At the conclusion of the Paris AIDS Summit in 1994, 42 governments issued a declaration supporting the greater involvement of people living with HIV/AIDS (PLHA) in policy formulation and service delivery. Despite growing recognition of its importance, there has been little research that examines PLHA involvement in the delivery of prevention, care, and support services in developing countries and its effects on PLHA, others affected by HIV/AIDS, and nongovernmental organizations (NGOs). To address this gap, the Horizons Program and the International HIV/AIDS Alliance conducted a study of PLHA involvement in NGOs in Burkina Faso, Ecuador, Maharashtra State, India, and Zambia …

Although medicine possesses the knowledge and technology for preventing or relieving most pain, poor pain control is still widespread. Unrelieved pain causes unnecessary suffering and increases health care expenditures. Among the barriers to improving pain control are poor provider education in pain management, misguided beliefs about the inevitability of pain and the dangers of pain medication, provider resistance to changing practice patterns, and administrative resistance to implementing improvements that incur short-term costs but lead to long-term savings. In short, poor pain relief in America's health care institutions is a system issue, and improvement requires a system-wide change. An effective program …

The Impact Of Family Planning Household Service Delivery On Women's Status In Bangladesh, James F. Phillips, Mian Bazle Hossain

Since 1982, the Maternal and Child Health and Family Planning Extension Project has compiled longitudinal panel data on rural women’s contact with household service providers who visit homes to discuss family planning and offer services to women on request. This study tests the hypothesis that home-based services reinforce customs of purdah (female seclusion) by sustaining the dependency and isolation of the women served by the program. Results show that household services improve women’s status. This effect is largely attributable to the impact of outreach on effective fertility regulation. Findings do not support the hypothesis that household service delivery is detrimental …

This technical assistance project supports the goal of the Asia Near East Operations Research and Technical Assistance (ANE OR/TA) project of helping strengthen family planning (FP)/reproductive health (RH) operations research (OR) capabilities in the Philippines. In an expansion of its efforts to fight a growing HIV epidemic and high teenage pregnancy rates, the Government of Botswana is encouraging nongovernmental organizations (NG0s) to be more active in providing reproductive health (RH) services, and especially HIV/AIDS education and care, to adolescents. NG0s have the potential to assume a larger role as health and education providers as well as implement pilot projects that the public health sector could learn from to increase the availability and quality of RH services for youth. Descriptive Epidemiology Of Physical Activity In Adolescents, Russell R. Pate, Barbara J. Long, Gregory W. Heath

This paper reviews the descriptive epidemiology of physical activity in adolescents. Large population-based studies were reviewed, along with smaller studies using objective monitoring of physical activity. Estimates showed that adolescents engage in physical activity of any intensity for a mean of one hour per day. Approximately two thirds of males and one quarter of females participate in moderate to vigorous activity for 20 min 3 or more days per week. Activity levels decline with increasing age across adolescence, and this decrease is more marked in females than in males. Comparison of these data to physical activity guidelines for adolescents suggests …

This study involved the development of an instrument to identify and quantify sources of stress within a give profession. The purpose of this study was to identify the areas of responsibility that were perceived by worksite health promotion directors as most important to the success of their organization's health promotion program. A survey instrument was designed and mailed to 111 health promotion directors in the state of Iowa, Minnesota, and Wisconsin that were 1992 members of the Association of Fitness in Business (AFB). Subjects were asked to rate the importance that 27 responsibilities associated with four areas of competence receive within their organization. This study investigated the financial performance of not-for-profit hospitals in 10 Southern states acquired by either the for-profit or not-for-profit multihospital systems between the years 1978 through 1982. The impact of system affiliation on acquired hospitals was investigated by looking at average financial performance from the two years before acquisition to 1984/1985. Differences between the performance of hospitals acquired by for-profit and not-for-profit multihospital systems were examined as well. with regard to the latter, major findings revealed both for-profit and not-for-profit multihospital systems increased debt in acquired hospitals and made improvements to plant and equipment.
