Abstract

This evidence-based practice Process Improvement (PI) project involves using the COPD Assessment Test (CAT) to decrease rehospitalization rates for home health patients with a diagnosis of Chronic Obstructive Pulmonary Disease (COPD). The project includes having therapy clinicians complete the CAT during their in-home skilled visits, between home health nursing visits to increase the frequency of completion. The goal is to recognize symptoms of exacerbation early, engage patients in their own care, encourage communication with providers, initiate early intervention at home, and prevent rehospitalization. Background: Traumatic injuries in the geriatric (> 65) patient population can have devastating effects. Only a small percentage of these patients return to independent ambulation and living. As the population continues to age and life expectancy continues to increase, the incidence of traumatic injuries in geriatric patients will continue to increase. Geriatric friendly hospital wards in several disciplines have been shown to reduce length of stay and improve patient outcomes. Significance and Background: Obesity is a multifactorial disease resulting in multiple health complications. Telehealth is a strategic method incorporating obesity management into primary care (PC). External evidence supports the use of telehealth for obesity management, as it can increase access to care and promote weight loss. The identified benefits of telehealth include convenience, efficiency, communication, privacy, comfort, increased patient satisfaction, and minimized cost. Primary care providers (PCPs) need designated time with patients to discuss obesity and weight loss in addition to the typical patient encounter.

Incarcerated individuals have a higher incidence of type 2 diabetes (T2D) than the general population and face unique challenges in managing their condition. These include limited access to nutritious food, disruptions in the healthcare continuity, and socioeconomic barriers such as unstable housing and unemployment. Upon release these challenges can compromise glycemic control.

The Transitions Clinic Model supports formerly incarcerated individuals in managing chronic conditions, including T2D during their reintegration. This thesis aims to explore how racial health disparities, underrepresentation in gynecology, and socioeconomic barriers contribute to adverse pregnancy outcomes among women of color in New York City. The increase in off-label Glucagon-like peptide-1 (GLP-1) receptor agonist drugs, such as semaglutide (Ozempic), for weight loss has sparked concern in regards to the not fully understood drug outcomes and safety. This paper examines the impact of a culturally centered cognitive behavioral therapy (CBT) and postpartum depression (PPD) education on reducing PPD which will prevent maternal suicide among Asian American mothers. There is stigma around PPD among Asian American mothers, and there’s a lack of research on culturally sensitive interventions. This quality improvement (QI) project investigated female collegiate athletes’ knowledge and prevalence of Relative Energy Deficiency in Sport (REDs) and the Female Athlete Triad (Triad). The increase in female athletics brings a growing concern for performance-related health risks, including low energy availability (LEA), menstrual dysfunction, and low bone mineral density (BMD). This study assessed the effectiveness of education and self-screening in identifying at-risk athletes and encouraging resource application for intervention. การวิจัยนี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดผลก่อนและหลังการทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบ 1) พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทก่อนและหลังได้รับโปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษา และ 2) พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทระหว่างกลุ่มที่ได้รับโปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษากับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างเป็นผู้ป่วยจิตเภทมีคุณสมบัติตามเกณฑ์ที่กำหนดและเข้ารับการรักษาแผนกผู้ป่วยในระยะคงสภาพเตรียมจำหน่ายที่โรงพยาบาลพระศรีมหาโพธิ์ จำนวน 40 คน ได้รับการจับคู่ให้ผู้ป่วยจิตเภทมีคุณลักษณะที่ใกล้เคียงกันในเรื่องเพศและระยะเวลาการเจ็บป่วยแล้วถูกสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คน กลุ่มทดลองได้รับโปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษาและกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) โปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษา 2) แบบประเมินพฤติกรรมการใช้ยาตามเกณฑ์การรักษา และ 3) แบบประเมินการหยั่งรู้เกี่ยวกับการรักษา โดยเครื่องมือทุกชุดผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 คน เครื่องมือชุดที่ 2 และ 3 มีค่าดัชนีความตรงเชิงเนื้อหาเท่ากับ .81 และ .88 ตามลำดับ และมีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .82 และ .81 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติทดสอบที ผลการวิจัยสรุปได้ ดังนี้ 1. พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทหลังได้รับโปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษาสูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทกลุ่มที่ได้รับโปรแกรมเพิ่มการหยั่งรู้เกี่ยวกับการรักษาสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยนี้เป็นการศึกษาแบบความสัมพันธ์เชิงทำนาย มีวัตถุประสงค์เพื่อศึกษาความร่วมมือในการรับประทานอาหารและอำนาจทำนายของปัจจัย อายุ ความรู้ด้านโภชนาการ ระยะเวลาเป็นโรคไตเรื้อรัง ระยะเวลารักษาโดยการฟอกเลือด สถานะเศรษฐกิจและสังคม การสนับสนุนทางสังคม และสัมพันธภาพกับบุคลากรสุขภาพ ในผู้ที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม กลุ่มตัวอย่างประกอบด้วยผู้ที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมจำนวน 121 ราย ซึ่งมารับบริการที่โรงพยาบาลระดับตติยภูมิ 3 แห่งในกรุงเทพมหานคร คัดเลือกแบบเฉพาะเจาะจงตามเกณฑ์คัดเข้า เก็บรวบรวมข้อมูลด้วยแบบสอบถาม และวิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสถิติถดถอยพหุคูณแบบ stepwise ผลการวิจัยพบว่า ความร่วมมือในการรับประทานอาหารโดยรวมอยู่ในระดับปานกลาง (x̅ = 52.76, SD = 14.67) ปัจจัยที่ทำนายได้ดีที่สุดคือการสนับสนุนทางสังคม สามารถอธิบายความแปรปรวนได้ร้อยละ 10.6 (R² = .106, p< .001) เมื่อเพิ่มปัจจัยสัมพันธภาพกับบุคลากรสุขภาพ สามารถอธิบาย เพิ่มเติมได้ร้อยละ 10.1 (R² Change = .101) ระยะเวลาการฟอกเลือดอธิบายเพิ่มขึ้นร้อยละ 7 (R² Change = .071) ระยะเวลาเป็นโรคไตเรื้อรังอธิบายเพิ่มขึ้นร้อยละ 3.7 (R² Change = .037) อายุอธิบาย เพิ่มขึ้นร้อยละ 2.9 (R² Change = .029) ความรู้ด้านโภชนาการอธิบายเพิ่มขึ้นร้อยละ 3.1 (R² Change = .031) และสถานะทางเศรษฐกิจและสังคมอธิบายเพิ่มขึ้นร้อยละ 3.1 (R² Change = .031) รวมกัน สามารถอธิบายความแปรปรวนได้ทั้งหมดร้อยละ 40.6 (R² = .406, F = 5.971, p < .05) โดยพบว่า การ สนับสนุนทางสังคมเป็นปัจจัยทำนายที่สำคัญที่สุด (β = .270, p < .01) รองลงมาคือสัมพันธภาพกับ บุคลากรสุขภาพ (β = .269, p < .01) ผลการศึกษานี้สามารถนำไปใช้เป็นแนวทางสำหรับผู้ให้บริการด้านสุขภาพรวมถึงพยาบาลใน การพัฒนาโปรแกรมส่งเสริมความร่วมมือในการรับประทานอาหารของผู้ป่วยที่ได้รับการฟอกเลือดด้วย เครื่องไตเทียม โดยมุ่งเน้นการเสริมสร้างการสนับสนุนทางสังคม และส่งเสริมสัมพันธภาพที่ดีระหว่าง ผู้ป่วยกับบุคลากรสุขภาพ เพื่อเพิ่มประสิทธิภาพการดูแลรักษาโดยรวมต่อไป

การวิจัยเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อศึกษาสมรรถนะตามบันไดวิชาชีพของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก โรงพยาบาลตติยภูมิ โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือ กลุ่มผู้เชี่ยวชาญจำนวน 20 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ 1) สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับสมรรถนะตามบันไดวิชาชีพของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก 2) วิเคราะห์ข้อมูล และสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประเมินระดับความสำคัญ และ3) คำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์เพื่อสรุปผล ผลการวิจัยพบว่า สมรรถนะของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก โรงพยาบาลตติยภูมิ ประกอบด้วย 9 ด้าน รวมจำนวน 143 ข้อรายการสมรรถนะ ดังนี้ 1) ด้านการพยาบาลผู้ป่วยแผลไฟไหม้น้ำร้อนลวกในภาวะวิกฤต จำนวน 24 ข้อ 2) ด้านการควบคุมการติดเชื้อและภาวะแทรกซ้อน จำนวน 14 ข้อ 3) ด้านการดูแลบาดแผลไฟไหม้น้ำร้อนลวก จำนวน 18 ข้อ 4) ด้านการบริหารจัดการความปวดจำนวน 17 ข้อ 5) ด้านการฟื้นฟูสภาพร่างกายและจิตใจ จำนวน 12 ข้อ 6) ด้านการจัดการโภชนาการ จำนวน 9 ข้อ 7) ด้านการวางแผนจำหน่ายและการดูแลระยะท้าย จำนวน 17 ข้อ 8) การสื่อสารและการทำงานร่วมกับทีมสหวิชาชีพ จำนวน 17 ข้อ และ9) การใช้เทคโนโลยีและอุปกรณ์ทางการแพทย์ จำนวน 15 ข้อ ซึ่งพยาบาลที่ปฏิบัติงานในหอผู้ป่วยแต่ละระดับมีสมรรถนะทั้ง 9 ด้าน แต่สมรรถนะย่อยมีความแตกต่างกัน การศึกษานี้สามารถใช้เป็นแนวทางในการประเมินสมรรถนะและมอบหมายงานตามความสามารถของพยาบาลแต่ละระดับ

Women’s health decision-making autonomy is fundamental for the health and well-being of women and their children. It empowers women to make health decisions and exercise their rights and choices surrounding their health. Like most parts of Africa, women’s autonomy in Tanzania remains contentious, with an estimated 19% prevalence of health decision-making autonomy in 2015. Necrotizing enterocolitis (NEC) is one of the leading causes of death in infants < 32 weeks gestation and/or < 1500g. Human breast milk lowers NEC rates through immunologic, microbial, and gut-protective mechanisms. When maternal or donor milk is unavailable, infants require formula with fortification but the safest type of fortifiers remains unclear. Bovine-derived fortifiers may increase gut inflammation; human milk-derived fortifiers may offer more protection. Limited research exists comparing these fortifiers when infants are exclusively formula fed. Determining if human milk-derived fortifiers still reduce NEC risk could influence NICU feeding guidelines and improve outcomes.

Primary care frequently misdiagnoses prediabetes, which serves as a diabetes type 2 precursor, resulting in failed early treatment opportunities. A structured staff education program was the goal of the quality improvement project during the DNP implementation. The initiative addressed knowledge gaps alongside screening protocol standardization all to enhance early detection as well as intervention efforts. Using both Lewin’s Theory of Change and Kolb’s Theory of Experiential Learning, a pre-post-design was applied across seven weeks.
