Productivity Of The Roll-Streak Method To Perform Anaerobic Bacteriology In The Routine Clinical Laboratory, Thomas R. Neblett

Data presented are derived from one year of performing routine clinical anaerobic bacteriology by the roll-streak tube method. 826 specimens from 1508 (54.8%) contained anaerobes. 162 specimens were polymicrobic (19.6%) and contained 528 strains of a total of 1051 isolated (50.2 %). 479 blood specimens contained 525 strains of anaerobes. Propionibacterium, considered a contaminant, occurred 209/210 times in blood as the sole isolate. Bacteroides occurred alone or polymicrobically 308 times and comprised 54.7% of the blood isolates and 32.5% of the total anaerobic genera isolated. Clostridium, Peptococcus and Fusobacterium were the next most frequently occurring anaerobes. The roll-streak method is …

This study was designed to test the hypotheses that ambulatory cardiac patients given a special teaching program would have increased understanding about their medication, make fewer medication errors, and know more about undesirable effects and how to handle them than patients not included in the special teaching program. Forty-one ambulatory cardiac patients were, after a random start, alternately assigned to an experimental or to a control group. All the patients were taking one or more of three different medications before hospitalization, during their hospital stay, and after their discharge. Three medications, digoxin (lanoxin), lasix, and nitroglycerin, were studied. The program …

The bacterial flora of acute biliary tract disease has not been completely elucidated. The organisms identified most often were coliforms. Anaerobic culture technics were not precise enough to provide proper identification. In a 12-month study of 250 patients operated upon for inflammatory gallbladder disease, culture specimens were collected in a most strict and rigorous fashion. It was hoped that a more exact understanding of the relationship between bacteria and inflammatory biliary tract disease might be appreciated. Important technics were developed for gathering specimens, delivery to the laboratory and culturing. More meaningful results are to be expected In the future.

It was the purpose of this study to identify the attitudes of a selected group of women which influenced their participation in Papanicolaou testing. These attitudes were inferred from the reasons women gave for obtaining the test. The subjects were randomly selected from the telephone book of a medium-sized town in Southern California. The 135 women who participated in the telephone interview were considered to be the respondents in the study.

A review of the literature revealed that through the annual use of the Papanicolaou smear, approximately 11,000 deaths every year from uterine and cervical cancer could be prevented, Attitudes …

Synovial fluid from a case of pyarthrosis was cultured for fungi, mycobacteria, aerobes and anaerobes with negative results. A Cell Wall Deficient (CWD) form was isolated in numerous cultures which showed the physico-chemical reactions characteristic of wall deficient M. tuberculosis.

The authors' experience with the rare entity of sclerosing cholangitis suggests that several anatomical patterns of involvement of the extrahepatic biliary tract may occur. The involvement may include the intraluminal ducts segmentally or diffusely, or the process may primarily be external to the ducts as pericholedochitis. It may or may not represent one component of a systemic disease, particulady autoimmune disease. Although involvement may be diffuse, progression may be slow and compatible with many years of life. The hypertension which follows an intrinsic increase in pulmonary vascular resistance may be easily diagnosed by utilizing current catheterization and radiologic techniques. The real challenge is establishing a rational etiology in a given case before final pathological examination. Data from 33 proven cases are presented in an attempt to show that most ofthe proposed etiologic concepts may be represented in a large clinical series. Congenital and familial factors, multiple pulmonary embolization with resulting vasospastic sequela, arteritis, and degenerative states, were encountered, together with such specific clinical entities as scleroderma and systemic lupus erythematosus. A 55-year-old man had a large superior mediastinal tumor producing obstructive symptoms. The tumor was removed through an upper median sternotomy incision. It was found to be composed of so-called Hurthle-cells, which are of thyroid origin, and the pathologist's interpretation was that it was of low grade malignancy. A pulmonary metastatic nodule was excised five years later, and 8 1/2 years after the original operation, a nodule was removed from a pectoral muscle. The patient lived 12 years and then expired with multiple metastases, but he was relatively asymptomatic until the last few months.

Life-endangering effects of deep abscesses make early and precise identification and localization important to permit adequate surgical drainage. An experiment was undertaken to study the feasibility of detecting abscesses by using leucocytes tagged with radioactive isotopes. For the labeling, 51chromium appeared preferable to 131iodine or 99technetium. Leucocytes so labelled will concentrate sufficiently in abscesses to permit identification of abscesses by scintillation scanning. For clinical application, techniques require further simplification and refinement to eliminate interference by collection of radioactive isotopes in the liver and spleen.

Proteolytic activity of Streptococcus cremoris HP, P2 and AM2; Streptococcus lactis-C2, ML8 and a commercial mixed culture was determined in skimmilk and whey by measuring the increase in Absorbency at 274.5 nm of a pH 4.6 soluble filtrate, following incubation in a 2.5% casein solution. Proteolytic activity, rate of acid production and growth rate decreased in order of C2, HP, mixed culture, AM2, ML8 and P2 at a regulated pH (6.1 - 6.2) and 32 C. Quantitative histologic measurements of haversian bone remodelling were made of mineralized rib cross sections in 13 healthy rhesus monkeys. With the aid of previous tetracycline labelling, coupled with the Villanueva Osteochrome stain, Frost's quantitative bone analysis revealed that: a) the mean haversian bone formation rate is 0.359 z 0.186 mm2/mm2/yr; b) the mean number of osteoid seams/mm2 is 4.03 ± 1.64; c) the mean appositional rale is 1 33 ± 0 23 microns/day while the radial closure rate when all the labelled systems are corrected is 0.42 r 0.080 mm/year; d) the mean circumference of …

Two hundred patients have been reviewed in whom culdoscopy or laparoscopy was performed for infertility study. During these two procedures, methylene blue was injected through an endocervical cannula for detection of tubal patency under direct visualization. When compared with 89 patients tested by utero-tubal insufflation and hystero-salpingography, results indicated that direct visualization of the tubes and dye hydrotubation appears to be the most reliable method. HSG proved to be the second most reliable method with a failure rate of 19.2%. The least accurate procedure in this study was UTI with CO2 gas, where the failure rate was 24.7%. The …

The records of 13 patients with proven dissecting aortic aneurysm were studied with particular attention to the diagnosis, treatment, and prognosis in each case. The three essential steps in reaching an early correct diagnosis included (1) history and physical examination, (2) serial chest x-rays and (3) aortography. Electrocardiogram and SCOT and LDH measurements seem to be of value only when incompatible with the diagnosis of acute myocardial infarction. Significant differences between Type 1 and Type 3 dissections were observed in their clinical manifestations, results of treatment and natural prognosis.

The various methods for diagnosis of gonorrhea are reviewed. Gram-stained smear of urethral exudate in the mate is appropriate, but for the female emphasis is placed on the use of culture on Thayer-Martin medium. Currently effective modalities of treatment are detailed, pointing out that higher doses of antibiotics are generally required for females with pelvic inflammatory disease than for males with acute gonococcal urethritis. The necessity for follow-up is also discussed.

The study was to provide an approach to help teen-age unwed mothers increase their feelings of self-esteem and thereby attain a conceptual buffer for better coping with life. Self-esteem was defined as individual perception of the worth of oneself relative to that of significant others. The hypothesis tested was that the participating relationship a community health nurse has with a teen-age unwed mother will significantly increase the mother's self-esteem as measured by statistical Q-Sort analysis.

Arthrography is a safe and relatively simple diagnostic procedure which provides an accurate and graphic preoperative means of defining pathology of the knee. In those cases where the expeditious evaluation and diagnosis of knee injury are of prime importance, ie, the athlete and industrial compensation case, arthrography provides a significant contribution. The clinical pathological features, salient anatomy and technic of study of internal derangements of the knee are discussed. Selected double contrast arthrograms illustrate normal and abnormal findings. Arthrotomies were performed in 20 of the 32 patients in this study with a positive arthrogram-arthrotomy correlation of 85%.

A method has been developed in vitro for quantitation of aortic valvular insufficiency (Al) based on the use of ladioactive traceis lo genei-ate graphic recordings of the movement of blood out of the ventricle and back again. The regurgitant fraction of stroke volume (peirent of Al) is read from this recording. The method was evaluated in a mechanical heart model by comparing Al values read from the istope recordings and Al values determined volumetrically. The recordings gave excellent agreement with the volumetric results (Corr. Coeff. .9). These in vitro results are of such quality that clinical trials seem justified.

Although data are available indicating the complication rates for routine cardiac catheterization, information has been lacking on the morbidity of pulmonary angiography in symptomatic patients. This retrospective study was undertaken to assess the "risks" of this procedure, by analyzing all cases undergoing pulmonary angiography from June 10, 1964. to December 31, 1967. Only two deaths were considered to be causally related to the pulmonary angiogram, a mortality rate of 1.4%. The overall complication rate was 15%. There was no correlation of the complications with the average age, or the concentration of the Hypaque used. There was a correlation between complications …

A Negro woman with alcaptonuria, ochronosis, ochronotic arthropathy, diabetes mellitus and mycosis fungoides died of acute renal failure due to diabetic glomerulonephropathy and ochronotic nephrosis. The role of the renal lesions of ochronosis is presented in potentiating the effect of an underlying renal disease producing rapidly progressive kidney failure. The occurrence of ochronosis, diabetes mellitus and mycosis fungoides is apparently coincidental only. Special stains of tissue sections confirm the similarity of melanin and ochronotlc pigment.

Although several mechanisms have been proposed to explain the transport of hydrophylilc cations across lipoprotein barriers, the nature and identity of the carrier mechanism have not yet been elucidated. The present communication is concerned with a mechanism postulating that monoesterified phosphate on the inositol moiety of phosphatidyl inositol might provide anionic sites that function as cation carriers via a phosphorylation-dephosphorylation cycle, catalyzed in its simplest sequence by phosphatidyl inositol kinase and diphosphoinositide phosphomonoesterase activities. Samples of aqueous humor were obtained from the anterior chamber of patients with uveitis. Bacterial L-forms, or cell wall deficient forms, were recovered from 11 of 19 eyes with unexplained uveitis. These bacterial variants were cultured on newer, highly specialized media and stained with acridine orange. This stain demonstrates fluorescence of nucleic acids under UV microscopy.
