Hospitals are challenged to understand factors related to staff nurse retention in the context of the nursing shortage slowdown (Huselid, 1995; Mancino, 2008; Mancino, 2009). Leadership is a major factor in organizational success (Gandossy & Guarnieri, 2008; Gettler, 2003; Wooten & Crane, 2003) and nurse managers are the direct link between the executive nurse leaders and the registered nurses (RN) in the staff position (Taunton, Boyle, Woods, Hansen, & Bott, 1997).

Despite recent advances in healthcare, Streptococcus pneumonia-associated infections contribute to significant morbidity and mortality in older adults. Because of the pneumococcal vaccine’s efficacy for preventing invasive disease, guidelines for vaccinating older adults have been developed; yet, the nation has not met immunization goals. The purpose of this EBP project was to determine if a telephone reminder, as compared to provider recommendation, would increase pneumococcal vaccination rates. The Iowa model and Kotter’s eight stages of change guided this project within a multi-provider office in Northwest Indiana. Interventions To Improve The Management Of Medically Uninsured Adult Patients With Type 2 Diabetes Mellitus In Primary Care, Community-Based Settings, Lynn Miskovich

This project measured the effect of a clinical algorithm on the provision of care to medically uninsured adult patients with type 2 diabetes and a low socioeconomic status (SES). Primary providers often fail to implement established standards for diabetes care to their maximal benefit and do not achieve American Diabetes Association (ADA) treatment standards. Saydah, et al. (2004) reported that only 48% of patients with diabetes achieved the recommended HbA1c goal, and 33% reached blood pressure and LDL targets. Goals for all three clinical parameters were obtained by only 7% of patients. The Stetler Model of Evidence-based Practice (EBP) provided …

PROBLEM.  Using theories from several disciplines, the concept of organizational culture remains controversial. Conflicting definitions, lack of semantic clarity, and debate over the most appropriate methods for assessing organizational culture have led to disagreement over the value and validity of such inquiry.

METHODS.  This paper reviews development of the concept of organizational culture and methods for assessing organizational culture, focusing on the healthcare environment.

FINDINGS.  Most work on organizational culture concerns the traditional corporation. Therefore, some adaptation to the central goals and focus of a human services organization are necessary before application to healthcare settings.

Objective: This 3-year field experiment engaged 60 nurses and 282 patients in the design and evaluation of an innovative home-care nursing model, referred to as technology-enhanced practice (TEP).

Methods: Nurses using TEP augmented the usual care with a web-based resource (HeartCareII) that provided patients with self-management information, self-monitoring tools, and messaging services.

Results: Patients exposed to TEP demonstrated better quality of life and self-management of chronic heart disease during the first 4 weeks, and were no more likely than patients in usual care to make unplanned visits to a clinician or hospital. This study investigated whether providing counselors as care managers to individuals who were having difficulty managing their Type II diabetes improved their self-management, health, and functioning. Counselors used the Motivational Interviewing Model and met weekly over a six month period with patients who had Type II diabetes who had been identified as not functioning well in managing their disease. The success of the intervention was determined by a number of measures including physical symptoms of patients; patients' perceptions of the care they received; and patients' self-efficacy in managing their diabetes. Stroke patients initially experience dysphagia approximately 42 - 67% of the time with noted improvement reducing rates to approximately 43% at seven days post-acute event (Perry & Love, 2001). The dysphagia experienced by these patients makes them at high risk to develop aspiration pneumonia. Overall, the financial impact of reducing aspiration pneumonia reduces the national cost of morbidity, mortality, and disability burden in this population. Assisting smokers to quit and stay quit is the most important intervention clinicians can undertake to improve the length and quality of life of patients who use tobacco. The chronic, relapsing nature of tobacco dependence complicates tobacco treatment. Tobacco treatment counseling programs provide on-going support to help patients avoid relapse. Assistance with a referral increases the likelihood that patients will participate in counseling, but few clinicians regularly assist with referrals to tobacco treatment programs. This dissertation examined health system processes and clinician attitudes that influence the likelihood that clinicians will refer their patients for tobacco treatment counseling.

A hip fracture is a devastating injury for an older person, often resulting in long-term or permanent loss of mobility and independence. Although research has been conducted on various aspects of hip fractures, little research has described the experience of a patient with a hip fracture. This phenomenological study explored the lived experience of a patient receiving treatment on an orthopedic unit after a hip fracture. A purposeful sample of two participants who were hospitalized at Saint Mary's Hospital following a hip fracture were interviewed after surgical repair of the fracture. The purpose of this study was to investigate the relationship between leadership styles and organizational culture within schools of nursing. A non-probability convenience sample (N=149) of subjects was obtained from BSN schools of nursing within the Southern Regional Education Board geographical area. Three data collection tools were utilized including a demographic questionnaire; the Multi-factor Leadership Questionnaire (Bass & Avolio) which measures transformational, transactional, and laissez-faire leadership styles; and the Organizational Culture Assessment Inventory (Cameron & Quinn) which measures four culture types including clan, market, adhocracy and hierarchy cultures. This descriptive study assessed the perceptions of the work environment and the retention plans among registered nurses (RNs) who worked in the Intermediate Intensive Care Unit at a tertiary university hospital. Results from the Moos Work Environment Scale found that nurses perceived high levels of work pressure and low levels of clarity, coworker cohesion, and supervisor support. Results indicated that the intent to leave the unit was dependent on the nurses having other options in nursing, desiring further education, an undesirable scheduling process and unsafe patient care. Examines the contextual effects of social persuasion (represented by self, client, peer and supervisor's feedback) and mastery experiences (represented by formal level of education and work experience) on specific self-efficacy outcomes and perceived advancement potential in a sample population of nurses at a Southern California hospital. Results of the study suggest that self, client, peer, and supervisor's feedback consistently predict significant self-efficacy outcomes.

It is vital to recognize the need for a paradigm shift in nursing that utilizes new informatics tools required for optimum use of evidence related to the delivery of quality nursing care.

To address the shortage of qualified candidates interested in academic administration, this study explored factors related to recruitment of nursing academic administrators, including leadership practices of current administrators, career aspirations of potential administrators, and perceptions of both groups toward a career in academic administration.

Method. Nursing academic administrators and full-time faculty from randomly selected National League for Nursing Accreditation Commission (NLNAC)-accredited nursing programs in private colleges or universities in the United States participated in the study. The purpose of this study was to examine the relationship between leadership styles of nurse managers and job satisfaction of registered staff nurses in hospital settings in an Appalachian state. Names and addresses of 7,190 registered staff nurses were obtained from the state Board of Examiners for Registered Professional Nurses and 200 subjects were selected using simple random sampling. The Multifactor Leadership Questionnaire was used to measure leadership style of nurse managers as perceived by staff nurses. Purpose: The purpose of this study was to examine the relationships between domestic violence education, organizational support, screening, danger assessment, safety planning, and referral practices among professional registered nurses in two ED settings.

Sample & Setting: A convenience sample of 63 professional registered nurses provided data from two EDs. An ED located in Kentucky where state mandated continuing education in domestic violence was required (n = 33) and an ED in West Virginia (n = 30) where mandated continuing education in domestic violence was not required was the setting for the study.
