Incidence Of Cesarean Delivery In Nulliparous Patients Based Upon Timing Of Epidural Analgesia Initiation, Wendy H. Sandelin Aug 1996

Incidence Of Cesarean Delivery In Nulliparous Patients Based Upon Timing Of Epidural Analgesia Initiation, Wendy H. Sandelin

Theses and Dissertations

Epidural analgesia for the parturient is a safe and effective method of pain relief during labor. This method of pain management permits the pregnant woman to have a virtually pain-free labor while remaining alert and able to participate in the delivery. However, the effect of epidural analgesia on nulliparous labor, and ultimately on the mode of delivery, vaginal or cesarean, remains controversial. Cesarean delivery is associated with greater maternal morbidity and mortality, as well as increased financial cost for the health care system. Therefore, the purpose of this study was to determine the incidence of and circumstances related to cesarean …

Pediatric Intensive Care Nurses : Post-Traumatic Stress Disorder-Like Symptoms, Juanita J. Allen Jun 1996

Pediatric Intensive Care Nurses : Post-Traumatic Stress Disorder-Like Symptoms, Juanita J. Allen

Loma Linda University Electronic Theses, Dissertations & Projects

Medical personnel are not only exposed to routine pressures of demanding roles, but research reveals that they can suffer from severe stress related to exposure to critical incidents. These events can overwhelm an individual's ability for emotional adjustment leading to negative symptoms such as emotional numbing, mood changes, estrangement from friends or family, decreased ability to perform or function on the job, development of posttraumatic stress disorder (PTSD) symptoms, loss of work through attrition, and depression. Contact with seriously injured children or children who have died has been shown to constitute just such a critical incident for pre-hospital care personnel …

Explication Of The Construct Of Shared Care And The Prevention Of Pressure Ulcers In Home Health Care, Margaret Sebern Jun 1996

Explication Of The Construct Of Shared Care And The Prevention Of Pressure Ulcers In Home Health Care, Margaret Sebern

College of Nursing Faculty Research and Publications

The purpose of this investigation was to render a more complete understanding of subjective perceptions of pressure ulcers from the perspective of family dyads, and to study the effect of these subjective experiences on preventive behaviors and pressure ulcer outcomes. A naturalistic inquiry, combined with objective measures, was used. Twenty-one dyads participated in four in-depth interviews to explore how they mentally represented and responded to the risk of pressure ulcers. Through the process of concept development, a lay representation of pressure ulcers was developed. This process produced a new concept, identified as “shared care,” that explained how the dyads interaction …

A Correlative Study Of An At-Risk Population And Low Birth Weight/Infant Mortality In A Northeast Oklahoma City Community, Estacia D. Thrower May 1996

A Correlative Study Of An At-Risk Population And Low Birth Weight/Infant Mortality In A Northeast Oklahoma City Community, Estacia D. Thrower

McCabe Thesis Collection

Studies show that numerous and complex variables influence pregnancy outcomes and infant mortality rates. These variables include demographic, medical, physical, environmental, behavioral, and attitudinal factors and prenatal care. Improving the health of mothers and infants is a national challenge. In 1987, more than 3.8 million infants were born in the United States. Of these, 38,408 died before their first birthday. Although the infant mortality rate is at an all-time low, the pace of progress has slowed. According to Dr. Robert Deppen of the Oklahoma State Health Department, "In 1992, Oklahoma ranked 29th in the United States for infant mortality and …

Breast Self-Examination Teaching For Women In Chemical Dependency Programs, Michelle B. Lessard Jungwirth

Master's Projects

Fifty-two women from 5 chemical dependency programs participated in a 1 hour health education program teaching breast self-examination using breast models. Tactile skills and general information about breast cancer and breast self-examination were presented. The program was evaluated for its ability to teach this high risk population. Nine true/false questions and lump detection skills were evaluated using_a pretest/posttest non-experimental design. A level of significance for the true/false questions was set at .01, and for lump detection skills it was set at .05. Dependent t tests was used to statistically analyze the data. Participants improved their general knowledge about breast cancer …

Adolescent Tobacco Use : A Generation Up In Smoke, Kay L. Provo May 1996

Adolescent Tobacco Use : A Generation Up In Smoke, Kay L. Provo

Theses and Dissertations

Adolescent tobacco use is on the rise in the United States. It is a significant health problem. In spite of the fact that adult smoking has decreased dramatically since 1964, the number of adolescent smokers remains high and has not decreased for the last 7 years (Winkelstein, 1992). The purpose of this study was to investigate adolescent tobacco use among ninth-grade students in five rural communities. Specifically, the following questions were addressed:

1. What is the incidence of tobacco use among ninth graders in rural communities?

2. Which variables influence rural adolescents' tobacco use?

3. Where and from whom do …

Exploring The Boundaries Of Pediatric Practice: Nurse Stories Related To Relationships, Joan P. Totka May 1996

Exploring The Boundaries Of Pediatric Practice: Nurse Stories Related To Relationships, Joan P. Totka

College of Nursing Faculty Research and Publications

Pediatric nurses struggle to find the right level of involvement with children and families. The purpose of this study was to illuminate nurses' struggles and insights as they learned to find the interpersonal boundaries of their own practice. The phenomenological method of Heideggerian hermeneutics was used to analyze data from audiotaped and transcribed single interviews of five pediatric nurses. The analysis of the transcriptions involved multiple stages of interpretation using a research team. The themes that emerged from the data were: (a) pediatric nurse as family caregiver: (b) finding the right level of care: over-involvement vs. crossing the line; (c) …

Multidimensional Factors Affecting Well-Being: A Pni Based Model For Therapeutic Nursing Intervention., Mary P. Bennett

Nursing Faculty Publications

This paper is posted to assist graduate nursing students and others who are interested in my early PNI theory development work. The paper documents development and initial testing of a midrange theory of the effects of various nursing interventions on the psychoneuroimmunological (PNI) network. The hypothesized effects of interventions on various immune factors and subsequent health outcomes are explored, with special interest given to the use of PNI theory in nursing. More traditional nursing theory design principles are imbedded in this theory, including a definition of nursing and nursing intervention, as well as outlining how nurses educated at various levels …

The Development Of Community Mental Health Nursing Services In Western Australia : A History (1950-1995) And Population Profile, Phillip M. Maude

Theses: Doctorates and Masters

This descriptive• study discusses the development of community mental health (CMH) in Western Australia (WA) and describes the current practising population of CMH nurses. The study explores literature pertaining to the emergence of the CMH movement and the deinstitutionalisation of the mentally ill. A conceptual framework was developed by adapting Lewin's change theory. CMH nurses practising in WA (n= 130), were invited to participate in the study and were asked to complete a survey questionnaire. This resulted in a 66% response rate (n = 86). Quantitative data was analysed using the Statistical Package for the Social Sciences (SPSS). Open ended …

Living With Unstable Coronary Artery Disease, Marcie M. Tempel Jan 1996

Living With Unstable Coronary Artery Disease, Marcie M. Tempel

Honors Projects

Coronary artery disease (CAD) remains a leading cause of mortality and morbidity in the United States. CAD can be a long term, recurrent health problem placing a heavy burden on the afflicted individual. Although the mean age of individuals experiencing their first clinical sYmptoms of CAD is usually reported to be in the fifth or sixth decade of life, individuals may also experience sYmptoms of disease at an earlier age when actively engaged in career, family, and vigorous leisure activities. There is a sparsity of experiential information and nurses need to be sensitive to the psychosocial impact of this experience …

Social Support And Adaptation To Disability, Deborah J. Russell Jan 1996

Social Support And Adaptation To Disability, Deborah J. Russell

Masters Theses

Based on Roy's Model of Adaptation, this study examined the relationship between social support and functional independence following hip fracture. A descriptive correlational design was used with a convenience sample of 29 women age 65 and older. The Personal Resource Questionnaire was used to measure social support and the Functional Independence Measure was used to measure locomotion as an aspect of physical function.; Data indicated that women who had higher levels of functional independence one week following hip fracture had higher levels of social support. However, this difference was not statistically significant. The hypothesized relationship between social support and functional …

Basic Needs Satisfaction And Social Support In The Adult Trauma Victim, Gail A. Mercer Jan 1996

Basic Needs Satisfaction And Social Support In The Adult Trauma Victim, Gail A. Mercer

Masters Theses

Traumatic injury prevention and treatment is an increasingly important focus for the nursing profession as trauma tops the list for causes of death among young people in the United States. The specific purpose of this study was to explore whether a pre-existing state of low basic need satisfaction and low social support contribute to traumatic injury in adults. A descriptive correlational design was used. The theoretical frameworks of Erickson, Tomlin, and Swain (1983) and Lazarus (1964) formed the foundation for this study. Two survey tools were employed, the Basic Need Satisfaction Inventory (Kline Leidy, 1994) and the Personal Resource Questionnaire …

The Effect Of A Change In Patient Care Delivery On Nurse Satisfaction And Cost Of Care, Kathleen M. Allen

Masters Theses

This quasi-experimental study was designed to compare the satisfaction of nurses assigned to a medical-surgical unit on which care delivery was changed to a Dyad model (combining elements of team nursing and case management) to nurses on similar units utilizing total patient care delivery. A convenience sample o f nurses from the Dyad model unit was compared to a control group comprised of nurses from three similar medicalsurgical units. Nurse satisfaction was measured using an instrument developed by Wade and Degerhammar (1991). Cost per case and length of stay on selected diagnoses, and labor cost per patient were compared across …

An Evaluation Of Needleless Intravenous Therapy Devices, Tina Jones Jan 1996

An Evaluation Of Needleless Intravenous Therapy Devices, Tina Jones

Theses

As the number of persons infected with bloodborne diseases continues to increase, it is paramount that all health care workers exhibit behaviors reflecting unerring compliance with universal precaution.

The majority of occupational exposures to potentially infectious blood and body fluids occur via needlestick injury. The morbidity and mortality from these exposures is significant. A review of research examining work practices mandated by the Bloodborne Pathogen Standard challenges their effectiveness. Principles identified provide insight into why work practice may not prevent needlesticks.

Several industries are developing new products to address this situation, but there are few data on how well these …
