response, John C. Lane

Issues in imaging, screening, and information processing discussed in a special issue of this Journal (Henry Ford Hosp Med J 1985;33:65-148) have implications for the decision-making algorithms of all clinicians who use those imaging and screening techniques. Epidemiologic and psychological research show that clinicians, like other professionals, do not obey the laws of conditional probability in their judgments of risk or outcomes under uncertainty. Although physicians cannot be expected to make complex probabilistic calculations every time they receive the result of a screening test, teachers of medicine should present algorithms that would allow the physician-in-training to make efficient use of …

Factors That Influence Prepared Childbirth Couples' Use Of Epidural Anesthesia For Labor And Delivery, Terri S. Chambers

The purpose of this study was to identify factors that influence a Prepared Childbirth couple's decision to utilize epidural anesthesia for labor and delivery. Review of the literature indicated a high correlation between epidural use for uncomplicated labors and deliveries and a high incidence of maternal complications and low neonatal neurological and behavioral scores.

A sample of twenty-nine couples was used in this prospective, non-experimental field study for the research. Data were collected utilizing two developed demographic and factual event tools, Rotter's Internal-External Locus of Control Tool and Spanier's Dyadic Marital Adjustment Tool.

The importance of mind, emotion, and behavior as they relate to the heart, particularly coronary artery disease, is growing in recognition. A review of the research shows increasingly sophisticated demonstrations of links unifying heart function and mind. "Psychocardiology" would be an appropriate term for the diverse areas of interest which focus on the psychosocial correlates of heart disease. This paper illustrates this concept through a review of psychosocial and behavioral aspects of coronary artery disease.

This article will study and evaluate the efforts of the state and the federal government to approach the problem of procreational autonomy and to resolve the perimeters of its recognition and application. In order to posit a framework for principled decision making, an analysis of the Australian and British efforts to deal with this problem, respectively, through their Waller and Warnock Commissions, will be undertaken together with a consideration of the famous case of Melbourne's "orphan" Rios embryos where, for the very first time, the issue of excess fertilized embryos was presented.

This edition of SEEDS focuses on a project developed to integrate low-income women into Jamaica's construction industry. In two years, 34 women passed through the project's basic training and skills upgrading courses. More than 90 percent of these women became employed, the majority as masons and carpenters. The story of how this field was identified as a potential source of income for women, and how the project developed and evolved in response to changing circumstances, presents many useful lessons. Decision making for catastrophically ill newborns has recently emerged as a social issue. Advances in biomedical technology and practice, and changes in other economic, social and political factors have led to controversy about norms to guide treatment choice. While much has been written on how such decisions should be made, there has been little social science research on how such decisions are actually made.

Since acquired immune deficiency syndrome (AIDS) was first identified in I98I, its rate of spread among a primarily young and vibrant population has chilled the medical and lay communities. Today, the public response is sober and oriented toward the examination of specific policies that could lessen the impact of the disease. After six years' experience it is now feasible to propose a strategy for combating AIDS. Consensus around the policies outlined in this article should form the nucleus of the public health strategy to combat AIDS before the intervention of an effective vaccine or treatment.

Although lead encephalopathy is rare, lead intoxication continues to be a significant problem in Detroit. Neuropsychological effects may occur even at low levels of lead. Since treatment of lead poisoning consists of a painful series of injections, it is imperative that more attention be given to the prevention of this disease. To confirm the magnitude of the problems, our experience with lead poisoning during a three-year period at Henry Ford Hospital's Pediatric Department is reviewed.

The risk for professional burnout has become commonly recognized and in the past several years has been a focus of research efforts. Nursing is one of the professions generating a high level of stress which results in burnout.

The purpose of this study was to test the effects of an intervention program for nurses who felt they were experiencing burnout. Nursing burnout was defined as a chronic condition of stress among nurses, which leads to a syndrome of emotional exhaustion, depersonalization, and lack of personal accomplishment. This study, conducted by a Health Maintenance Organization in Southern California, evaluated the impact of a stress management program in reducing stress/anxiety, stressors/hassles, and perception of stressors. Impact of the program in reducing negative coping mechanisms and increasing positive coping mechanisms as well as the retention of stress management training over a three month follow-up period was examined.

A quasi-experimental -- non-equivalent control group design was used. The experimental group consisted of 54 clients who registered for stress management classes. The control group consisted of 105 matched subjects selected from a larger group of 212. All subjects were tested prior …

The effectiveness of stimulation therapy for the cognitively impaired elderly living in an institution was assessed. Residents with varying degrees of cognitive impairment were treated with twenty, thirty-minute daily periods of reality orientation, reminiscence, resocialization, remotivation, and other stimulating therapies and were compared with a control group_ receiving no intervention. Treatment effects were assessed with a variety of instruments reflecting functioning across several domains. Included were ability to perform both basic self-care functions and activities of daily living, cognitive performance, affect, and satisfaction with the care received and the living environment in the nursing home. Nonadherence to therapeutic regimens which necessitate major lifestyle change is recognized as one of the most serious problems in medical practice today. Attempts to prevent and control chronic disease, including the development and progression of cardiovascular disease, are only effective when recommendations to the patient are carried out. Few studies have systematically studied the relationship of psychosocial and behavioral factors to adherence outside the theoretical framework of a specific behavioral model; no study has done so with a carefully measured diet as the outcome variable in a controlled prospective clinical trial with patients suffering from cardiovascular disease.

In assessing professional caregivers of Alzheimer's Disease victims, this study tested whether, 1) knowledge would influence perceptions of burden in providing care to such patients, and if 2) individual locus of control would influence perceptions of burden in caring for; Alzheimer's I patients. Professional nurses (BSN's, RN's, and LPN's) from nursing banes, hospitals and psychiatric facilities were asked to complete a three part survey. Studies have confirmed that many hemodialysis patients experience numerous psychological difficulties as a result of their physical illness. One behavioral manifestation of these psychological states is noncompliance to imposed dietary restrictions. In spite of life threatening implications, many hemodialysis patients find it very difficult to maintain dietary compliance.

Recently, guided imagery and progressive relaxation have been used as methods to assist clients to overcome negative psychological states. In most cases, the treatments are adjunctive to on-going counseling or psychotherapy. The purpose of this study was to determine predictors of smoking behavior for the senior student nurses in the state of Alabama using Ajzen and Fishbein's Theory of Reasoned Action. Smoking prevalence by level of educational preparation and selected health behaviors were examined for relationship to cigarette use.

A sample of 11 schools of nursing were selected from Alabama's 34, using a random numbers table. There were 13 total programs as two of the schools had both A.D. and B.S. nursing programs. Ce rapport de recherche est issu d'un projet-pilote réalisé dans 7 compagnies forestières, dont quatre dans la région du Saguenay - Lac-St-Jean trois de l'Abitibi-Témiscamingue. Au total, 89 accidents du travail ont été documentés à partir de questionnaires complétés par chaque accidenté et son supérieur immédiat. Cette cueillette d'information s'est déroulée à l'automne 1983 et à l'hiver 1984.

L'intention du projet-pilote était de valider une grille d'analyse d'accidents/incidents du travail, élaborée en collaboration avec différents intervenants des milieux de travail, à partir d'une problématique de la sécurité du travail axée principalement sur l'organisation technique et humaine du travail.

Background: There is emerging interest in obesity and its prevalence, outcomes, and management in people with multiple sclerosis (MS). Body mass index (BMI) is the traditional marker of obesity in MS, whereas body composition, inclusive of specific body tissue compartments (e.g., fat, bone, and muscle), is often overlooked despite its relevance.

Objective: This narrative review (a) underscored the use and utility of dual-energy X-ray absorptiometry (DEXA) as an accurate and reliable measure of body composition; (b) thematically analyzed and synthesized the current evidence regarding body composition (using DEXA); and (c) determined gaps to be addressed in future research.

Screening of first-degree relatives of patients with medullary thyroid cancer (MTC) gave normal values of pentagastrin-stimulated serum calcitonin and 24-hour urinary catecholamine levels in the relatives of 18 of 22 patients. This result is considered to be valid evidence for sporadic MTC. Absence of C-cell hyperplasia maybe another indication of sporadic MTC. Four hereditary MTCs were represented by one fully expressed MEN-2B patient without affected relatives, one fully expressed MEN-2A case in one family, and two first cousins with MTC as the only MEN-2A lesion in another family. Fourteen relatives of the MEN-2A patients had elevated serum calcitonin levels, and …

Caring For The Sick Poor: The State Almshouse At Bridgewater, 1854-1887, Lucille O'Connell

Bridgewater Review

The number of helpless poor in Massachusetts increased during the period of the 1840s, and the problem was greatly magnified by Irish immigration to the United States. Too large a problem to be solved on the local level, the Board of Commissioners of Alien Passengers recommended, in 1852, that the State establish three almshouses. Accordingly, the institutions were established at Bridgewater, Monson to the west, and Tewksbury to the north of Boston, and were opened in May 1854. By the end of the first year the number of inmates far exceeded expectations.
