Abstract
Background
Parturition has been widely described as an immunological response; however, it is unknown how this is triggered. We hypothesized that an early event in parturition is an increased responsiveness of peripheral leukocytes to chemotactic stimuli expressed by reproductive tissues, and this precedes expression of tissue chemotactic activity, uterine activation and the systemic progesterone/estradiol shift.
Methods
Tissues and blood were collected from pregnant Long-Evans rats on gestational days (GD) 17, 20 and 22 (term gestation). We employed a validated Boyden chamber assay, flow cytometry, quantitative real time-polymerase chain reaction, and enzyme-linked immunosorbent assays.
Results
We found that GD20 maternal …

Division of Internal Medicine Faculty Papers & Presentations
BACKGROUND: This study aims to evaluate the length of time elapsed between reports of the same incidents related to avian flu and H1N1 outbreaks published by the WHO and ProMED-mail, the two major global health surveillance systems, before and after the amendment of the International Health Regulations in 2005 (IHR 2005) and to explore the association between country transparency and this timeliness gap.
METHODS: We recorded the initial release dates of each report related to avian flu or H1N1 listed on the WHO Disease Outbreak News site and the matching outbreak report from ProMED-mail, a non-governmental program for monitoring emerging …
Evaluation Of Mass Media Campaigns To Change Smokers' Knowledge, Attitudes, And Behaviors In China And Taiwan, Li-Ling Huang
Evaluation Of Mass Media Campaigns To Change Smokers' Knowledge, Attitudes, And Behaviors In China And Taiwan, Li-Ling Huang
Theses and Dissertations
Research in high income countries (HICs) suggest that anti-smoking television advertisements are most effective if they contain emotionally evocative graphic messages or personal testimonials that depict serious consequences from smoking. The ability to effectively translate these messaging strategies to low- and middle-income (LMICs) countries outside of Western cultures remains understudied. It is critical to determine which smoking cessation messages work best in LMICs because they increasingly bear the global burden of tobacco-related disease. To date there is limited evidence on the efficacy of tobacco control mass media campaigns and the relative effectiveness of different tobacco control messaging strategies in China …
Race, Ethnicity, And Differential Perceptions Of Sexual Victimization (Assault), Kaleea Regina Lewis
Race, Ethnicity, And Differential Perceptions Of Sexual Victimization (Assault), Kaleea Regina Lewis
Theses and Dissertations
The purpose of this pilot study is to ascertain how a sample of female college students perceives a hypothetical victim of a fabricated sexual assault. Participant perceptions of the sexual assault were explored through three distinct parameters: (1) victim responsibility attributes, (2) levels of trauma suffered due to the experience, and (3) levels of social support. This study addressed the two following research questions: (1) how does the hypothetical sexual assault victim's race and ethnicity affect how the study parameters are assessed and (2) how do participants perceive a hypothetical sexual assault victim, when presented in different races and ethnicities, …
Promoting Early Childhood Development Through Community Health Worker Interventions: Implementation And Measurement Issues In Rural Pakistan, Zaeem Haq
Theses and Dissertations
Appropriate nutrition and psychosocial stimulation during the first three years of life can have long-lasting effects on a child's health and development. However, not all children in the world receive adequate nutrition and psychosocial stimulation during this period. There has been enormous interest in developing the evidence on the most effective ways of implementing and assessing early childhood development (ECD) promotion interventions in resource poor settings. Despite the interest, the integration of ECD interventions into pre-existing public health programs, and assessment of these interventions in extended family, rural household environment have not been addressed.
The dissertation research presented here was …
Fishing And Fish Consumption Patterns In The Gullah/Geechee Sea Island Population, Jamelle Heyward Ellis
Fishing And Fish Consumption Patterns In The Gullah/Geechee Sea Island Population, Jamelle Heyward Ellis
Theses and Dissertations
The Gullah/Geechee (G/G) people are descendants of West African slaves who remained in seclusion on the Sea Islands of South Carolina, North Carolina, Georgia, and Florida until the end of the Civil War in 1865. In addition to retaining their unique cultural language, they also remain largely a fishing subsistence population. Decades of urban development have reduced the G/G population's access to traditional fishing locations and have increased exposures to environmental contaminants in some bodies of water that are frequently fished by the G/G. Approximately 98.4% of SC rivers and streams and 77.6% of lakes have been assessed for aquatic …
Bringing Families Together: Parent-Student Reunification Procedures In School Crisis Planning, Ashleigh Welko
Bringing Families Together: Parent-Student Reunification Procedures In School Crisis Planning, Ashleigh Welko
Master of Public Health Program Student Publications
The purpose of this paper is to evaluate and review current best practices with regard to parent-student reunification procedures following crises in schools. No national recommendations for school crisis planning exist, resulting in varying degrees of completeness in school planning. Parent-student reunification is one of the most frequently ignored or under-developed aspects of school emergency planning, yet a poorly developed reunification plan has the potential to become an enormous source of chaos in the aftermath of school crisis. Expedient reunification reduces the potential for lasting trauma to both children and their parents. A best practices review of existing literature was …

Fetal Deaths Related To Congenital Anomalies In The State Of Ohio: 2006-2010, Ashley M. Rhoby
Master of Public Health Program Student Publications
This report analyzes the 2006-2010 fetal death files for the state of Ohio by a variety of maternal and fetal characteristics. The issue of fetal deaths related to congenital anomalies is a growing concern as very few epidemiologic studies have been conducted in order to assess the correlation between maternal factors and the development of congenital anomalies. For this study, maternal demographics, the presence of a congenital anomaly, and maternal risks towards the pregnancy were analyzed. A Chi-Square analysis to test for variance between fetal deaths with and without the presence of a congenital anomaly was conducted. The results showed …
Parasites In Paraguay: An Analysis Of The Presentation And Management Of Intestinal Parasites By Short-Term Medical Mission Volunteers In Paraguay, Robert A. Eick
Master of Public Health Program Student Publications
Introduction. International medical aid in the form of short-term medical missions (STTMs) receives more than $250 million in annual donations. Needs assessments prior to the STTM and outcomes analysis for improvement are often lacking. Objective. Describe the patient population attended to during a one-week STTM in Paraguay, South America, while examining “parasites” as a presenting concern, as a diagnosis by the providers, and the prescription of antiparasitic treatment. This analysis identified variance in intestinal parasite presentation and management by age, gender, and location. The administration frequency of antiparasitic medication was considered as prophylactic versus disease-treatment based.
Methods. This study was …
A Case For Gun Violence Prevention In Medical And Public Health Curriculum, Shaun Hamilton
A Case For Gun Violence Prevention In Medical And Public Health Curriculum, Shaun Hamilton
Master of Public Health Program Student Publications
Gun violence profoundly impacts the landscape of violence in the United States. The costs of gun injuries are, estimated to be $100 billion each year in the United States. Although many physicians and health professionals agree that they have a role to play in the prevention of violence (American Academy of Pediatrics [AAP], 2002), there is a distinct disconnect between perceived responsibility and actual practice (Sidelinger, Guerrero, Rodriguez-Frau, & Mirabal-Colon, 2005). Physicians inherently have a role to play in the prevention of violence. The nature of their occupation places them in the “process” of violence in the form of treatment. …

The Impact Of Discontinuing Coverage Of Second Generation Antihistamines In A Managed Care Organization, Matthew D. Harman
Theses and Dissertations--Public Health (M.P.H. & Dr.P.H.)
ABSTRACT Background: Second generation antihistamines (SGAs) are approved to treat seasonal and/or perennial allergic rhinitis (AR) and chronic idiopathic urticaria (CIU). It is estimated that 82% of Americans with AR use antihistamines, with the majority using SGAs due to their low side effect profile. As policy, over-the-counter (OTC) SGAs were not covered in this health plan population (cetirizine and loratadine products were available OTC prior to the study period, while fexofenadine products became available OTC during the study period). On January 1, 2012, the policy was extended to remove coverage of prescription-only SGAs (included desloratadine and levocetirizine products). Objectives: To …

Reproductive Health
In late 2006, the Kenyan Ministry of Health (MOH), the Population Council, and Population Services International (PSI) launched an initiative to mainstream emergency contraception (EC) in Kenya. The initiative included a core set of activities aimed at improving overall awareness of EC across the country and strengthening the quality of EC services in both the public and private sectors. It was intended to serve as a model for other countries interested in improving access to EC, and to generate in-depth knowledge on EC program strategies and utilization characteristics in sub-Saharan Africa. The MOH drew lessons and experiences from the initiative …

The Unfinished Agenda To Meet Fp2020 Goals: 12 Actions To Fill Critical Evidence Gaps, Ian Askew, Martha Brady
Reproductive Health
More than 200 million women in the developing world who want to avoid pregnancy are not using a modern contraceptive method. The international community convened at the London Family Planning Summit in July 2012 and strengthened their resolve to address unmet need. National governments, civil society, and the private sector committed US$2.6 billion toward the goal of making contraceptive information, services, and supplies available to an additional 120 million women and girls by 2020. This book elaborates on one of two influential publications disseminated by the Population Council that assess the situation: Reviewing the Evidence and Identifying Gaps in Family …

Quality Of Care And Client Willingness To Pay For Family Planning Services At Marie Stopes International In Burkina Faso, Placide Tapsoba, Dalomi Bahan, Emily Forsyth Queen, Gisele Kaboré, Sally Hughes
Reproductive Health
Many sexual and reproductive health programs operating in a limited-resource setting face a challenging balancing act between making services accessible and affordable to clients, including those most in need, and remaining financially sustainable. To achieve these dual objectives, the Population Council, with Marie Stopes International in Burkina Faso (MSI BF), launched a willingness-to-pay (WTP) study that examined the current situation and numerous pricing options, proposing optimal prices for products and services. The process of implementing the WTP survey was insightful for MSI BF as it provided evidence on client satisfaction with services and willingness to contribute to the cost of …

Reviewing The Evidence And Identifying Gaps In Family Planning Research: The Unfinished Agenda To Meet Fp2020 Goals, Ian Askew, Martha Brady
Reproductive Health
The Population Council produced this paper to help guide discussions and considerations regarding the key evidence gaps and research investments needed to achieve the FP2020 goal and objectives. The paper focuses primarily on the social science, implementation, and operations research that will be needed to achieve the first three objectives. Research shows that through high-quality voluntary family planning (FP) programs, governments can reduce fertility and generate substantial improvements in health, wealth, human rights, and education. Family planning programs for the 21st century will require thoughtful design—engaging both public and private sectors—to meet the growing need for safe and effective FP …

Comprendre Les Grossesses Non Désirées Au Sénégal: Rapport Du Profil Pays, Kaila Clarke, Mohamadou Sall, Babacar Mane, Nafissatou Diop, Bocar Mamadou Daff
Reproductive Health

Understanding Unintended Pregnancy In Bangladesh: Country Profile Report, Fauzia Akhter Huda, Sabiha Chowdhuri, Yolande Robertson, Noushin Islam, Bidhan Krishna Sarker, Ashrafi Jahan Azmi, Laura Reichenbach
Reproductive Health
The objective of this report is to identify the determinants of unintended pregnancy and unmet need for family planning in Bangladesh and therefore provide a strong body of evidence that will contribute to issue identification, evidence generation, and communication for use of evidence in policy and programming. The evidence generated can be used to find ways to reduce the rate of unintended pregnancy and hence reduce the risk of abortion-related morbidity and mortality; ultimately this will aid Bangladeshi couples in reaching their fertility goals. Results demonstrate that Bangladesh has shown progress and promise in several areas of family planning and …

Insights Into Unmet Need In Senegal, Kazuyo Machiyama, John C. Cleland
Reproductive Health
Results presented in this STEP UP Research Report show that over half of women having unmet need for family planning in Senegal did not intend to use family planning in the future and this was consistent across all population strata. Reasons for non-use confirm the existence of widespread opposition to the use of modern contraception. Unfamiliarity and lack of information is likely to be a reason for such opposition; initial suspicion of contraception has also been documented. Thus a first policy priority, and one that is relatively easy to achieve, is to increase knowledge through the mass media and localized …
