Workplace Flexibility 2010 defines Extended Time Off (EXTO) as time taken off from work for a single reason that extends for more than five days but less than one year.

EXTO may be brief in nature (e.g., a few weeks), when taken, for example, for a vacation, to recover from minor surgery, or to comply with a public health quarantine request. EXTO may also be longer in nature (e.g., a month or more), when taken, for example, for maternity/paternity purposes, for elder care, for military duty, or for a sabbatical from work.

Nationally, hospice care and the signing of advance directives are underutilized by minority populations. Research on this phenomenon includes cultural differences, access to medical care and language barriers. A retrospective study of 1,817 closed patient charts from a local hospice was conducted to determine significant differences among the White, Black and Hispanic patient populations. The number of White patients admitted to both hospice homecare and to the hospice inpatient was significantly greater than the number of admissions of either the Black or Hispanic patients. There were no significant findings in the total number of services provided to each of the …

This Honors Capstone Project focuses on community service projects of medical students. Extensive research was done on the types of community service programs that exist in medical schools. The types of programs were divided into four different categories representing the various ways that schools conduct their community service initiatives. Schools may have multiple types of programs, and a combination of any of these programs is offered at many institutions. This project also examines nationally recognized programs and the awards received for excellence in community service initiatives.

Across Africa, the AIDS pandemic has forced HIV+ individuals and orphaned children into the care of elderly women. Though this has occurred in millions of families, little support has been offered for the grandmothers supporting children and grandchildren on a limited income. This study reports on a practicum study at Grandmothers Against Poverty and AIDS (GAPA), the first African organization to recognize the important role played by grandmothers in light of HIV/AIDS. Today, people train in academic bioethics programs and seek careers as bioethicists. Hospitals, universities, government organizations, and corporations hire bioethicists, where they use their training to help make decisions regarding life or death issues in science and medicine. This article explores the decision-making process nonprofit organizations use to determine whether to pursue merger or other forms of interorganizational restructuring. The research uses a case study design, analyzing four examples of interorganizational restructuring. The findings describe both the structure and the characteristics of the processes used in four cases. The research found that the decision-making processes used in the four cases had several core elements. Participants in restructuring customized decision-making processes to meet their needs, and power dynamics shaped those processes. Findings about the characteristics of the process emphasize the importance of communication and trust. How partners used power …

And there's very limited pressure on the system to provide high quality geriatric care. Why is that? Because the quality measures haven't been adequately developed and implemented, and it's more difficult to measure care for an older sample. This report, prepared for the Maine Hospice Council and funded by the Carpenter Foundation, presents findings of a qualitative study of barriers to hospice utilization. This study includes the perspectives of all 26 Medicare certified hospice providers in Maine. Significant findings of the qualitative study include: A continuing need to educate the general public about hospice and the Medicare benefit. Maine health care providers have low referral rates to the hospice programs and often misunderstand the regulations and guidelines of the hospice benefit. Elderly persons in long-term care settings are exceptionally vulnerable to abuse, neglect, and exploitation necessitating special protective measures by criminal justice, social services, and health care agencies. In 2006, 28.6% of Michigan households with a family member in long-term care reported that person having experienced one or more forms of abuse including physical, caretaking, verbal, emotional, neglect, sexual, and exploitation (Post, 2006). Criminal justice agencies were scrambling to identify programs aimed at reducing elder abuse in long-term care. Michigan was selected as one of seven states designated as a federal pilot test site. The purpose of this study was to investigate the controversy about shifts in the U.S. Administration's policy related to the Labor Department (OSHA) shift from enforcement to partnerships and voluntary protection programs (VPP). A metaevaluation of four OSHA VPP evaluation reports was conducted and included the investigation of objectives, methodologies, strengths, weaknesses, and areas for improvement. A program evaluation of the Nevada Child Seekers (NCS) organization was conducted from October 2006 through April 2007. The evaluation consisted of a qualitative assessment of the organization's volunteer recruitment and retention program. The evaluation was conducted by performing a series of interviews of staff and by conducting a web-based survey of available volunteers. Information gathered from the interviews and surveys was compared with benchmarking data from multiple scholarly and community sources.

While some workers' needs for lexibility can be addressed by short Term Time Off (STO) or by a Flexible Work Arrangement (FWA), there are other workers who need time off on a more episodic basis. These workers may have an illness, such as cancer of kidney disease, which requires them to attend numerous medical appointments on a relatively set basis. Or they may have a chronic conditions, such as migraine headaches or fibromyalgia, that flares up sporadically. Many countries have health-care providers who are not trained as physicians but who take on many of the diagnostic and clinical functions of medical doctors. We identified non-physician clinicians (NPCs) in 25 of 47 countries in sub-Saharan Africa, although their roles varied widely between countries. In nine countries, numbers of NPCs equalled or exceeded numbers of physicians. In general NPCs were trained with less cost than were physicians, and for only 3–4 years after secondary school. All NPCs did basic diagnosis and medical treatment, but some were trained in specialty activities such as caesarean section, ophthalmology, and anaesthesia. Many NPCs …

Since the 1990s, hospitals in the United States have had a great deal of pressure to produce high quality care at minimum expense. This study seeks to examine the impact of certification by the National Board for Professional Teaching Standards (NBPTS) on burnout levels in educators. Strong medicines classified as controlled substances by the DEA are often used to treat chronic pain conditions and are also known to be diverted to non-medical uses, thus a solution to one problem may happen at the expense o f the other. People living with HIV/AIDS in India, as elsewhere, face stigma and discrimination in a variety of contexts. Research in India has shown that stigma and discrimination against HIV-positive people and those perceived to be infected are common in hospitals and act as barriers to seeking and receiving critical treatment and care services. In a widely publicized decision issued in 2004, the United States Department of Health and Human Services removed language from the Medicare Coverage Issues Manual which stated that obesity is not an illness, a pronouncement that paves the way for Medicare coverage of evidence-based obesity treatments. This determination by HHS also has important implications for public and private insurance coverage of health care services and interventions that have the potential to reduce the risk of lifelong obesity in children.
