Nursing and Health Professions Faculty Research and Publications

Cambodia’s 100% Condom Use Programme is credited with an increase in consistent condom use in commercial sexual interactions and a decrease in HIV prevalence among female sex workers (FSWs). There has been little improvement in condom use between FSWs and non-commercial partners, prompting calls for more innovative approaches to increasing condom use in these relationships. To understand why condoms are used or not used in sexual interactions involving FSWs, we examined condom negotiation across different types of relationships. We conducted 33 in-depth interviews with young (15 to 29 years) women engaged in sex work in Phnom Penh. There was an …

Eastern Equine Encephalitis: An Assessment Of Vermont’S Treatment Plan And The Lack Of Preventative Treatment, Lauren E. Bierman Jan 2013

Eastern Equine Encephalitis: An Assessment Of Vermont’S Treatment Plan And The Lack Of Preventative Treatment, Lauren E. Bierman

Writing Across the Curriculum

When an avian host and the Culiseta melanura mosquito meet in fresh water hardwood swamps, the Eastern Equine Encephalitis virus (EEE) is spread to the bird community. When that same bird is bitten by a mosquito that feeds on humans, such as the Aedes, Coquillettidia, and Culex species, the EEE virus has found its bridge vector. EEE is a rare disease in humans; the Centers for Disease Control and Prevention (CDC) reported in 2010 that the virus affects an average of 6 people per year (Epidemiology & Geographic Distribution section, para 2). In 2010, blood collected from deer …

Epidemiology Faculty Publications

The United States has the world’s highest prison population, and an estimated one in seven HIV-positive persons in the USA passes through a correctional facility annually. Given this, it is critical to develop innovative and effective approaches to support HIV treatment and retention in care among HIV-positive individuals involved in the criminal justice (CJ) system. Information and communication technologies (ICTs), including mobile health (mHealth) interventions, may offer one component of a successful strategy for linkage/retention in care. We describe CARE+ Corrections, a randomized controlled trial (RCT) study now underway in Washington, that will evaluate the combined effect of computerized motivational …

Transmission Rate In Partial Differential Equation In Epidemic Models, Alaa Elkadry Jan 2013

Transmission Rate In Partial Differential Equation In Epidemic Models, Alaa Elkadry

Theses, Dissertations and Capstones

The rate at which susceptible individuals become infected is called the transmission rate. It is important to know this rate in order to study the spread and the effect of an infectious disease in a population. This study aims at providing an understanding of estimating the transmission rate from mathematical models representing the population dynamics of an infectious diseases using two different methods. Throughout, it is assumed that the number of infected individuals is known. In the first chapter, it includes historical background for infectious diseases and epidemic models and some terminology needed to understand the problems. Specifically, the partial …

The Feasibility Of Implementing The Acp-215 At The Dayton Community Blood Center: A Cost-Benefit Analysis, Angela Albrecht Jan 2013

The Feasibility Of Implementing The Acp-215 At The Dayton Community Blood Center: A Cost-Benefit Analysis, Angela Albrecht

Master of Public Health Program Student Publications

The community blood supply is a limited resource with a relatively short shelf-life. To ensure an adequate supply, a balance must be maintained between the numbers of donors providing blood and blood products required by hospitals to meet patient needs. Any change in the number of blood donors or in the volume of blood required by hospitals can create a blood shortage. During times of disaster, a blood shortage may be inevitable. Additional strategies must be in place to ensure availability of blood for essential transfusions across the community. Cryopreservation of red blood cells to build and maintain a blood …

Transmission Potential Of Influenza A/H7n9, February To May 2013, China, Gerardo Chowell, Lone Simonsen, Sherry Towers, Mark A. Miller, Cecile G. Viboud

Epidemiology Faculty Publications

Background

On 31 March 2013, the first human infections with the novel influenza A/H7N9 virus were reported in Eastern China. The outbreak expanded rapidly in geographic scope and size, with a total of 132 laboratory-confirmed cases reported by 3 June 2013, in 10 Chinese provinces and Taiwan. The incidence of A/H7N9 cases has stalled in recent weeks, presumably as a consequence of live bird market closures in the most heavily affected areas. Here we compare the transmission potential of influenza A/H7N9 with that of other emerging pathogens and evaluate the impact of intervention measures in an effort to guide pandemic …

A Case–Control Study Of Incident Rheumatological Conditions Following Acute Gastroenteritis During Military Deployment, Kathryn Deyoung, Mark A. Riddle, Larissa S. May, Chad K. Porter Jan 2013

A Case–Control Study Of Incident Rheumatological Conditions Following Acute Gastroenteritis During Military Deployment, Kathryn Deyoung, Mark A. Riddle, Larissa S. May, Chad K. Porter

Epidemiology Faculty Publications

Objectives The aim of this study was to assess the risk of incident rheumatological diagnoses (RD) associated with self-reported diarrhoea and vomiting during a first-time deployment to Iraq or Afghanistan. Such an association would provide evidence that RD in this population may include individuals with reactive arthritis (ReA) from deployment-related infectious gastroenteritis.

Design This case–control epidemiological study used univariate and multivariate logistic regression to compare the odds of self-reported diarrhoea/vomiting among deployed US military personnel with incident RD to the odds of diarrhoea/vomiting among a control population.

Setting We analysed health records of personnel deployed to Iraq or Afghanistan, including …

A Toolkit For Developing Bilingual Lexicons For International Hiv Prevention Clinical Trials, Catalina B. Ramirez, Natasha Mack, Barbara Friedland Jan 2013

A Toolkit For Developing Bilingual Lexicons For International Hiv Prevention Clinical Trials, Catalina B. Ramirez, Natasha Mack, Barbara Friedland

HIV and AIDS

No standardized process exists for developing a set of agreed upon terms, definitions, and translations in local languages that ensures participants understand both technical and nontechnical terms used in a clinical trial context. HIV prevention trials deal with a disease that is stigmatized and with sexual behaviors that may not be openly discussed, thus care must be taken in the selection of words to ensure terms are appropriate for use in the context of a clinical trial and understandable but not offensive. The lack of a standardized process can make it difficult for researchers to decide which words to use, …

Introduction

Men who have sex with men (MSM) in Kenya are at high risk for HIV and may experience prejudiced treatment in health settings due to stigma. An on-line computer-facilitated MSM sensitivity programme was conducted to educate healthcare workers (HCWs) about the health issues and needs of MSM patients.

Methods

Seventy-four HCWs from 49 ART-providing health facilities in the Kenyan Coast were recruited through purposive sampling to undergo a two-day MSM sensitivity training. We conducted eight focus group discussions (FGDs) with programme participants prior to and three months after completing the training programme. Discussions aimed to characterize HCWs’ challenges in …

Bloomberg’S Health Legacy: Urban Innovator Or Meddling Nanny?, Lawrence O. Gostin Jan 2013

Bloomberg’S Health Legacy: Urban Innovator Or Meddling Nanny?, Lawrence O. Gostin

Georgetown Law Faculty Publications and Other Works

Michael Bloomberg leaves the mayoralty of New York City, with his health legacy is bitterly contested. The public health community views him as an urban innovator—a rare political and business leader willing to fight for a built environment conducive to healthier, safer lifestyles. To his distractors, however, Bloomberg epitomizes a meddling nanny—an elitist dictating to largely poor and working class people about how they ought to lead their lives. His policies have sparked intense public, corporate, and political ire—critical of sweeping mayoral power to socially engineer the city and its inhabitants.

Here, I seek to show how Bloomberg has fundamentally …

Host Genetic Factors Associated With Cervical Human Papillomavirus Clearance, Staci Lynn Sudenga Jan 2013

Host Genetic Factors Associated With Cervical Human Papillomavirus Clearance, Staci Lynn Sudenga

All ETDs from UAB

This dissertation research focuses on how variants in various host genes are associated with clearance of HPV in three longitudinal cohorts from the United States. I first examined the definition of the intermediate phenotype to cervical cancer, HPV persistence and then I incorporated this phenotype in all my three aims by examining variations in xenobiotic metabolism genes in HIV-1 negative and immune-related genes in HIV-1 positive females and how these contribute to HPV infection outcomes. Several significant variants were associated with HPV clearance for the three aims. For the first aim, in ALTS assessing functional variants within xenobiotic metabolism genes …

Alternative Vaccination Schedules (Avs) : Detection Of Avs In New York State Using Immunization Registry Data, Jessica A. Nadeau Jan 2013

Alternative Vaccination Schedules (Avs) : Detection Of Avs In New York State Using Immunization Registry Data, Jessica A. Nadeau

Legacy Theses & Dissertations (2009 - 2024)

Intentional deviation from the recommended childhood vaccine schedule (RVS), referred to as an alternative vaccination schedule (AVS) is becoming increasingly prevalent in the United States. Physicians may accommodate vaccine-hesitant parents with an AVS as a compromise position to allow some vaccinations to be given, but in doing so children remain vulnerable to vaccine-preventable disease for a longer period of time. This research expands upon the nascent literature by proposing a unified definition of AVS, quantifying the prevalence of infants following an AVS in New York State (NYS) (exclusive of New York City) by geographic area and community and medical practice …

Quantitative Evaluation Of Antibiotic Exposure-Response Relationships Among Patients With Serious Staphylococcal Infections, Thomas Patrick Lodise Jan 2013

Quantitative Evaluation Of Antibiotic Exposure-Response Relationships Among Patients With Serious Staphylococcal Infections, Thomas Patrick Lodise

Legacy Theses & Dissertations (2009 - 2024)

This dissertation advances our understanding of antimicrobial exposure-response relationships among patients with serious infections due to Staphylococcus aureus. The need is pressing as treatment of this invasive pathogen has been complicated by the development of antibiotic resistance. One antibiotic resistant phenotype of particular concern is methicillin-resistant S. aureus (MRSA). Due to the multi-drug resistant nature of most MRSA strains, vancomycin emerged as the cornerstone of therapy. Although widely used for serious MRSA infections, many clinicians now question its continued utility due to the emergence of MRSA strains with reduced vancomycin susceptibility phenotypes (rVSPs). The literature is developing rapidly, but characterization …

Effect Of Data Aggregation At Two Geographic Levels On The Association Between Absms And Gonorrhea Rates In Nys, 1999-2001, Christina Michelle Gray Jan 2013

Effect Of Data Aggregation At Two Geographic Levels On The Association Between Absms And Gonorrhea Rates In Nys, 1999-2001, Christina Michelle Gray

Legacy Theses & Dissertations (2009 - 2024)

This study compared the association between residential area factors and gonorrhea, aggregated at both the census tract versus ZIP code levels. Prior research has found that census block groupings are most optimal in neighborhood-level studies, followed by census tracts and ZIP codes. Because several healthcare databases currently only provide ZIP code level data, the aim of this study was to examine whether there exists a meaningful difference between the interpretation of census tract and ZIP code aggregate data. Our study measured the association between socioeconomic factors and gonorrhea in New York State, exclusive of New York City (NYC), census tracts …

Data Mining And Pattern Discovery Using Exploratory And Visualization Methods For Large Multidimensional Datasets, Hsin-Fang Li Jan 2013

Data Mining And Pattern Discovery Using Exploratory And Visualization Methods For Large Multidimensional Datasets, Hsin-Fang Li

Theses and Dissertations--Epidemiology and Biostatistics

Oral health problems have been a major public health concern profoundly affecting people’s general health and quality of life. Given that oral health data is composed of several measurable dimensions including clinical measurements, socio-behavioral factors, genetic predispositions, self-reported assessments, and quality of life measures, strategies for analyzing multidimensional data are neither computationally straightforward nor efficient. Researchers face major challenges to identify tools that circumvent the processes of manually probing the data.

The purpose of this dissertation is to provide applications of the proposed methodology on oral health-related data that go beyond identifying risk factors from a single dimension, and to …

A Mathematical Modeling Framework For Phlebotomist Scheduling And Blood Draw Assignments In Laboratory Medicine, Laquanda T. Leaven

Dissertations

Laboratory services in healthcare delivery systems play a vital role in inpatient care. Studies have shown that laboratory data affects approximately 65% of the most critical decisions on admission, discharge, and medication. Laboratory testing accounts for approximately 10% of hospital billing. Reducing laboratory costs would contribute to reducing total healthcare cost, which is one of the major goals for the U.S. healthcare delivery system. This research focuses on improving the performance of the hospital laboratory in a large hospital system. The intention of this study is to identify and then optimize the most critical stage to improve the entire laboratory …

This analysis identified variance in intestinal parasite presentation and management by age, gender, and location. The administration frequency of antiparasitic medication was considered as prophylactic versus disease-treatment based.

Methods. This study was …

A Case For Gun Violence Prevention In Medical And Public Health Curriculum, Shaun Hamilton Jan 2013

A Case For Gun Violence Prevention In Medical And Public Health Curriculum, Shaun Hamilton

Master of Public Health Program Student Publications

Gun violence profoundly impacts the landscape of violence in the United States. The costs of gun injuries are, estimated to be $100 billion each year in the United States. Although many physicians and health professionals agree that they have a role to play in the prevention of violence (American Academy of Pediatrics [AAP], 2002), there is a distinct disconnect between perceived responsibility and actual practice (Sidelinger, Guerrero, Rodriguez-Frau, & Mirabal-Colon, 2005). Physicians inherently have a role to play in the prevention of violence. The nature of their occupation places them in the “process” of violence in the form of treatment. …
